RFI # QUESTION RESPONSE BY

1 There is mention of the Vets-4212 in the solicitation.

Are we to submit a Vets-4212 with our proposal?

Yes

VA

2 Specification 01 00 00 General Requirements, Will all work hours be required to be other than normal – nights and weekends?

Yes

VA

3 Specification 01 00 00 General Requirements, Will the other ORs remain in use / full operation during the construction, 24 hours / 7 days a week?

Yes

VA

4 Drawing GI200 General Note F, Please define

“Professional Terminal Cleaning”, is that a certified company, licensed, someone from the manufacturer of the equipment? If so, what is the manufacturer of the equipment that requires professional terminal cleaning.

"Professional Terminal Cleaning" company is a cleaning company experienced in the cleaning of surgical rooms of hospitals.

VA

5 Drawing GI200 Drawing Note 6, Is plastic sheeting required in the adjacent rooms encircling the OR under construction? In addition to the other ICRA requirements on this sheet?

The intention is to isolate room OR-3 from the other areas. The walls currently do not go to the deck. Contractor to install wall to deck from the top of the existing surgery room wall from the renovated room side. Attach drywall on the inside wall face and seal with plastic during construction. VA/FFE

6 Drawing GI200, Please clarify how the negative air exhaust is to be routed out of the work area, for all three OR's?

The contractor will need to provide a flexible duct connected to the exhaust fan in the roof nearby. This could pass thru the ceiling areas above the rooms as long as the ICRA barrier is sealed around the duct. Fan location is over OR 2. FFE

7 Drawing AD141 General Note H, Are there as-builts identifying the current configuration of the sprinkler lines/heads?

No. [FFE can drawings be produced from lidar imaging?]

VA/FFE

8 Specification 01 00 00 General Requirements, Does

Steris install the new heads? How much advance notice does Steris require to schedule?

Steris installs supply heads. 30 days’ notice to Steris to schedule installation of supply heads and integration. They are different teams for Steris. Wiring and data cable by contractor. VA

9 Per contract drawings GI200 and GI201 drawing note 6, dash line table, and elevation 2 titled TYPICAL ICRA ELEVATION. All seem to indicate that the contract is requiring an ICRA plastic sheeting curtain wall be hung from the existing ceiling system completely around each OR room in corridors and adjacent rooms. Will the government please confirm that is the intention?

This is not the intention. We need to provide a ICRA barrier above the ceilings and a solid wall and airlock at the door. See GI200, General Infection and Dust Control Notes - Part H.

FFE

10 Contract drawing S501 sections A, B, C, and D detail the mount support steel requirements. Are the scaled dimensions you takeoff these section mount details correct to true field conditions?

a. If, not were the elevations from the existing OR ceiling heights to the bottom of the existing joists chords, that the new mount support steel attaches to, verified during the contract design investigations above ceiling. Will the government please confirm that these heights were verified? b. This information is required to properly estimate the hanger leg lengths due to the cost of steel. If these heights were not verified can the government, please provide a length for bidding and estimating purposes?

The structural support sections are accurate from our field takeoffs and ceiling scans. The height of the mounting plate may be closer to the ceiling than shown. Verify with Steris the required height above the ceiling.

Keep in mind that the steel bar joists are sloping and will require the contractor to field verify the exact dimensions required before fabrication.

FFE

11 Per contract drawing S501 sections A and B there is a note that states “EXISTING JOISTS (SLOPE).” What is the existing slope of the joists?

2.5% slope or approximately 1/4" per ft.

FFE

12 Are pre-engineered bottom mount supports allowed to be used instead of the steel fabricated mount supports detailed on contract drawing S501 sections A, B, C, and D?

Contactor may submit for review by the engineer another system for consideration.

FFE

13 Will the government please identify the location of the electrical closet and panel that welding equipment power should come from?

Electrical closet is just off the elevator lobby. Room B418, Panel 4BEMG-A VA/FFE

14 Contract drawing AD101 does not show or refence demoing the wall location where the new nitrogen control panel gets install that is called for on contract drawing PL101 detail 2 drawings note 6. Is the wall not supposed to be demolished and patch back where the new nitrogen control panel goes? a. If the wall is to get demoed and patched back, will the government please provide information on the extent of the demolition and replacement for bidding and estimating purposes?

The wall material will need to be removed and replaced and patched to allow for piping to be routed down the wall to the new nitrogen panel. Paint disturbed or new wall surfaces to match existing adjacent.

