SPECT-CT Bidder RFIs
| RFI No. | Question | Responsible | AE Response (include initials & date) |
1. It appears the following sheets are listed on the cover sheet but are not included in the set of plans. Please verify if the sheets should be part of the set and issue to the bidders. M002, MD101, M101, M501, E001, E002, ED101, E101, E111 E501 | PM, VA | NM 6/25/21: All drawing sheets are in the 4/20/21 drawings PDF that AE sent to the VA.
2. Factor 3 – Past Performance – Would the Contracting Officer allow for CPARS to be emailed directly to the Contracting Officer by July 1, 2021 at 11 am CST, or to be included as an addendum that does not count against the page requirement in the final proposal, instead of the PPQs to be completed by the project Point of Contact? It is often difficult to obtain PPQs due to the busy nature of Contracting Officers, particularly for projects where they have issued a CPARS report. | VA | PPQ has been uploaded and must be submittied by the contractor's customer to scheronica.cochran@va.gov not later than July 6, 2021 at 11am CST.
3. Are additional Factors with project profiles and key personnel, etc. supposed to be required for this solicitation, or do we only need to submit PPQs and a price? Also, we will submit a formal RFI for the following, but I was wondering if you would accept CPARS reports in lieu of PPQs to satisfy the Factor 1 requirements? | VA | No additional factors. PPQ has been uploaded and must be submittied by the contractor's customer to scheronica.cochran@va.gov not later than July 6, 2021 at 11am CST.
4. The Drawings Part 1 and Part 2 included on Beta.Sam do not include the following drawing pages: M002, MD101, M101, M501, E001, E002, ED101, E101, E111 E501 (seems like they are missing from the PDF). Do you mind checking into this and issuing a Part 3 set? | PM, VA | NM 6/25/21: All drawing sheets are in the 4/20/21 drawings PDF that AE sent to the VA.
5. Will a Past Performance Questionnaire be provided for the subject project? Or should we use templates provided for other projects at this facility, and adjust them for this project? | VA | PPQ has been uploaded and must be submittied by the contractor's customer to scheronica.cochran@va.gov not later than July 6, 2021 at 11am CST.
6. The original bid for this project had 120 calendars to complete the work. Now the revised solicitation has a contract duration of 60 calendar days. We do not believe 60 calendar days is enough time to work through submittals, procurement materials (especially long lead items), construct the project, coordinate installation of the imaging equipment with the OEM and closeout. We request the contract duration be change to match that of the original solicitation of 120 calendar days. | VA | This project is expected to be completed with in 60 calendar days.
7. The RFP lists only two factors for the proposal listed: Factor 1: Past Performance and Factor 2: Price; however, the language on page 11 of the RFP has information about page limitations relevant to a full blown proposal package. Are additional Factors with project profiles and key personnel, etc. required for this solicitation, or do we only need to submit PPQs and a price? | VA | No additional factors.
8. Factor 3 – Past Performance – Would the Contracting Officer allow for CPARS to be emailed directly to the Contracting Officer by June 23, 2021 at 12PM, or to be included as an addendum that does not count against the page requirement in the final proposal, instead of the PPQs to be completed by the project Point of Contact? It is often difficult to obtain PPQs due to the busy nature of Contracting Officers, particularly for projects where they have issued a CPARS report. | VA | No. PPQ has been uploaded and must be submittied by the contractor's customer to scheronica.cochran@va.gov not later than July 6, 2021 at 11am CST..
9. RFI 001: Sent 12/18/20 | PM, VA | NM 6/25/21: All questions were addressed by AE and VA as of 12/23/2020.
10. On other projects at the VA with radiation shielding, the VA has utilized their on-staff physicist to perform the pre-construction and post-construction radiation test to ensure the radiation shield is sufficient. Will the VA perform all necessary shielding tests related to the radiation shield or will the Contractor be required to cover this cost? | VA | Yes. The VA will have the existing shielding tested, perform caluclations, and provide a report with recommendations regarding radiation shielding.
