This amendment consists of:
1. Amendment provides VA responses to Questions/Requests for Information (RFI) submitted by contractors, updated SOW, revised bid schedule, adds VHA Supplemental Contract Requirements for Combating COVID-19 and updated Wage Determination.
2. Project Bid Opening is hereby EXTENDED TO 01/05/2022 at 1:00pm Local Time FROM 12/23/2021 1:00pm Local Time.

REQUEST FOR INFORMATION (RFI) RESPONSES:
RFI No. 1 SOW: Personnel Qualifications Q1: The requirements for the PM and Superintendent certifications appear excessive, are costly and difficult to obtain. Can industry experience be substituted for the certification requirements?
A1: This requirement has been removed.
RFI No. 2 SOW: Personnel Qualifications Q2: Does the CHC/NCCER certified Superintendent have to be dedicated full time on this one project, or can he/she work on multiple projects as the Superintendent?
A2: At all times during performance of this contract and until the work is completed and accepted, the Contractor shall have a competent superintendent who is satisfactory to the Contracting Officer and has authority to act for the Contractor. The Superintendent assigned to this Contract cannot superintend other Contract's worksites when the crews performing this Contract are on the worksite.
RFI No. 3 01 35 26 - Safety Q3: 01 35 26 – 13 1.7C: “However, the SSHO has be a separate qualified individual from the Prime Contractor’s Superintendent and/or Quality Control Manager with duties only as the SSHO.” Please confirm 3 separate full-time employees will need to be onsite during periods where construction activity is in progress.
A3: Superintendent can also fulfill SSHO duties only when they meet the qualification requirements of the SSHO as listed in the specifications. QC duties cannot be fulfilled by the Superintendent. QC duties cannot be fulfilled by the SSHO.
RFI No. 4 Abatement Q4: Please provide specification for Asbestos and list all applicable quantities required to be abated as a part of this solicitation.
A4: Based on previous survey, Asbestos abatement is not expected to be necessary as a result of construction and is not part of the scope of this contract.
RFI No. 5 01 35 26 - Safety Q5: As prime contractor, we are aware of being responsible for temporary interior signage at door locations on which we are working. However, this project will require notifications and regular updates to the Life Safety Plan due to changing ingress/egress. Who is responsible for these notifications to patients and staff?
A5: Prime Contractor is responsible to provide advance notice and coordinate with the COR when work will impact ingress/egress. Additionally, contractor is responsible to post signage in the corridors (not just door location), including floors above/below, impacted by construction. Contractor is not responsible to notify VA staff or patients (besides COR) directly.
RFI No. 6 01 00 00 1.3 Statement of Bid Items Q6: Solicitation Bid Schedule and Statement of Bid Items Specification conflict in Price Schedule for this Contract. Please confirm if Bid is to be for Base Bid Only (per solicitation), with Alternatives (per Specifications), or if a different bid schedule is required.
A6: Revised solicitation schedule including deductive alternates to be provided from CO. Please see RFI 7 & 8 regarding alternatives.
RFI No. 7 10 26 41 Bullet Resistant Fiberglass Panels Q7: Please confirm the above Drawing Sheets and Spec is NOT included in this contract as it relates to the Police Observation Room (Rm1438, door 18) A7: Deductive Alternate 1: Provide alternate where police observation room 1438 is not included in the scope of construction. For deductive alternate 1, specifications relating exclusively to the police observation room (i.e. 10 26 41) shall not be included.
RFI No. 8 Security CameraRef Sheet: 1-SC101E KN2 Q8: Given that the work at the Police Observation Room is removed from scope, is the camera outside of the Police Observation Room still required within this contract or is it removed as well?
A8: See RFI 7: The camera shall be included in the base bid. The camera is part of the police ops room and therefore shall be removed in Deductive Alternate 1 bid.
RFI No. 9 Sheet: 40-SC102 KN1 Q9: Is a new exit sign required at this location – police observation room?
A9: Existing exit sign to remain.
RFI No. 10 Electrical / Security As-Builts Q10: Can the VA provide Electrical or Security As-Builts to show what devices and/or wiring are existing? This may have a drastic cost/schedule impact based on the amount (time/quantity) of work required per each door.
A10: Existing door hardware and new hardware are listed in the construction documents.
RFI No. 11 A601 Q11: Door Schedule on A601 has a note on door #25 and #26 in building 32 “deleted from scope” (note 8). These doors are found elsewhere in the drawings (sheet 32-G101, 32-A101, 32-E101, & 32-SC101).
Please confirm these doors are no longer included in the scope of this project.
A11: Footnote 8 should be deleted. 25 & 26 still in scope.
RFI No. 12 08 71 00 Q12: Please confirm that it is acceptable to surface mount the required electrical/hardware due to exterior walls being brick (outside) and masonry (inside).
A12: See RFI 56: Yes, hardware can be surface mounted on brick (exterior) and masonry (interior) walls as needed. Hardware shall be mounted above the ceiling where drop ceilings exist.
RFI No. 13 Sheet: SC002Spec: 28 13 00 Q13: Sheet SC002 notes that Card Readers are to be provided by others. Please confirm what, if any, work is to be completed by Contractor (Supply/Install/Wiring/Connections) in regard to the Card Readers.
A13: Confirmed - Contractor to provide door hardware listed in contract documents and wire connections from door junction box above the ceiling and from junction box to card reader if present. Otherwise, contractor shall install wires and junction box above the ceiling and terminate wiring in junction box.
RFI No. 14 Sheet: SC002Spec: 28 13 01 Q14: Sheet SC002 notes that LA1 is to be provided by others. Please confirm what, if any, work is to be completed by Contractor (Supply/Install/Wiring/Connections) in regard to the Local Alarm Units.
A14: Provide rough-in as indicated on the drawings.