FFE

15 Contract drawings AD141 and AD142 only provides drawing notes 2, 7, and 8 at one of the existing anesthesia columns in each OR room. Does the second anesthesia column that is only referenced as a box in the ceiling require the same removal drawing notes or not?

There are two anesthesia columns in each of (3) OR rooms that need to be removed. Notes 2, 7, & 8 apply to both.

FFE

16 Contract drawing AD101, AD102, AE100, and AE101 do not show where the wall is to be demolished, blocking is to be install, and where wall needs to be patched back for the “CONTRACTOR TO FURNISH

MONITOR ARM AND MOUNTING BRACKET FOR

STERIS CONTROL MONITOR” called out for under general note A on contract drawing EP104. Will the government please provide locations for where the contractor furnished monitor arm and mounting bracket is to be installed?

The arm is next to item "N2" shown on Detail 2/ sheet E601 and EP104 in OR1.

FFE

17 Contract drawing AE101 detail 1 provides a note at the bottom of new bump out wall locations that states, “NEW EPOXY BASE TO MATCH EXISTING.” Isn’t the existing flooring in the OR rooms sheet vinyl flooring that is rolled up the wall for base?

The floors are sheet vinyl rolled up to the wall for a base.

18 Did the VA and A/E verify the existing med gas valve boxes are working?

The OR rooms are currently in use so the med gas valves should be in working order. FFE

19 On contract drawings ED104 in OR 3 and OR 2 there is a note pointing to a TV and Data outlets that states, “CEILING MOUNTED.” Are the TV and Data outlets to remain and need to be protected during the ceiling demolition or do they need to be removed during the ceiling demolition and reinstalled in the new ceiling?

The TV monitors and data outlets are to remain and need to be protected or reinstalled during construction.

FFE

20 On contract drawing EL104 in OR 4 the new isolation panel is shown to be installed in an existing wall.

However, all other OR rooms new isolation panels are shown to be installed in the new bump out wall location. Due to the size of the new isolation panel in OR 4 should it be installed in the new bump out wall location just like the other ORs require?

The isolation panel will be mounted in a 12" deep wall.

There should be enough depth to install without requiring a bump out wall.

FFE

21 On contract drawing EL104 the new lighting circuit note 2 states, “CIRCUIT ORIGINATESFROM PANEL 4BNA IN ROOM B-403” and note 3 states, “CIRCUIT

ORIGINATES FROM PANEL 4BEM IN ROOM B-

403.” On contract drawings EP104 detail 2 give a room layout location of room B-403 but does not provide a true scalable distance layout of how fair room B-403 is from the OR rooms. For biding and estimating purposes can the government please provide a distance from room B-403 to the OR rooms?

Electrical room B-403 is approximately 165' from the room to column line 21.3/N at the corner of OR-3. Electrical room B-403 is 70' from the same point.

FFE

22 On drawing ET104 drawing note 2 (CONNECT

CONTRACTOR FURNISHED BOOM ARM DATA

PORTS AND PLATES TO EXISTING CABLES

ROLLED UP ABOVE CEILING OUTSIDE EACH

OR ROOM. ID DATA CIRCUITS ON BOTH ENDS

OF THE WIRE.) is provided at each new STERIS anesthesia boom location, while note 3

(CONTRACTOR TO PROVIDE (2) 4 PORT DATA

PORTS AND PLATES FOR STERIS ANESTHESIA

BOOM. INSTALL PORTS AND PLATES AT THE

TWO BLANK OUTLETS ON THE GAS SIDE OF

THE BOOM ARMS.) is to only provide at one new STERIS anesthesia boom location in OR 1. Should drawing note 3 apply to all new anesthesia boom locations?

a. If not, then how many data ports and plates need to be provided at the five STERIS anesthesia boom locations that do not have drawing note 3 indicated at these locations?

Drawing note 3 applies to all 6 boom arms. Total of (2) 4-port plates per arm. (Note: the existing ceiling-mounted data cables are included in the total cables needed for the arms)

23 During the previous boom mount project in OR4 it was found that the existing sprinkler lines were installed right on top of the existing ceiling framing system. Are the existing sprinkler pipes in this projects OR rooms installed right on top of the existing ceiling framing system? a. If they are, was it verified if any of the existing sprinkler pipe interferes with any of the new boom mount locations?

b. NOTE A on SHEET FX101 states” REWORK

SPRINKLER HEADS AND PIPING IN OPERATING

ROOMS AS NECESSARY TO ACCOMMODATE

NEW CEILING LAYOUT” for bidding purposes is the contractor to assume a complete demo and rework of the system?

c. If a complete rework of the fire suppression system is required, can the contractor use existing components and piping as allowed by the new work?

a. The exact location of the sprinkler pipe is unknown. From the scans, sprinkler pipe is supported by rods from the roof deck with sprinkler branch lines close to the ceiling framing.

b. includes demo of the piping due to new layout.

c. Contractor may reuse existing components.

