About this file

This is an Addendum containing revised drawings, specifications, and responses to Requests for Information (RFIs) for the Restoration of Building 1 Envelope project at James A. Haley VA Hospital in Tampa, Florida (Project #673-21-116).

The addendum, issued February 23, 2026, addresses 140 RFIs and implements numerous revisions to construction documents. Key scope modifications include: revised exterior asbestos abatement specifications increasing paint removal to 50,000 SF; addition of a complete 5/9/22 Asbestos Survey Report as a new specification section; reissued glazing specifications (Section 088000) clarifying windborne debris-impact resistance requirements (Level E and Level D ratings per Florida Building Code) and removing VA Physical Security and Resiliency Design Manual compliance requirements; and revised exterior elevation sheets reflecting ACM removal locations, guardrail reinstallation scope on the Northeast wing, relief angle installations, and brick facade demolition limits. Multiple drawing sheets were reissued including floor plans (A-101 through A-109), exterior elevations (A-203 through A-208), demolition elevations (AD-202), and wall details (A-311, A-602, A-620). RFI responses clarify that contractors may submit different projects for specialized experience and past performance factors, past performance examples must meet the project magnitude (between $5,000,000 and $10,000,000), the superintendent may serve dual roles as SSHO or QC Manager but not as triple-hat, exterior work shall be suspended late July through end of October during hurricane season with period of performance extension at VA convenience, 50% of replaced windows require water testing, and interior work includes only ancillary repairs to match existing conditions due to window replacement.

Quantities shall be based on the extent of cracking and shall be verified in the field by the

GC. For bidding purposes, an allowance of 10 feet of crack length may be assumed for each location called out on the plans. An additional allowance of 10% may be assumed for any repairs that occur outside of areas of repairs already noted on plans.

For install of relief angles, is the design intent to remove all brick from these sections from bottom of wall to top? BLUE CORD

VA Response

Yes, the brick shall be removed and reinstalled for full height to install missing relief angles at each floor level. Scope of removal added in revised exterior elevation sheets included herein.

Will the VA please provide approximate quantities of all facade repairs required (brick replacement, tuck pointing, etc)? This will allow all bidders to be on a level playing field cost and quantity wise. BLUE CORD

Is it the intent of the VA for our exterior facade repair pricing to be “allowance” pricing? If not, will the VA please provide approximate quantities to be repaired? BLUE CORD

VA Response Please see responses to RFI’s #67 and 68.

For remove/reinstall brick facade locations, existing through wall flashing to be removed and replaced, including term bar, and sealants. Will brick be quantified separately or incidental to this work? Will assume that we need 4 courses to be removed in order to access the location and install temp brick shoring for while performing work, is this accurate? BLUE CORD

VA Response

Ledge Angle Detail #7, Sheet A-311 revised to clarify relief/ledge angle size. See re-issued sheets included herein. Existing relief angle sizes vary.

For WI 2.93 “remove and strip off exterior coating from all precast and prep for new coating”, is this to include replacing all associated sealants and masonry to concrete, concrete to concrete, and concrete to metal? Is all of this work incidental to the work item or to be broken out separately? BLUE CORD

VA Response

Remove and replace sealant joints between surfaces in contact with existing concrete coating system incidental to coating removal.

Are concrete repairs to be quantified or can an allowance be provided? BLUE CORD

VA Response

Quantities shall be based on the extent of cracking and shall be verified in the field by the

GC. For bidding purposes, an allowance of 10 feet of crack length for crack repairs and 1 cubic feet for spalling repairs may be assumed for each location called out on the plans. An additional allowance of 10% may be assumed for any repairs that occur outside of areas of repairs already noted on plans.

Will the VA please provide a hazardous material report showing ACM’s, Lead, and any other hazardous materials that will require abatement? BLUE CORD

VA Response See response to RFI #33.

Will a change order be issued if there is additional unforeseen damage that is discovered while addressing the deficiencies shown in the plans? BLUE CORD

VA Response Yes, at the AE and VA discretion.

75 Will the VA be providing their own VPIH?

VA Response Yes. The VA does have an Industrial Hygienist on Staff.

Will the VA be utilizing TEM or PCM air clearances? BLUE CORD

VA Response PCM

What will the turnaround time be for TEM clearance BLUE CORD

VA Response 24 hours

78 Frequency of air monitoring test

VA Response

Follow OSHA standards. When in close proximity to air intakes/louvers. VA requires daily testing

It was stated during the site visit that the exterior walls of the building contains asbestos, however this is not indicated on the drawings. Please provide a hazardous material survey for all work areas and a drawing showing the locations of the hazardous material BLUE CORD

The drawings state for the Contractor to provide

MERV 8 air filter media rolls to cover the outside air intakes for the RTUs. Please provide a list of all RTUs that will require MERV 8 air filter media. BLUE CORD

VA Response

The VA does not use media rolls; we use pleated filters majority are 24x24x2. Contractor needs to check daily how loaded the filters are and change, as necessary. Contractor SHALL supply filters. VA HVAC section, request that they physically replace the filters in the AC units.

