Stacy Hoover Contracting Officer Amendment of Solicitation Amendment 0006 Solicitation 36C25221B0033 Security Corrections Project# 537-19-904 Jesse Brown VA Medical Center, Chicago, IL.

This amendment consists of:

1. Amendment provides VA responses to Questions/Requests for Information (RFI) submitted by potential bidders.

The following changes are made on project specifications:

REQUEST FOR INFORMATION (RFI) RESPONSES:

RFI No. 59 SOW: Aluminum Doors Fire Ratings Q59: Door schedule generally states to “Replace door. Frames to remain.” at Aluminum Door Locations, and all doors require 90min fire rating. If the fire rating is required, then CTR should replace the frame and associated system as well as the door. If the fire rating is not required, then CTR may be able to replace the door only, pending the current condition of the frame. The Option requiring the fire rating and new door & frame (&system) will incur a substantially greater cost.

A59: Where listed in construction documents, fire rated doors are to be provided. Frames are not to be provided unless stated.

Per VHA-Joint Commission Clarification Document (1-May-2021): "Labeled fire doors installed in locations where life safety drawings indicate that a fire door is not required do not need to comply with requirements of NFPA 80. " - Frames and fire-rating certification not required in these locations.

Reference solicitation attachment 4 'Exhibit 1_537-19-904_Jesse_Brown_SOC'

RFI No. 60 SOW: A601 - Doors Q60: AMD 004 states that keynote 8 on the door schedule is to be deleted. Please confirm that comment applies only to doors 18, 25, and 26 and that no work is expected at doors #01, 03, 05, 08 within this Contract.

A60: Door 01, 03, 05, 08 are NOT in scope (they do not appear on floorplans).

Door 25 & 26 are in scope.

Door 18 (police observation room 1438) should be included in the base bid, but not Deductive Alternative 1

RFI No. 61 Card Reader Coordination Q61: Is installation of card readers expected after this contract is complete? Or should Contractor expect to coordinate with card reader installation during this project?

A61: Yes. COR will bring all stakeholders together and all coordination between projects will take place, as needed, during construction progress meetings.
