ASBESTOS PRE-RENOVATION INSPECTION
VA-Madison Third Floor Renovation B – Wing Third Floor
Environmental Management Consulting, Inc. (EMC) was contacted by Nagel Architects to conduct a pre-renovation asbestos survey of an area of the VA Hospital located in Madison, WI.
Mr. Jeremy Noegel of EMC inspected the subject property on October 4, 2013. The building was inspected for the presence of asbestos containing building materials. Bulk samples were extracted for each building material found suspect to contain asbestos that has not been previously sampled in a past inspection. All samples were submitted to a NVLAP certified laboratory for analysis via Polarized Light Microscopy (PLM). This report will summarize the findings of this inspection.
The only area of the building inspected at this time was the 3rd Floor B Wing. The following is a summary of the findings of this inspection.
ASBESTOS CONTAINING MATERIALS (ACM)
The following is a summary of materials identified to contain >1% asbestos or assumed to contain asbestos.
1) Asbestos Containing Pipe Insulation (Previously Sampled)
Material Code: TA5, TC5 & TM5
The existing pipe insulation (Aircell type, cardboard type and magnesia type) have been previously tested during past inspections and determined to contain asbestos.
This material is friable and mainly found above ceilings and inside wall chases.
There is approximately 1800 LF of asbestos containing pipe insulation located throughout this area.
2) Asbestos Containing Pipe Fitting Insulation (Previously Sampled) Material Code: TF5F
The existing pipe fitting insulation located on fiberglass straight run insulation was previously tested during a past inspection and identified to contain asbestos. There are approximately 15 asbestos containing pipe fittings located in this area.
3) 9” Floor Tile & Adhesives Material Code: MF9
The 9” floor tile and adhesives found throughout this area were both found to contain asbestos. This material is located throughout much of the area, and much of the 9” floor tile is covered with a second layer of flooring. Because of this condition it is impossible to determine the approximate quantity of this material.
4) 12” Floor Tile & Adhesives Material Code: MF12
The 12” floor tile is located throughout much of this area. The 12” floor tile was found positive for asbestos, however, the yellow mastic was found negative. In many areas this 12” floor tile was applied over the 9” floor tile (ACM) or applied over the old black mastic from the 9” tile (ACM).
5) Ceramic Tile With Yellow Adhesive Material Code: MCTM(1)
This material is located inB3095 Toilet/Bath/Shower rooms. This material is covered with a second layer of ceramic tile. The yellow adhesive was found to contain asbestos.
6) Fire Doors Material Code: MFD
Fire doors are located in the corridor on the third floor. The core of these doors is assumed to contain asbestos.
7) Black Paneling Adhesives Material Codes: MPM
The material is located above the ceiling, on the wall in room B3079. (Not listed in summary below)
Summary of Asbestos Containing Materials
The following is a summary of asbestos containing materials in the third floor “B” Wing.
Much of the flooring is assumed to be present below a second layer of flooring. Most of the piping is located inside walls and chases. The quantities are approximations based on past experience in the facility.

These materials must be removed by personnel certified by the Department of Health Services, if they are likely to be disturbed during the renovation project. Removal is not necessary if the materials will not be disturbed during the project.
NON-ASBESTOS MATERIALS
The following is a summary of materials, which have been sampled and found not to contain asbestos.
Third Floor Wing B
Material Material Code Description
MDW Drywall and Joint Compound MCTM(2) Ceramic Wall Tile, Grout and Adhesives MV6N 6” Vinyl Base (Brown) and Adhesives MSCT1 2’x2’ Ceiling Tile – Revealed Edge MCLKY* Duct Caulk-Gray MCLKW Sink Caulk - White MCLKT Wall Caulk – Tan
*Note – This material was sampled earlier with a positive result indicated by the laboratory. After discussion with the laboratory regarding this sample, it was determined that the sample may have been contaminated from a different source and additional sampling was recommended. Three additional samples were collected, all with negative results. Therefore, the material was determined to be negative for asbestos.
No work is necessary for these materials.
ADDITIONAL REQUIREMENTS
If this renovation will involve the removal of greater than 160 square feet or 260 linear feet of asbestos containing materials a “Notification of Demolition and/or Renovation and Application for Exemption” (Form 4500-113) must be submitted to the Wisconsin Department of Natural Resources (WDNR). Residential property as defined by the WDNR chapter 447 may be exempt from notification requirements. However, if any amount of asbestos removal is required for the project, the above stated notification will be required to be submitted to the Wisconsin Department of Health Services. The owner/operator is responsible for submitting all notifications.
If during the course of renovation, suspect materials are found, that have not been identified or sampled, work must stop, and a State of Wisconsin Certified Asbestos Building Inspector must be contacted to assess the material. There are inaccessible areas within the building that could not be accessed during the inspection, which could result in additional suspect materials discovered.
All bulk sampling information including a bulk sample chart and the laboratory results are attached to this report. Please contact the EMC office if you have any questions.

This report relates only to the samples tested and may not be reproduced, except in full, without the written approval of SAI. This report may not be used by the client to claim product endorsement by NVLAP or any other agency of the U.S. government. Estimated MDL is 0.1%.