11. Sheet F2/G-004 shows what appears to be a fire rated temporary wall. According to Sheet G-101 and AD101, the two sections of wall to be demoed which receive temp barriers are not rated walls. Therefore, they do not require a fire rated ICRA barrier. Will the hospital rated temporary wall system called Starc Panel Systems be acceptable in lieu of the hard barriers? | AR | Per code, temporary walls must match the existing structure fire rating, since the existing is being demo’ d, or the rating was not provided, the fire rating of the temporary barrier must match the fire rating of the new wall construction.
12. Temporary Construction Barrier General Note states in Note #8 on sheet G-004 calls for bots/shoes to be prewashed prior to walking out of the construction area. However, the note #7 calls for sticky mats. If the boots are wet the sticky mats will be ineffective. Usually, the boot wash stations are for contaminated situations, which does not appear to apply here. Should the sticky pads or the boot wash stations be utilized? | AR | A boot wash station or walk off mats are acceptable to prevent dust from leaving the construction area.
13. Note #4 on sheet A-101 calls out new SCTV camera and ceiling track but appears to be pointing to the lead lined wall. Where is the SCTV and track located? | AR | NM 6/25/21: This seems to be referencing the 11/16/2020 drawing set; there is no reference to SCTV camera in updated drawings dated 4/20/2021.
14. Is this SCTV denoted by #4 as furnished by the VA. Is it installed by the VA as well or installed by the Contractor? | AR | NM 6/25/21: This seems to be referencing the 11/16/2020 drawing set; there is no reference to this item in updated drawings dated 4/20/2021.
15. Note #2 calls on sheet A-101 calls for wall cabinets, but the plan appears to show base cabinets under the Quartz countertop? What is the condition here? Is there base cabinets under the quartz countertop and wall cabinets above or just wall cabinets above? | AR | [Note numbers are based on 11/16/2020 drawings]
16. The sink denoted by note #6 on sheet A-101 is in a base cabinet with countertop. Neither is noted here or on the elevation shown on E1/A201. Is this base cabinet Klearvue and countertop Quartz the same as that of the new control station denoted by notes #1 and #2 on sheet A-101? | AR | [Note numbers are based on 11/16/2020 drawings]
17. Note #3 on sheet A-101 denotes new ceiling mounted monitor by VA. Is this monitor furnished and installed by the VA? | AR | NM 6/25/21: This seems to be referencing the 11/16/2020 drawing set; there is no reference to this item in updated drawings dated 4/20/2021.
18. The finish schedule on sheet A-201 says to match existing ACT. What is the existing ACT to be matched? | AR | Existing ACT to be matched by contractor, unless the VA can provide the data
19. The finish schedule on sheet A-201 says to match existing wall base. What is the existing wall base to be matched? | AR | Existing wall base to be matched by contractor, unless the VA can provide the data. VA – Base cove mold to be 4”.
20. The finish schedule on sheet A-201 says to match existing flooring. What is the existing flooring to be matched? | VA | VA – Mannington “Natures Paths” Vinyl Backing “American Walnut Saddle” Product Code 1693398. This is a strip vinyl glue down flooring.
