About this file
This is a Lessor Performance Scorecard appendix for a federal facility lease, establishing quarterly performance standards and evaluation criteria for the lessor's operational and maintenance responsibilities.
The scorecard measures lessor performance across nine key areas with specific performance thresholds tracked quarterly (Q1-Q4). Performance objectives include: completing work orders associated with Expedited Outpatient Care (EOC) round findings within 14 calendar days (100% threshold); completing 85% of routine calls within five working days and the remainder within ten working days (90% threshold); performing housekeeping tasks per specifications (80% threshold); maintaining preventative maintenance per manufacturer recommendations (85% threshold); limiting validated staff complaints to no more than five per quarter; achieving 90% compliance on The Joint Commission (TJC) inspections and documentation; maintaining bathroom and flu station supplies with no more than five stock outage reports per quarter; ensuring adequate staffing with no more than three reports per quarter for delayed or lack of response (100% threshold); and performing semiannual privacy curtain replacement or laundering (100% threshold). When quarterly performance falls below the specified Performance Threshold, the lessor must submit a performance improvement plan to the VA Contracting Officer Representative (COR) within 10 working days for review and approval, after which the lessor implements the plan while the VA COR monitors for compliance.