RFI No. 15 08 71 00 Q15: Please clarify Automatic Door Operator Requirements. Specifically, there are differences between the Hardware Schedule and Integrated Door Assembly schedule as well as between notes of “ADO” and “Automatic Door Operators and Controls by Section 08 71 13, Automatic Door Operators.”
A15: More specifics regarding ADO differences needed. A detailed answer can not be provided based on the RFI.
RFI No. 16 Sheet: A601 / SC002Spec: 08 71 13 Q16: The door schedule notes under SEC EQPT “AD” which is a Kouba Automatic Door Controller. Is the contractor expected to reuse existing Automatic Door Operators? If so, could the VA provide equipment specifications and sequence of operations for equipment to be reused? If the Contractor is expected to install new, please confirm new locations and if a specific make/model is required.
A16: Provide new hardware, including automatic door operators, unless otherwise directed by VA. The Kouba controller is an interface device for interfacing the security system to the automatic door operator (refer to security drawings).
RFI No. 17 Infection Control Q17: Please confirm that an Edge guard infection control barrier is an acceptable means for partitioning off the work areas. This system is not 1-hr rated, however plan page G003 – details 2 and 3 show modular type Edgeguard system for infection control purposes.
A17: Contractor to provide shop drawings for review by VA Fire Safety. System presented as basis of design, not specifically "Edge Guard" system. Must be 1 hour rated as detailed.
RFI No. 18 Phasing Q18: Please confirm how many openings prime contractors will be allowed at (1) time. Can locations be worked on concurrently if there is no impact to patient and staff egress?
A18: Provided doors do not conflict and impact egress, refer to Phasing & Life Safety Drawing notes, then multiple doors can be worked on simultaneously. VA must approve work locations in advance, and ontractor is expected to provide enough personnel to complete construction in a timely manner.
RFI No. 19 On site Storage Q19: Please confirm that no storage will be available on site for the project? There are significant costs for material storage if needing to be done off site and delivered for the night’s worth of work.
A19: Per SOW Part 6. Section 5. b. "No on-site storage or storage trailer is allowed." This is a hospital, not a storage facility, minimal space is available. Only material immediately scheduled for use (<1 week) will be accepted on-site. Contractor must make arrangements for off-site storage and schedule deliveries appropriately.
RFI No. 20 Work hours Q20: Per the SOW provided, it states work hours to be 5:00pm –5:00AM. Does this mean that ALL contractual work is to be performed on off hours, or can prep-work with no impact to the facility be done during the day?
A20: Regularly scheduled hours for this project will be nights. Prep-work and coordination, which does not impact the facility may be performed during the day.
RFI No. 21 Infection Control Q21: Please confirm locations that will need a temporary dust control barrier (detail 1 on G003) versus a temporary infection control barrier (detail 2 and 3 on G003).
A21: Locations shown on the drawings.
RFI No. 22 Temporary Security Q22: The duration of each door installation from demolition to completion could be 5 to 7 days can the VA define the temporary wall/door configuration, temp door hardware, temp. cameras, temporary card readers, Temp construction cores, NFPA 101 egress issues?
A22: Life Safety / Egress conflicts are detailed in the notes of Phasing & Life Safety drawings (e.g. 1-G101E). Temporary cameras and card access not required. Construction barrier requirements are listed in drawings.
RFI No. 23 Door Hardware Q23: Is there a schedule of finishes? Called out in automatic door operator and door hardware. Should be spec section 09 06 00 A23: Finishes are called out in individual specs. See hardware spec, and drawings for door & frame finishes.
RFI No. 24 Door Hardware Q24: Door Hardware calls out for card readers. During the walk through it was discussed only rough in the electrical and the rest is on a separate contract. Please advise A24: Hardware indicates card readers associate, but the card readers will be provided by VA in another project, as indicated in the documents.
RFI No. 25 Door Hardware Q25: Door Hardware 08 71 00 calls out automatic door operators as ADO on hardware sets HW-SH 4A, HW-SH-4B and HW-SH-10A. None of these hardware sets are called out in drawing A601. Do existing operators stay and are there no new auto operators? Please advise A25: If not in door schedule, not required.
RFI No. 26 28 05 00 - Maintenance Q26: 28 05 00 Maintenance & Service 1.9.F.1. (first of a number of sections) requires a 4 hour onsite service requirement. Doesn’t this section require then a local security integrator certified in Avigilon & Lenel (existing and required systems currently in application)?
A26: Certifications in the VA systems (Lenel and Avigilon) are not required since the scope of work does not require work on the respective systems (provided by VA under separate project).
RFI No. 27 28 13 00 Q27: 28 13 00 Summary C.2, C.3, & C.4 distinguish between “replacing” access control equipment at doors and “providing” new access control equipment at doors previously not access controlled. In the latter “future connection” is ‘by owner”. Is this to say only the components at the new door are to be provided and no Lenel hardware or tech labor to bring these into the system? “Future connection” might also be interpreted as no cabling from junction box to panel. Is this the case?
A27: Correct - scope requires provision of rough-in and wiring to junction boxes above ceiling/doors, as indicated on the drawings. The connections to the panels from the above ceiling junction boxes will be provided by the VA under a separate project.
RFI No. 28 28 13 00 Q28: 28 13 00 1.4 Permits. Is this to say any maglock permitting is to be submitted and provided?
A28: Electric lock permitting with the AHJ and any other permitting shall be provided as required by the AHJ.
RFI No. 29 Product Details Q29: Part 2 Products 2.2.C.1. Card readers are defined as HID iClass SE series but doesn’t the VA now require piv class readers to comply with requirements of FIPS 201-2?
A29: Card reader models shall be determined by the VA and provided under a separate project, as indicated in the documents.