FFE

24 During the previous boom mount project, the electrical contract that purchased and installed new isolation panels had to have the manufacture of the isolation panels commission them before the panel were operational and warranty was provided. Since the VA is providing the new isolation panels for this project. Is it the responsibility of the contractor or the VA to provide the manufacturer commissioning of the new isolation panels?

a. If it going to be the responsibility of the of the contractor to provide this commissioning. Who is the manufacturer of the isolation panels?

b. If commissioning of the electrical panels is by the contractor, what is the contact information of the manufacturer representative that handled phase 1 of the project?

MFG: PG LifeLink. A:

commissioning is the responsibility of the contractor.

B. PGL Sales Order: 20320, (800)287-4123 For Reference, the following breakers from Eaton C-H are included in the PG LifeLink isolation panel: 50A Primary Main Breaker GHC2050 and 20A Branch Breakers BAB2020. See attached "Prebid RFI 23-PG LifeLink Isolated Power Systems".

FFE

25 The specifications call out for the steel erector to be

“AISC-Certified Erector, Category ACSE per specification 05 12 00 paragraph 1.5 subparagraph B.”

This certification requirement is more for Bridge and completed building erection professionals, not retrofitting existing steel supports. Per AISC website there are only two erection companies in Kentucky close to Louisville that have a AISC certification. In the past neither company are interested in such a small complex project inside of a hospital OR. This is also causing a significant cost impact to get AISC erectors to travel to the jobsite and be willing to perform the work. Will the government please remove the AISC requirements?

The specification shall be followed as written. The AISC- Certified Erector, Cat. ACSE shall remain as a requirement of the specification.

26 Phase 1 of the project has materials being stored in the government warehouse. Please confirm that the government will be responsible for bringing these items over to the workspace.

Materials stored at 800 Zorn the contractor shall be responsible to move the items from storage to the work site. Coordinate with COR. Materials stored at VA Storage, Mellwood, the VA will bring the materials to the building 1 warehouse.

Contractor is responsible to move materials to these from the building warehouse to the work site. VA

27 Has the government and or the designer confirmed that the above ceiling scans match the drawings?

Yes

VA/FFE

28 Were the scans completed prior to design documents or after?

Prior

FFE

29 Can the government please clarify the amount of duct cleaning that is needed after construction?

Contractor is to provide duct cleaning from the block off point to the OR rooms. VA

30 Are the existing steel structures to be removed completely under this contract?

Yes, for the existing column arms. FFE

31 Who is responsible for the commissioning on this project?

No commissioning. Contractor shall test and certify isolation panels. VA/FFE

32 Can the government please provide clarification on what STERIS will be installing?

a. Can the government please provide additional details on the type of support that the contractor will need to provide STERIS during installation by others?

b. Can the government please provide a duration for the STERIS support to be provided?

Steris will install the boom arms and other equipment shown.

Contractor to provide structure, wiring, cables, piping, back boxes, to the new arms and equipment.

FFE

33 How will the STIRIS installation duration be addressed under the current 270-day POP?

Increase duration to 300 days.

VA/FFE

34 If there is any work to be done outside of the stated OR rooms, what ICRA barriers and Infectious Control Measures will be required?

a. An additional concern when working outside the perimeter walls of the existing ORs is containing the dust above ceiling with plastic to deck etc. How does the government want these conditions mitigated for work outside the OR suite itself?

Suggested ICRA barriers are detailed on the drawings. A Phasing Plan is to be developed by the Contractor per specification 01 00 00 Part 1.6.I.

Per 01 35 26 Part 1.12. B, the "primary" project scope area is ICRA Class IV and work outside the primary project scope area may vary in Class. VA Infection Control is to determine how to classify work within the corridors. VA/FFE

35 Sheet GI200 Note F states, “CONTRACTOR SHALL

PROVIDE FOR PROFESSIONAL TERMINAL

CLEANING OF PHASED WORK AREAS PRIOR TO

TURNING OVER TO THE VAMC FOR

OCCUPANCY” With the contractor not being trained and certified to provide the terminal cleaning this would be very difficult to accomplish. Typically, the VA provides the terminal cleaning, and the contractor is to only provide a construction cleaning. Can the government please confirm that the VA will provide the terminal cleaning and the contractor is only to provide the construction cleaning?

