Revised - STATEMENT OF WORK (SOW) Project Number/Title: 552-26-502 B-330 Expanded Parking Lot Place of Performance: Dayton Veterans Affairs Medical Center; 4100 W. Third St., Dayton, Ohio 45438

Period of Performance: 120 Calander Days after receipt of Notice to Proceed (NTP). The first 30 days will be dedicated exclusively to administrative activities, followed by 90 calendar days for construction activities.

1. Background: This Statement of Work (SOW) outlines the requirements for the construction of a gravel parking lot at the Dayton VA Medical Center. Due to the RTO order, the facility is proposing to extend the staff parking lot on the West side of Bldg.330 adjacent to Liscum Rd., Michigan Ave., and Minnesota Ave. The temporary gravel parking lot will cover an area of approximately 123,000 square feet and accommodate 320 parking spaces. The purpose of this project is to provide ample parking space for staff.

2. Scope: The contractor shall furnish all labor, materials, equipment, supervision, and transportation necessary to perform the construction tasks required to complete an approximately 123,000 square foot gravel parking lot with 320 spaces. The work will include, but is not limited to, site preparation, grading, gravel installation, and placing parking blocks in parking spaces. The contractor shall comply with all applicable federal, state, and local laws and regulations, including VA-specific guidelines. The contractor shall maintain a safe work environment and adhere to VA Master Construction Specifications, and OSHA safety standards.

3. Specific Tasks:

3.1 Site Preparation: Contractor shall identify and protect any existing utilities or structures and conduct a site survey to determine existing conditions and establish boundary lines. A four-foot construction fence around the perimeter of the construction site will be acceptable for this project. The fencing shall be maintained throughout the entirety of the project and shall have proper temporary signage installed. Clear and grub the site to remove any vegetation, curbs, debris, and other obstructions. The contractor shall protect existing storm drainage systems from soil or debris entering the drainage system and protect any existing pavement to new gravel parking lot.

3.2 Grading and Drainage: Perform necessary grading to ensure proper drainage and create a level surface to prevent water accumulation. Where the newly installed gravel meets the existing paved surfaces, elevations shall be the same allowing for vehicular access across conjoining surfaces. Contractor will utilize survey points of reference in drawing (“Dayton VA-Slope-Grade”) to establish grade for positive flow.

Areas between roadways and existing swells shall be landscaped, in a manner to provide erosion control and positive drainage for stormwater runoff.

3.3 Gravel Installation: Supply and install a geotextile fabric to stabilize the subgrade and prevent gravel migration. Install layers of gravel to achieve a minimum compaction depth in compliance with industry standards. Compact gravel layers using appropriate equipment to achieve a firm and level surface.

3.4 Parking Space Layout: Provide approximately 320 concrete parking blocks, layout and install parking spaces according to local standards for each parking space to ensure orderly and safe parking. Securely anchor the concrete parking blocks to prevent movement or migration at each parking space to ensure durability. Ensure that the concrete parking blocks are aligned with the layout of the parking spaces and meet any relevant safety standards.

3.5 Inspection and Quality Control: The contractor shall provide a site superintendent with an OSHA 30 construction safety certification dated within five years. The contractor shall provide OSHA 10 construction safety certifications for staff that will be performing work on the construction site.