21. Are the GE design drawings (OEM) available for coordination of equipment? On projects such as this there are usually many pieces and parts to be coordinated with the OEM that is only shown on their own drawings. This will include items which are either Furnished by OEM Furnished/OEM installed, OEM furnished/Contractor installed, Owner furnished/Contractor installed, or Contractor furnished/Contractor installed. | ST, VA | ATB 6/25/21: ST remains unable to supply GE information. VA to coordinate
22. Example of info needed from OEM drawings is the Seismic floor plate assemblies referred to in note #9 on sheet S-102. Are these equipment base plates furnished and installed by GE, furnished by Ge/installed by Contractor, or furnish and installed by the Contractor? | ST, VA | ATB 6/25/21: This information is provided by GE in their design drawings and/or information documents. ST is unable to supply GE information. VA to coordinate
23. Sheet S-301 refers to Equipment Bearing Pads in several of the details (see D1 for example). What is the bearing pad? Is it a steel plate to be furnished and/or installed by the Contractor? | ST | ATB 6/25/21:This information is provided by GE in their design drawings and/or information documents. ST is unable to supply GE information. VA to coordinate
24. Note #6 on sheet S-301 says equipment to be mounted on rails. What rails is this referring to? | ST, VA | ATB 6/25/21: Note 6 says "EQUIPMENT TO BE MOUNTED ON RAILS OR TIE-DOWNS FOR SEISMIC ZONE, SEE GE SEISMIC MANUAL FOR INFORMATION ON RAILS, TIE-DOWNS, AND ATTACHMENT TO SLAB AND/OR PLATFORM." The specifics of the equipment mounts, including any tie-downs or rails or straps or attachment, are in that information are provided by GE in their design drawings and/or information documents. ST is unable to supply GE information. VA to coordinate
25. Regarding Note #6 on sheet S-301, who is to furnish and/or install these rails for mounting equipment, GE, the VA or the Contractor? | ST, VA | ATB 6/25/21: This information is provided by GE in their design drawings and/or information documents. ST is unable to supply GE information. VA to coordinate
26. Note #10 on sheet A-101 calls out a metal platform for OEM equipment to be by the VA. However, note #10 on sheet S-102 calls out the platform to be by the Contractor. Please clarify who is to both furnish and install the platform. | AR | TB 6/28/21: This seems to be referencing the 11/16/2020 drawing set; there is no reference to this item in updated drawings dated 4/20/2021.
27. General note #11 on sheet S-102 says the existing steel has spray applied fireproofing to be removed and replaced for new steel and modification of existing steel. To clarify, at the locations above the new C and HSS beams, there are 42” wide and 30” wide steel plates which get bolted to the existing steel deck above. At these locations, will the existing fireproofing be removed to allow new steel plate, install all new steel plates and beams, and cover all new steel plate, steel beams and angles, and existing steel with spray applied fireproofing? If so, this amounts to nearly all of the existing fireproofing on the metal deck in the work area, correct? | ST | ATB 6/25/21:Spray on fireproofing is visible in pictures of existing condition. Existing structural steel is covered and existing conform metal deck is not covered. Where existing fireproofing interferes with new steel installation, please remove, install steel, then replace in kind. All structural steel including new steel channels, HSS, and w-beams and their connections to support framing should be covered with fireproofing. Note that steel plates and angles are part of the GE installation and will need to be free of fireproofing until that installation. Conform deck in the area does not need to be covered in fireproofing.
So we would expect that this removal and eventual replacement to NOT include nearly all of the existing fireproofing. However, the contractor should judge the extents of their work themselves based on their experience.
28 28. Note #10 on sheet S-102 says the existing floor shall be levelled and flattened as necessary for the new imaging equipment. What is the intention of this note? Is it only to fill uneven areas of the floor? In past experience, flatness may require grinding high spots on the existing, which would be a significant cost which would not be easily determined pre-bid with the information provided. Can this requirement be clarified that the levelling and flatness referred to here is only referring to self-leveling fill of up to 3/8” thick and any grinding determined to be needed after award not be construed as part of this scope of work. ST ATB 6/25/21:Coordinate with GE documents. They have strict requirements for leveling and for testing of that level. Also, the GE site specific drawings are forthcoming will provide additional information.
Per the GE document, filling discrete low spots is not adequate. We anticipate testing per GE instructions, then some grinding for high spots and then a topping slab or a self-levelling filler (up to 3/8” thick) for a zone no less than 3’-0” around the equipment, then testing again.
