About this file
This file is a Questions and Answers document for Amendment 0003 related to a solicitation for ambulance services in Columbus.
The solicitation (36C25026Q7571) is issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 10 as an RFQ for ambulance service procurement. The opportunity is set-aside exclusively for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs), with a NAICS code of 621910 (Ambulance Services) and a small business size standard of $22.5 million. The FSC/PSC code is V225 for transportation/travel/relocation services. The RFQ incorporates multiple supporting documents including a Statement of Work (WD 2015-4729 Rev 33), Department of Labor wage determination compliance statements, subcontracting limitations compliance documentation, a past performance questionnaire, start date confirmation, and a required compliance checklist. As a Questions and Answers document specific to this amendment, the file contains clarifications and responses to inquiries from potential vendors regarding solicitation requirements and evaluation criteria rather than detailing specific service requirements or deliverables.

Questions and Responses
1. D.4 requires certifying compliance based on "the Performance Requirements Summary (PRS) section of the PWS," but D.3 states there is no PRS section, and the PWS itself does not contain a section titled "PRS" (only a Performance Standards table and a QASP reference). Where in the PWS should offerors look to satisfy the D.4 certification?
Response: Language on the PRS has been updated. An older version must have been attached to the RFQ. Updated version is attached. Which version is submitted with quote will not determine compliance as long as a completed D.4 is submitted.
1. PWS Section 4 requires the contractor to provide CCT Tier 1, 2, and 3 services, but the Price Schedule only has CLINs for BLS, ALS, and Stretcher/Lay-Flat (base rate, mileage, wait time — 9 CLINs per period). There are no CLINs for CCT. Should CCT be billed under the ALS rate, or will a separate CLIN/rate structure be added?
Response: Please quote a price for each CLIN in the price schedule. PWS is written and approved at the regional level. We are limited as how much we can tailor it to the VAMC/VAACC. CCT was not included in the CLIN because Columbus VAACC does not order trips for CCT. Please only bill ALS trips under the ALS rate. A CCT CLIN will not be added.
1. C.2 references 52.219-6 (Notice of Total Small Business Set-Aside), while Section E.3 separately evaluates SDVOSB/VOSB status as a scored factor rather than a set-aside restriction. Please confirm - Is this a total small business set-aside (not restricted to SDVOSB/VOSB firms) with SDVOSB/VOSB status scored only as an evaluation factor?
Response: This solicitation is for a Small Business (SB). Please see question 8 for more detail on the response.
1. In Ohio, Stretcher or Lay Flat Transports are not an authorized form of transportation. The state will not authorize a license for providing Stretcher or Lay Flat Transports. Will you be removing the language from the PWS referring to Stretcher or Lay Flat Transports?
Response: The CO has conducted research and spoke with the The Ohio State Board of Emergency Medical and Fire Transportation Service (EMFTS). The EMFTS does not specify how the patient must be transported when issuing licensing for Special Mode Transportation. The Stretcher or Lay Flat Transports are when a patient is required to be in a position other than sitting upright but otherwise requires no medical attention and an EMT or Paramedic is not required. Medication is not required on the trip as well. The PWS for Ambulance and for Non-Emergency Medical Transportation Contracts (NEMT) have always been written to give authorization for patients with this need. In the past contracts did not separate BLS and stretcher, Vendors are now encouraged to price trip cost without EMTs/Paramedics to fulfill this need.
1. Are the completed Past Performance Questionnaires due 11 July 2026, 1pm EST? I ask because that is different than the quote due date of 16 July 2026, 1 pm.
Response: All documents will be due at the time of the response date. Updated version of the D.5 is attached to Amendment 0003. Which version is submitted with quote will not determine compliance.
1. There is no mention of pricing on the Compliance Checklist. Will interested parties be compliant without submitting pricing??
Response: The factors for evaluation are Technical, Past Performance, Price and SDVOSB status. If quote is submitted that passes technical and past performance and has not filled in the cost schedule; that quote would be eliminated during the price evaluation stage as an unknown price cannot be determined to be fair and reasonable.
1. This is the only sentence in the solicitation that mentions a “confidence rating.” Can you please explain the confidence ratings you are referring to here? As in, what are the definitions of the various confident ratings?
Response: Past Performance is given five ratings. Exceptional, Very Good, Satisfactory, Marginal Unsatisfactory. Unless they have no rating then a sixth rating is given of “Unknown”. Those ratings are also assigned a risk factor ranging from Substantial Confidence to No Confidence. Where a vendor without relevant past performance for whom information on past performance is not available, the offer may not be evaluated favorably or unfavorably on past performance. [FAR 15.305(a)(2)(iv)].
1. Full credit, partial credit and some consideration are extremely vague and are not defined. Can you please provide definitions for full credit, partial credit and some consideration?
Response: This is verbiage that is required for requirements not set-aside for SDVOSB. The Department of Veterans Affairs, when feasible, gives the advantage to an SDVOSB or VOSB. This requirement will be evaluated on Technical, Past Performance and then Price. In the highly unlikelihood that those three evaluations are all equal with two or more vendors. We will look at vendors SDVOSB/VOSB status and subcontractor’s SDVOSB / VOSB status when making award decision.
