Hazardous Material Abatement Addendum

All tested hazardous materials that will be affected by the construction shall be removed by the Asbestos/Lead Abatement contractor. The quantities listed below shall be used for bidding purposes.

The contractor and the COR shall agree on the removed quantity of hazardous materials before abatement work is to begin. The contractor shall anticipate abatement in 38 different locations for bidding purposes. Any quantity above the amounts shown below will be subject to a change order based on the unit pricing.

The following General Notes apply:

GENERAL NOTES

1. CONTRACTOR SHALL REMOVE ALL ASBESTOS CONTAINING AND ASBESTOS CONTAMINATED

MATERIALS INDICATED IN THE SPECIFICATIONS, AND DISPOSE OF AS ASBESTOS WASTE. ASSUMED

(PRESUMED) ASBESTOS CONTAINING AND ASBESTOS CONTAMINATED MATERIALS INCLUDE: NON-

FRIABLE BLACK MASTIC LOCATED ON THE EXTERIOR SURFACE OF THE PIPE FITTING INSULATION, FRIABLE PIPE INSULATION, FRIABLE PIPE FITTINGS, FRIABLE PLASTER, FRIABLE CEILING TILE, CEILING

TILE, FRIABLE DRYWALL AND DRYWALL JOINT COMPOUND, VARIOUS FLOOR TILE AND FLOOR

COVERINGS AND ASSOCIATED MASTICS/ADHESIVES/GLUES AND ANY ACM DEBRIS AND ANY OTHER

SUSPECT ACM IDENTIFIED BY 3RD PARTY ACCREDITED ASBESTOS INSPECTOR.

2. CONTRACTOR SHALL BE RESPONSIBLE FOR RETAINING A 3RD PARTY ACCREDITED ASBESTOS

INSPECTOR TO FIELD VERIFY THE EXISTING ESTIMATED QUANTITIES AND CONDITIONS OF ASBESTOS CONTAINING AND ASBESTOS CONTAMINATED MATERIALS AND IMMEDIATELY NOTIFYING THE COTR

IN WRITING OF ANY DISCREPANCIES FOUND.

3. PRIOR TO PLACING EQUIPMENT ON JOB SITE, THE CONTRACTOR SHALL PROVIDE MANUFACTURER’S

CERTIFICATION TO COTR FOR REVIEW AND APPROVAL.

4. CONTRACTOR SHALL COORDINATE THIS ASBESTOS REMOVAL PROJECT WITH THE COTR TO MINIMIZE

INTERFERENCE WITH STANDARD OPERATING PROCEDURES OF THE HOSPITAL.

5. CONTRACTOR IS RESPONSIBLE FOR ENSURING ADEQUATE ELECTRICAL AND WATER SUPPLY IS AVAILABLE FOR THIS PROJECT. CONTRACTORS SHALL PROVIDE ADDITIONAL TEMPORARY UTILITY CAPACITY, IF REQUIRED. CONTRACTOR’S USE OF GOVERNMENT UTILITIES SHALL BE COORDINATED

WITH COTR.

6. CONTRACTOR SHALL ENSURE THAT FIRE ALARMS AND FIRE SUPPRESSION SYSTEMS ARE PROTECTED

DURING CONSTRUCTION AND REMAIN OPERATIONAL THROUGHOUT THE PROJECT.

7. CONTRACTOR SHALL SUBMIT AND OBTAIN APPROVAL OF RECOMMENDED WORK SCHEDULE AND

PHASING FOR SYSTEMS SHUT DOWN, ASBESTOS REMOVAL, AND RENOVATION/DEMOLITION

OPERATIONS TO THE COTR PRIOR TO BEGINNING ANY WORK.

8. CONTRACTOR SHALL BE RESPONSIBLE FOR PHYSICALLY SECURING ALL ENTRANCES AND OPENINGS TO

THE WORK AREA. ALL AREAS WHERE ASBESTOS CONTAINING MATERIAL ARE PRESENT SHALL BE

CONSIDERED AN ASBESTOS REGULATED WORK AREA UNTIL SUCCESSFUL COMPLETION OF PROJECT

FINAL CLEARANCE CRITERIA.

9. LOCATION OF DECONTAMINATION SYSTEMS, ASBESTOS ABATEMENT CONTAINMENT BARRIERS

SHALL BE APPROVED BY THE COTR PRIOR TO START OF WORK.

10. NEGATIVE AIR MACHINES EQUIPPED WITH HEPA FILTERS SHALL BE DISCHARGED TO THE OUTSIDE OF

THE BUILDING A MINIMUM DISTANCE OF 30’ FROM BUILDING MAKE-UP AIR AND 30’ FROM

LOCATIONS WHERE PATIENTS, STAFF AND VISITORS ARE.

11. CONTRACTOR SHALL ASSUME THAT FLOOR TILE AND ASSOCIATED MASTICS, GLUES AND ADHESIVES

MAY CONSISTS OF MUTIPLE LAYERS IN THEIR PROPOSAL.

12. CONTRACTOR SHALL REFER TO ARCHITECTURAL PLANS FOR DOORS, DOOR FRAMES, WINDOWS, WINDOW FRAMES, WALLS, CEILINGS, OR OTHER PAINTED SURFACES TO BE IMPACTED AS PART OF

THIS PROJECT. CONTRACTOR SHALL ASSUME THAT ALL ABOVE LISTED MATERIALS AND

COMPONENTS CONTAIN DETECTABLE LEVELS OF LEAD AND ACCORDINGLY WORK WILL BE SUBJECTED

TO THE OSHA CONSTRUCTION INDUSTRY STANDARD 29 CFR 1926.62 - LEAD. THE CONTRACTOR

SHALL PREPARE PAINTED SURFACES AS NECESSARY, TO ALLOW OTHER TRADES TO PERFORM

RENOVATION WORK WITHOUT DISTURBANCE OR EXPOSURE TO LEAD IN EXCESS OF THE OSHA

ACTION LEVEL. THE INTENT OF THIS PROJECT IS TO MINIMIZE DISTURBANCE TO PAINTED SURFACES.

CONTRACTOR SHALL ASSUME THAT LEAD SOLDER AND LEAD COMPOUNDS ARE PRESENT IN

MECHANICAL PIPING SYSTEMS AND SHALL COMPLY WITH OSHA 29 CFR 1926.62 WHEN PERFORMING

DEMOLITION OPERATIONS. RESULTING DEMOLITION DEBRIS FROM ABOVE LISTED COMPONENTS

SHALL BE TCLP TESTED FOR DETERMINATION OF DISPOSAL REQUIREMENTS IN ACCORDANCE WITH

LOCAL, STATE AND FEDERAL REGULATION.

13. CONTRACTOR SHALL ASSUME THAT FIRE DOORS AND DOOR FRAMES CONTAIN ASBESTOS AND SHALL

REMOVE AND DISPOSE OF FIRE DOORS AND DOOR FRAMES.

The attached specification sections 02 82 11 Traditional Asbestos Abatement and 02 83 33.13 Lead Based Paint Removal and Disposal shall apply to all testing and abatement.