Put filter/plastic material on the outside air intakes to eliminate need to clean the louvers, and prevent dust entering the hospital.

Will the VA be responsible for ensuring that the

RTUs are properly balanced on the MERV 8 filters are installed which could affect the outside air intake. BLUE CORD

VA Response

Covering louvers will not be a balance issue.

Recommend not using MERV 8 filters. refer to question 80. recomended to use duct work to route intake air away from work areas

Sheet G-101, General Note F states, “Prior to the start of masonry work, Contractor shall provide

"pre-filter" protection of the outside air intakes of

RTU's and intake building louvers adjacent to masonry work using MERV 8 air filter media rolls. Replacement of internal RTU filters is not required as part of the project scope.” Please provide clarification on the intent of this note and how the “pre-filters” are to be attached BLUE CORD

VA Response

We would recommend that the contractor replace the internal filters on the AHUs in lieu of providing external field media on the intakes, since the external media will get wet and dirty and installation of external media has led to the number of RFIs we received. Replacement of internal filters is quicker and easier for the contractor

Please Identify all AHUs and Intakes pertinent to full scope of project. BLUE CORD

VA Response

We would recommend that the contractor replace the internal filters on the AHUs in lieu of providing external field media on the intakes, since the external media will get wet and dirty and installation of external media has led to the number of RFIs we received. Replacement of internal filters is quicker and easier for the contractor

Please provide filter type requirements if asbestos are present in the project scope. BLUE CORD

On page 19 of the solicitation, under Factor 1:

Technical/Management Approach, item 2 states:

“Clearly describe the prime responsible firm (or firms if a J/V) and individuals as well as the roles and responsibilities of individuals proposed as key consultants and subcontractors. Provide a list of all consultants and all proposed major subcontractors, including telephone number, address, and name of contact.”

Immediately following this, the solicitation lists:

“Each individual's name, title, duty, years of experience, and experience level relating to similar contracts in size and scope (Restoration of Building 1 Envelope). At a minimum, the

Offeror shall provide information for the following disciplines:

• Site Superintendent

• Quality Assurance Manager

• Site Safety and Health Officer

• Project Manager”

Can the Government confirm whether the requirement to provide each individual’s name, title, duty, years of experience, and relevant experience applies only to the four key personnel positions listed above, or whether this level of detail is also required for all proposed consultants and major subcontractor representatives included in the project team list? AMERITECH

VA Response

SAME LEVEL OF DETAIL FOR ALL

PROPOSED CONSULTANTS AND MAJOR

SUBCONTRACTORS INCLUDE IN THE

PROJECT TEAM LIST.

On page 19, under Factor II – Specialized

Experience, the solicitation states: “The prime and/or subcontractors proposed that they were involved in the submitted projects, and the tasks carried out by each.” Can the Government please clarify that the following interpretation is correct?

For each example project submitted, the Offeror is required to identify which members of the

Offeror’s proposed project team (including the prime’s key personnel and any proposed major AMERITECH subcontractors) participated in that past project and describe the specific tasks or roles each entity performed on that project.

If a different interpretation is intended, could the

Government please clarify the required information.

VA Response THIS INTERPRETATION IS CORRECT.

On page 19, Factor II – Specialized Experience, the solicitation contains two statements regarding the involvement of the prime and/or proposed subcontractors in past example projects:

The earlier paragraph states:

“The prime and/or subcontractors proposed that they were involved in the submitted projects, and the tasks carried out by each.”

A subsequent paragraph states:

“These examples shall include the prime and/or subcontractors proposed for this project that were involved in the submitted projects and the tasks carried out by each.”

Can the Government confirm that both statements refer to the same required set of three

(3) to five (5) project examples, and that no additional or separate project examples are required beyond the 3–5 projects specified?

If a different interpretation is intended, will the

Government please clarify the total number and type of example projects required. AMERITECH

VA Response

THE WHOLE FACTOR DESCRIPTION

REFERS TO THE SAME REQUIRE SET OF 3

TO 5 EXAMPLE PROJECTS. PLEASE SEE

REVISED INSTRUCTIONS TO OFFERORS.

Understanding that PPQs are to be sent directly to Contracting Officer David Hernandez, can the

Government clarify if it would be acceptable to submit previously completed PPQs from past projects with our proposals, or do new PPQs need to be submitted and sent to David? AMERITECH

VA Response IF PPQs ARE LESS THAN 12 MONTHS OLD

AND HAVE SIMILAR SCOPE OF WORK

AND MAGNITUD THAT OUR PROJECT YES,

THEY CAN BE SUBMITTED TO DAVID OR

SUBMITTED AT THE SAME TIME AS THE

PROPOSAL.

Factor II requires the Offeror to submit three (3) to five (5) example projects demonstrating specialized experience similar to the scope of this procurement.