Q30: The SOW has a reference to “Nurse call system to be compatible with xxx. Can the VA provide what the “XXX” should read?
A30: That is a hold-over and should have been removed from the SOW. Nurse Call system is not within the scope of this project and should not be impacted by door replacement.
RFI No. 31

Q31: The Project Manager Qualifications: The project manager must be an American Hospital Association Certified Healthcare Constructor. Likewise, the Superintendent Qualifications: The project superintendent must be a National Center for Construction Education & Research Certified Construction Superintendent or be an American Hospital Association Certified Healthcare Constructor. Can the VA confirm if this is a requirement for both positions?
A31: This requirement has been removed - See Response RFI 1.
RFI No. 32 Price Adj Q32: There is reasonable doubt regarding the stability of certain conditions over the extended period of this project, such as the Market stability, Supply chain and Labor conditions. Given that we are to hold our bid for 90 days beyond the bid due date, will the VA consider including provisions for economic price adjustment affected by fluctuations in the economy?
Q32: The contract is firm-fixed price. As such, Contract award to be based on pricing at bid date. VA will not provide considerations.
RFI No. 33 01 45 00 Q33: Can the VA confirm that Safety and Health Manager, CQC System Manager are two independent positions and cannot have dual roles?
A33: QC duties cannot be performed by the SSHO. See RFI 3
RFI No. 34 01 45 00 Q34: Can the VA confirm the CQC System Manager and Alternate CQC System Manager are required to have completed the Construction Quality Management (CQM) for Construction course?
A34: See RFI 2
RFI No. 35 01 00 00 Q35: This para reflects 3 bid alternates. Can the VA confirm if these alternates are applicable to this project?
A35: See revised solicitation schedule. Deductive Alternate 1: remove police observation room 1438.
RFI No. 36 01 32 16.15 Q36: This section states “The Contractor shall cost load all work activities/events except procurement activities.” Can the VA confirm if the schedule has to be cost loaded?
A36: Contractor to provide schedule of values.
RFI No. 37 01 33 23 Q37: This section states “The VA will provide the initial submittal register in electronic format.” Can the VA provide this prior to bid?
A37: Submittals will be provide via email. Documents too large to send via email, can be sent on Compact Disc, or through a MS Teams share-site.
RFI No. 38 1 00 00-1.6 Q38: Can the VA confirm that that the General Contractor will have a designated storage area and location for a job trailer?
A38: See RFI 19: Per SOW Part 6. Section 5. b. "No on-site storage or storage trailer is allowed."
RFI No. 39 10 26 41 Q39: Can the VA confirm this section is not applicable and no Bullet Resistant Panels are required along with any scope identified on sheet A401 for room 1438?
A39: See RFI 7: Contractor to provide Deductive Alternate 1 (in addition to base-bid). Deductive Alternate 1 removes the police observation room 1438 from scope of construction, as such specifications relating exclusively to the police observation room (i.e. 10 26 41) should not be included in Deductive Alternate 1.
RFI No. 40 Police Ob Room Q40: Can the VA clarify if this door type is “C” or “B” as the door schedule has the glass as “NV” which is narrow lite?
A40: Reference door schedule A601 for details on doors. NV refers to narrow vision.
RFI No. 41 STORAGE 1T18 (Door #25) Q41: Can the VA clarify if this door lite is “HG” or “FG” as the door schedule has the glass as “HG” but the door type “EE”?
A41: Door 25 is HG (half glass)
RFI No. 42 Art Therapy 1T15A Q42: Can the VA clarify if this door lite is “SQ” or “FG” as the door schedule has the glass as “SQ” but the door type “EE”?