29. Please provide the type of existing fireproofing to be matched. | |||
| [S-102, note 11] | ST | ATB 6/25/21:Spray on fibrous non-ACM material. | |||
30. Spec 01 35 26 1.13 calls for TB screening of all employees onsite. Is this required? | VA | Yes, TB Screening is required by VA Standards. | Kevin Rawson answered this way last round. |
31. Will Covid Testing be required for onsite employees? | VA | COVID testing is currently not required unless the individual knows he or she was exposed to a person with the virus. The workers must truthfully answer the daily screening questions, wear a face mask and continue to socially distance. | Kevin Rawson answered this way last round. |
32. Spec 01 35 26 1.7 calls for the GC to designate an SSHO. Can the superintendent have a dual role as SSHO. | PM, VA | Yes. The Superintendent can be the Safety Officer and Quality Control Checker for this Jib. The contractor must provide a safety plan and a Quality plan and provide contact information for their safety and QC Representatives. | Kevin Rawson answered this way last round. |
33. Spec 01 45 00 3.2.B.1 requires a CQCM to report to the super. With a small project such as this, we would ask that the super be approved to fill this role in addition to the SSHO. | PM, VA | NM 6/25/21: Superintendent may be CQC System Manager per 01 45 00, 3.4.B: "The CQC System Manager is assigned as CQC System Manager but has duties as project superintendent in addition to quality control." A/E takes no exception to dual role if VA is acceptable.
| Yes. The Superintendent can be the Safety Officer and Quality Control Checker for this Jib. The contractor must provide a safety plan and a Quality plan and provide contact information for their safety and QC Representatives. | Kevin Rawson answered this way last round. | |||
34. Spec 01 45 35 includes an appendix which details the items required for special inspections. This is to include: seismic requirements for DSS and FX systems, structural steel work, concrete at anchor locations, spray applied fireproofing, fire protection mastic and intumescent coatings, fire resistant penetrations and joints, smoke control, and seismic requirements for MEP components. Please clarify the following: | ST, Multiple Disciplines | ATB 6/25/21: Struc has no comments
VA: A member of GE’s team will inspect the super Structure improvements, the floor leveling and floor penetrations for conduit runs. The GE representative plans to be at the construction kick off meet and meet with the construction team early in the mobilization. The VA will provide GE with the construction schedule once the contractor submits it to the VA. Deviations to the construction schedule must be clearly indicated to GE so that they can make their required inspections.
The emergency lighting circuit will be tested as part of the start up of the construction job,
| The smoke test and Dalt test is the same required for any HVAC ducting installation conducted by a qualified Test and Balance contractor. | Kevin Rawson answered this way last round. | ||||
35. What specific item or component is to be tested for the DSS emergency/standby power system? What test is required, is it only the walk-down inspection of is an actual test also required? | EL | See Response 34. | |
36. What is the fire protection mastic and intumescent. Coating? Is it fire caulking? | AR | See Response 34. | |
37. What is the fire resistant penetrations and joints? Is it rated assemblies such as those by Hilti? | AR | See Response 34. | |
38. What type of test is required for smoke testing ductwork? Is this the same as a duct leakage test performed by the TAB contractor? | ME | See Response 34. | |
39. Spec 03 30 53 1.1.2 notes Concrete topping slab. However, concrete topping did not appear to be included in the plans. Please confirm if a concrete topping slab is required on this project. Is this referring to the minor flooring levelling referenced in note #10 on sheet S-102? | ST | ATB 6/25/21:Concrete topping slab is an option to achieve the required floor leveling. Another option is self-levelling filler. | |
40. If not referencing the minor floor levelling from note #10 on sheet S-102, please provide additional information to define mix design, thickness, location of topping, etc. | ST | ATB 6/25/21: No response necessary | |
41. Spec 22 05 11 1.4.I requires full coordination drawings for new plumbing. With the small magnitude of plumbing modifications, can this requirement be excluded? | PL | Due to the size of the project the requirement for drawings can be deleted, however the contractors are still responsible for coordination with other trades. | Answered by Chuck last round. |
42. Spec 23 05 11 1.5.E.3 requires full coordination drawings for new HVAC. With the small magnitude of HVAC modifications, can this requirement be excluded? | ME | Due to the size of the project the requirement for drawings can be deleted, however the contractors are still responsible for coordination with other trades. | Answered by Chuck last round. |
43. Below are a few items from the RFI responses in Amendment 0003 which require additional clarification. | PM | TB 6/28/21: No response needed. | |