The contractor is to provide professional terminal cleaning.

The intent is for the contractor to provide terminal cleaning by organization experienced in cleaning hospital surgical rooms.

This is in addition to the contractor’s construction cleaning. The VA will provide its own terminal clearing after space is turned over to the VA.

FFE

36 If the contractor is to provide a Terminal Cleaning, can the government please provide a contact for any VA approved contractors for this service?

Contractor means and methods.

Provide terminal cleaning company information in submittals for review. VA

37 Can the government please provide an access panel specification?

A/E has downloaded the specification to the access door and will modify it to fit the project. FFE

38 Does the contractor need to have a med gas certification performed prior to start of work to verify med gas piping is not contaminated prior to our work?

This would be the existing piping from the zone valve to outlets in each OR.

Yes.

FFE

39 Sheet MD104 Note 4 calls to shut of supply airflow to

OR during construction. Above ceiling survey showed some red damper handles. (OR 1 and OR3) Are all these dampers existing or are some new dampers to be installed? (During review we could not locate second damper in OR2)

There are existing dampers at each OR supply duct serving the

OR.

FFE

40 Sheet MD104 General Note at beginning of each phase there shall be a T&B report.

Is this T&B report just to collect the data (CFMS) and not to adjust dampers?

TAB is to collect the airflow data to allow TAB to be performed towards the end of each phase to match the airflow recorded. FFE

41 Sheet AE100 detail 1/AE100 shows existing walls of the OR to be “topped” out and fire rated.

What is the adjacent room availability and ceiling construction to gain access to install the 5/8” sheetrock, tapping of sheet rock and fire caulking on the non-OR side of the room?

Most adjacent room are sheet rock. Only corridors have drop ceilings. These walls are smoke rated not fire walls.

FFE

42 Do any of the adjacent rooms have hard ceilings? Most adjacent room are sheet rock. Only corridors have drop ceilings. FFE

43 If any of the adjacent rooms have hard ceilings, can the government please identify the locations for bidding and estimating purposes?

Most adjacent room are sheet rock. Only corridors have drop ceilings. FFE

44 How will the contractor access these areas for the above ceiling work?

Once in the room of construction, then access the above ceiling condition. FFE

45 Are there existing access panels or will these ceilings need to be demoed and replaced as well?

There are (2) panels in each OR room. They will need to be replaced. FFE

46 Sheet GI100 note H: What is the timeframe between the contractor needs to allow for the Government between phases? a. How do these days impact the 270-day POP?

At room completion, Punch list, Contractor Terminal Cleaning, Steris Install (2 weeks), VA Terminal Clearing (1 week).

Increase project duration to 300 days. FFE

47 Sheet MD104 shows return duct work to be demolished per note 3. This duct services multiple Operating rooms.

What is the timeframe this ductwork can be disconnected before the new duct work needs to be installed?

This work needs to be completed on the weekends. The OR exhaust can be down for two consecutive non-normal workdays. Two non-workdays (weekend) the return duct may be out of service. FFE

48 Looking at the scans above ceiling. The med gas piping appears to be secured to the Unistrut with tie wire and it does not appear that there is isolation between the copper piping and the Unistrut for dissimilar metals.

Is this considered an existing condition with no action needed by the Contractor?

The copper piping is to be mounted correctly with isolation as required.

FFE

49 It appears that the insulation is damaged on some of the supply ductwork to remain.

Is the contractor required to patch/tape this insulation and if so, how does the Government want to quantify the amount of work for bidding and estimating purposes?

Insulation is to be corrected a needed.

FFE

50 Multiple conduits and flex are mounted to the existing hard ceiling framing.

Are these conduits/flex to be reworked to be supported by the structure above? If so, how does the Government want to quantify the amount of work for bidding and estimating purposes?

Conduits and piping shall be supported by the structure. Much of the ceiling framing will be replaced in this contract.

FFE

51 Existing fireproofing shows evidence of damage. Is it the Contractors responsibility to patch existing damage on fireproofing? If so, how does the Government want to quantify the amount of work for bidding and estimating purposes?

Damaged fireproofing is to be repaired.

FFE

52 Low Voltage cables are not properly supported.

Are these cables to be reworked to be supported by the structure above? If so, how does the Government want to quantify the amount of work for bidding and estimating purposes?

Low voltage cables are to be supported per code requirements.

FFE

53 Sheet S101 provides a layout of the locations of the additional structural work to support the anesthesia boom. Sheet MD104 shows return air duct to be removed to allow for the installation of the new boom.