A44: Door 50 is a single leaf door and requires only one (1) E2 and AD.
RFI No. 45 Infection Control Q45: The infection control details reflect prefabricated adjustable panel system. Can the VA confirm that this is the only acceptable method or are other construction methods allowed?
A45: Contractor to provide shop drawings for review by VA Fire Safety. System presented as basis of design, not specifically "Edge Guard" system. Must be 1 hour rated as detailed.

Q46: The way the SOW is laid out, it implies getting a set of doors replaced at one location and then getting them approved by the VA COR before starting the next door(s). Can multiple door openings be completed simultaneously (i.e. like an exterior door and an interior door be replaced by multiple crews simultaneously during an evening or weekend)?

RFI No. 47 Building 40 Q47: We understand that floors 4 thru 6 are COVID floors and that the 7th floor is Mental Health. Can the VA confirm if these floors will require VA escorts and are there any other special requirements that would limit the scope production we should know about for bid purposes?
A47: Within Building 40: Floor 7 requires VA escort or additional staff to be present. Floors 4 thru 6 require adherence to stricter PPE requirements, N95 mask for all contractor personnel. PPE provided by contractor.
RFI No. 48 08 71 13 Q48: “Based on what I see only hardware sets 4A, 4B, and 10A would have (ADO) automatic door operators (as covered in 087113) Those sets are not used on the door schedule A601” Please confirm Hardware sets per location A48: If not in door schedule, not required.
RFI No. 49 01 35 26 Q49: Spec Section 01 35 26 outlines Infection Control Classifications, but does not specifically call out a Class rating for this project. Please provide a Class rating for this project.
A49: This project is classified as a IV rating. Consturtion will pass through several high-risk areas (i.e. OR, ICU) and is expected to generate moderate dust. See specifications and ICRA permit for more information on IC control requirements.
RFI No. 50 Equipment Deliveries Q50: Please confirm if a Courier Delivery would be acceptable? If it is, please confirm who should receive the package?
A50: Contractor must be on-site to receive packages. VA is not responsible for lost or damaged material if not received by vendor.
RFI No. 51 Bid Opening Q51: Please confirm if we are to be present at the Bid Opening?
A51: Solicitations will be review internally be VA Contracting. Contractors are not part of the process.
RFI No. 52 SC Drawings Q52: Please provide a connection diagram of the proposed security devices A52: There are no connections to system head-end equipment included in this project, so connection diagrams do not apply.
RFI No. 53 SC Drawings Q53: Please list the cable type and qty being used for each type of security device A53: Refer to specification section 280513 for VA standard wire/cable.

Q55: Please confirm if the work hours will be weeknights or weekends A55: Regular schedule is for the projects is nights (5:00pm - 5:00am) per the SOW. Weekends will also be available for work, but need to be coordinated in advance with VA COR. Prep-work that does not impact hospital operations can be performed during regular hours with prior approval from VA.
RFI No. 56 08 71 00 Q56: Is the intent to install surface mount conduit or wiremold everywhere, or are any areas expected to be installed above the ceilings?
A56: The intent is for surface mount conduit or wiremold at locations with CMU or masonry walls. All conduit and cabling should be concealed in sheetrock walls and where possible/appropriate.
RFI No. 57 SC Drawings Q57: Please indicate what should be included for removal of existing security devices or conduit / boxes A57: All existing security devices, hardware and conduit/boxes that will not be reused or identified for future use shall be removed. Coordinate with the VA prior to removal of any devices or equipment.
RFI No. 58 SC Drawings Q58: Please indicate locations of the existing security devices to be removed A58: New security device locations are indicated on the SC drawings with a Keynote referencing the access control system upgrade. Existing device locations include security device symbols but do not indicate any reference to the access control system upgrade.