Factor IV also requires three (3) to five (5) past performance projects, but includes specific minimum dollar value thresholds, including: At least one (1) project valued at greater than

$10,000,000, and At least three (3) projects valued at greater than $8,000,000.

A. Can the Government confirm whether the

Offeror may submit different projects for

Factors II and IV, provided each factor’s requirements are met, or whether the

Government intends that the same set of

3–5 projects be used for both factors?

B. Can the Government confirm that the

$10M and $8M minimum project value thresholds apply only to Factor IV (Past

Performance) and do not apply to the 3–5 projects submitted for Factor II

(Specialized Experience)?

C. If the Offeror meets the minimum required number of >$10M and >$8M projects under Factor IV, will the

Government consider additional past performance examples below those thresholds as part of the 3–5 allowable submissions?

D. If the Government intends a different interpretation for any of the above, please clarify the total number of projects required and the applicability of dollar-value minimums to each evaluation factor. AMERITECH

A. The offeror may submit different projects for factors II and IV.

B. Please submit projects that meet the Magnitude of construction for this Project for both factor II and IV.

C. Please submit 3 to 5 projects that meet the Magnitude of construction for this Project no more.

D. Please submit projects that meet the

Magnitude of construction for this

Project.

A. Would an active multi-year contract— such as a regional preventative maintenance and service contract or BPA with discrete, completed task orders—be considered compliant and relevant past performance/experience, provided it includes scopes of work that are directly applicable to the solicitation?

B. If ongoing contracts may be considered, are there specific parameters or documentation requirements (e.g., percent complete, interim evaluations, summary of completed work orders) that must be provided to demonstrate relevance and performance success?

AMERITECH

VA Response

A. NO, this is not a service contract. We require construction contracts with the same scope as this project.

B. Please refer to Factor IV, section C.

Will the Government consider past performance examples from proposed subcontractors or team members, including those under an SBA-approved Mentor-Protégé Agreement, provided the projects meet the stated relevance criteria

(e.g., scope, complexity, dollar value)? AMERITECH

VA Response

The VA will consider past performance examples from proposed subcontractors or team members, including those under an SBA-approved Mentor-

Protégé Agreement, provided the projects meet the stated relevance criteria (e.g., scope, complexity, dollar value)?

If subcontractor or Mentor-Protégé past performance is permitted, is a Subcontractor

Consent Form or Letter of Commitment required at the time of proposal submission? If so, is there a specific format or template the Government prefers? AMERITECH

VA Response It is required, please see Past performance

For projects performed by subcontractors or

Mentor-Protégé entities, are there any specific labeling or formatting expectations (e.g., identification of prime vs. sub role, duration of involvement, or dollar share) the Government requires within the Experience or Past

Performance sections? AMERITECH

VA Response

For each example project submitted, the Offeror is required to identify which members of the

Offeror’s proposed project team (including the prime’s key personnel and any proposed major subcontractors) participated in that past project and describe the specific tasks or roles each entity performed on that project.

Will the Government consider and evaluate past performance examples that meet the dollar threshold (≥ $10M for one and ≥ $8M for three) but were not performed in an active healthcare environment? Conversely, will the Government consider and evaluate past performance examples performed in an active healthcare environment, but under the dollar threshold? AMERITECH

VA Response Please submit projects that meet the Magnitude of construction for this Project for both factor II and IV.

Would projects conducted in hospital support facilities such as dietary, laundry, or logistics buildings be considered as taking place in an “active healthcare environment” for evaluation purposes? AMERITECH

VA Response No, we require construction contracts with the same scope as this project.

108 Can the Government confirm the required time period for the EMR documentation? Specifically: AMERITECH

Is the Offeror required to submit EMR documentation for only the current year, or Is the

Offeror required to submit EMR documentation covering multiple years (e.g., a three-year EMR average)?

VA Response The EMR form provided required 3 years, so

YES, the offeror is required to submit documentation for 3 years

Factor II – Specialized Experience and Factor IV

– Past Performance each specify a 5-page limit for the narrative sections. The solicitation does not specify whether this page limit applies only to narrative text or also to supporting graphics.

A. Can the Government confirm whether the

5-page narrative limits for Factors II and

IV apply only to narrative text, or whether supporting materials such as tables, charts, photos, key personnel matrices, project snapshots, or graphic elements must also be included within the

5-page limit?

B. If supporting graphics may be included outside the narrative limit (e.g., as appendices or exhibits), does the

Government have a preference for their placement or format? AMERITECH

VA Response A. Applies only to narrative

B. Please keep with the Factor documents

The statement of work (SOW) states “Use of equipment and tools that transmit vibrations and noises through the building structure are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel.” Is this to any level or is there a Decibel and Hertz limitation? In the performance of the work, noise and vibration is a by product of the work. Use of drills, grinders, saws, etc. are needed. Please clarify.