After reviewing above ceiling conditions, it appears that other items will need to be removed and/or relocated.

For Bidding purposed can the Government quantify what other items need to be removed and/or relocated?

The boom arm structure and the MEP items have been coordinated to miss each other in the new layout.

54 Cannot determine 100% the condition of the rated wall partitions. For example, OR1_11 shows a wall with drywall without drywall tape or fire caulking of penetrations. Will the Contractor be required to patch rated walls and fire caulk existing penetrations and/or head of wall? If so, how does the Government want to quantify the amount of work for bidding and estimating purposes?

All rated walls shall be taped and where penetrations are present fire caulking shall be used to seal off.

FFE

55 Based on the nature of the surrounding areas and the infectious control mitigation required in an active OR area, the assumption is that all work will be performed after hours.

Can the government please confirm that all work on this contract is to be performed outside of standard hours?

Work may be required after hours.

FFE

56 During the site visit the extensive cleaning and dust mitigation efforts that must be adhered to during construction. Can the government please elaborate on the amount of cleaning inside the construction zone that is required daily?

General cleanup and dust removal to mitigate transfer into other areas. Inside the construction zone, contractor shall follow General Conditions, OSHA and ICRA. Work site shall be kept free of debris, is cleaned daily of excessive dust, trash removed daily, and no food or drink in the work site. FFE

57 During the site visit the extensive cleaning and dust mitigation efforts that must be adhered to during construction. It is an assumption that the amount of cleaning outside the work zone will be limited to the debris and dust generated by the contractor. Can the government please elaborate on the amount of cleaning outside the construction zone that is required daily such as but not limited to the areas to be mopped daily, the areas to be swept daily, will any floor scrubbing be required, any additional vacuuming needed etc.?

There should be no dust or debris contaminating areas outside the work zone. If areas outside the work zone become contaminated.

Contractor shall immediately address the contamination, evaluate work practices, and ICRA before work may continue.

Inside the work zone, contractor shall follow General Conditions, OSHA and ICRA. Work site shall be kept free of debris, cleaned daily of excessive dust, trash removed daily, and no food or drink in the work site.

Contractor is responsible for protecting finishes during construction, to include flooring. VA

58 It appears that the existing suites are not getting all walls painted. If that is the case would the government, please consider changing this requirement to include painting all walls?

The three ORs shall have all walls and ceiling painted with two-part cold-cure epoxy paint per the paint specification. VA

59 Do the existing walls in the OR suites have any type of wall coverings such as wallpaper on them now?

None.

VA

60 Who is responsible for removing all items attached to the walls VA or Contractor?

The Contractor. The Contractor shall note where the items are located on the walls, shall store items during construction, and install in same location before space is returned to VA. VA

61 The intention of the project is understood to be provide the work as outlined on the drawings and specifications. However, the government provided above ceiling scans of each space. With the project not being a design build it is the assumption of the contractor that the above ceiling scans are for reference only and not incorporated into the requirement. Can the government please confirm that this project does not have a design build component?

No design-build, Reference only.

It is the expectation that items which can be seen in the scans will be addressed by the contractor in their proposal.

VA

62 The intention of the project is understood to be provide the work as outlined on the drawings and specifications. However, the government provided above ceiling scans of each space. With the project not being a design build it is the assumption of the contractor that the above ceiling scans are for reference only and not incorporated into the requirement. After review of the above ceiling scans, we attempted to identify all concerns and addressed these concerns in subsequent RFI(s). It is assumed that the onus of missed items lies with the government and the designer of record and NOT the bidding contractor. As the bidding contractor on a bid/build project we can only bid what is in the specs and drawings. However, with so many unknowns in the project combined with limited accessibility, it appears that the designer was forced to include terms in the notes such as: as needed, replace as needed, all work shall be field verified, remove all, repair all surfaces, etc.… These “catch all” or “all inclusive” type notes are not quantifiable.

Will the government please confirm that the bidding contractor is not responsible for missed items or non-quantifiable items in the design?

No design-build, Reference only.

It is the expectation that items which can be seen in the scans will be addressed by the contractor in their proposal.

65 Questions 60-63 is intended to clarify the need for contingency monies in the project. Also so I wanted to reiterate the discussion had at the site visit regarding what is expected from the contractor for unforeseen and non-designated work on the plans required to complete the project. It is not my intention to be difficult or cause malice. I understand the nature of the work and the limited accessibility to the site. I just want to ensure our risk is mitigated and I only include costs for the correct items.

There are no contingency monies for the project.
