B.2 PERFORMANCE WORK STATEMENT

Veterans Integrated Service Network Serving Indiana, Michigan, and Ohio (VISN 10) Performance Work Statement Paperless Medication Guides Updated: 8/19/2025

INTRODUCTION:

VISN 10 has identified the need to have a quick-response (QR) code printed on their prescription labels that provide quick and convenient access to electronic medication guides to increase efficiency.

BACKGROUND:

Outpatient Pharmacies throughout VISN 10 dispense an impressive volume of approximately 185,750 prescriptions each month through window and local mail services. This number does not account for the prescriptions filled through the centralized Consolidated Mail Outpatient Pharmacy (CMOP).

The Food and Drug Administration (FDA) Code of Federal Regulations (21 CFR 208) requires that a medication guide be included with every prescription that is dispensed for a medication with package insert and instructions for use. The medication guides are crucial for educating patients about important aspects of their prescription including dosing instructions, potential side effects, and adverse reactions. Typically, each medication guide spans between 2 to 4 pages. On average, that’s greater than 550,000 sheets of paper that go out of the pharmacies in VISN 10 each year. These medication guides automatically print with the prescription label through pharmacy automation and staff must manually collate and affix to the prescription bag.

Medication guides play a critical role in ensuring patients are well-informed and can safely and effectively use their medications. As of 2023, the FDA approved electronic medication guides instead of the traditional paper formats unless otherwise requested by a patient. This innovative transition is a significant technological advancement that opens a door for resource conservation and cost savings opportunity. These electronic/digital guides can be accessed effortlessly by patients using a quick-response (QR) code printed on their prescription labels. By simply scanning this QR code with the camera on their smartphones, a website with the pushed medication NDC and ID is opened on the patient's smartphone that contains the most up-to-date information, including potential demonstration videos and other interactive content. No Protected Health Information (PHI) or VA sensitive data is integrated in the QR code.

REGULATORY COMPLIANCE:

· FDA Regulations on Medication Guides

· 21 CFR 208: This regulation requires that a medication guide be provided with certain prescribed drugs and biological products when the FDA determines that:

· Patient labeling could help prevent serious adverse effects.

· The product has serious risks that could affect a patient's decision to use or continue to use the product.

· Patient adherence to directions for the use of a product is crucial to its effectiveness.

· Federal Electronic Health Record (EHR) Regulations

· Health Information Technology for Economic and Clinical Health (HITECH) Act: This act promotes the adoption and meaningful use of health information technology.

· 21st Century Cures Act: This act includes provisions to improve the interoperability of health information technology and to facilitate patient access to electronic health information.