44. #22 Responses says revised plans corrected the casework and is depicted on sheet A201. Sheet A201 from the original and from the revised drawings appear to be identical with no visible differences. | PM, VA | TB 6/28/21: These plans were updated on set issued April 20, 2021. | |
45. The supposed Revised Drawings attached to the Amendment 0003 do not appear to have any changes from the original drawings. There are no revision clouds from the designers, an Adobe file compassion has turned up zero changes between the two files, and we cannot find any differences during our page-to-page review. If the wrong file was attached to the Amendment, please provide the correct Amendment 0003 plans with corrections. If we are overlooking the changes, between the original drawings and the Amendment 0003 drawings, please have the designer identify all of the changes they have made as is standard in the industry with the use of RED revision clouds and revision dates. | PM | TB 6/28/21: Drawings were not clouded as this wasn't an amendment to drawings, it was a reissue. Revised drawings with issue date April 20, 2021 shall be reviewed in entirety for this bid. | |
46. #28 RFI requested the site specific GE Equipment drawings. The response says they are included in the specs. Furthermore, many of other RFI responses say to reference the GE drawings. However, the specs only include the GE preinstall manual, and included with the AE’s Design Analysis document, a set of GE equipment drawings from a different project. Neither of these documents can or should be used to extrapolate information regarding what is VA/GE/GC furnish and/or install for this project. | ST, VA | ATB 6/25/21:This information is provided by GE in their design drawings and/or information documents. ST is unable to supply GE information. VA to coordinate | |
47. #32 RFI says “OEM Equipment Rails – Regarding Note #6 on sheet S-301, who is to furnish and/or install these rails for mounting equipment, GE, The VA or the Contractor?” There was a typo that may account for the non-response provided. The rails refenced here are note #6 on sheet S102 not S301. The response says to reference GE drawings, but the GE documents do not address these rails at all. Please provide details of what these rails are and if the GC is to furnish and install these. This could be a large variance in price for this item, so it would be helpful for the designer to identify what these rails are and who they are constructed? | ST, VA | ATB 6/25/21:The seismic kit from GE includes the mounting equipment. The information about this, and what exactly is included is provided by GE in their design drawings and/or information documents. ST is completely unable to supply GE information. VA to coordinate. | |

50. Demolition Plan [AD101] Sheet Note 8 states that demo and noise-making work will be nighttime work. Are there any other areas or types of work that must be done on off hours? | VA | Any work that would result in a temporary utility loss of 2hrs or greater. | |
51. Please provide survey report by Terracon dated July 15, 2020. | PM, VA | TB 6/28/21: G-003 General Project Access Note 3 is stated to show that survey was completed; however, Terracon report had no findings of Asbestos or Lead. VA to provide report. | |
52. Is there a proprietary DIV28 Fire Alarm contractor at the facility? If so, please provide company and point of contact. | FP, VA | The fire alarm systems are maintained by Johnson Danforth and Associates (JCI). Phone number: 501-404-4811 | |
53. Please confirm if contract has material staging location at facility. If so, where is it? Is prime contractor responsible for any costs associated with staging? | PM, VA | NM 6/25/21: Coordinate with VA per G-003 General Project Access Note 3: "GC TO COORDINATE WITH COR FOR REQUIRED SPACE AVAILABILITY AND ACCESS." | |
54. Submittal Exchange – Ask who owns them? | PM, VA | TB 6/28/21: Submittal Exchange shall be by VA. VA to coordinate. | |
55. Please confirm if this is a (1) line lump sum proposal or if there is alternate deduct? Pages 5 and 6 on the solicitation are conflicting, listing (1) and (2) total. | PM, VA | NM 6/25/21: There is one deductive alternate; see G-004 and spec 01 00 00.

64. The responses to COVID-19 from VA Contracting and Engineering staff and the construction industry around the US have brought about many changes and restrictions to our contract administration and construction processes. These responses/restrictions around the country have varied greatly in their intensity and impact on our projects. Some of these negative impacts include materials, lead-times on materials and equipment, price escalations, and a loss of man-power and production. Due to the issues stated above and the fact that this solicitation requires us to “hold our pricing for 120 calendar days for acceptance by the Government would the Government consider including in this solicitation by reference the FAR Clause 52.216-4, Economic Price Adjustment for Labor and Material? | VA | FAR Clause 52.216-4 will not be added as this award will be made by mid July.
