The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-01. NAICS Code is 334519. This procurement is set-aside 100% for Service-Disabled Veteran Small Businesses (SDVOSB). The Request for Quote is 36C24522R0059. UPDATED: SEE ALL ATTACHED DOCUMENTS.

Quotes Due by 5/9/2022 9:00 AM EST.

Please email all quotes to John.Ritter2@va.gov.

Questions and Answers: Universal Temperature Monitoring - RFP#36C24522R0059 If you have built-in freezers, please provide the number of units that are refrigerators only vs. those combined with freezers. Refer to the inventory list. We cannot differentiate which is which.

How many staff members are there at each facility that would require training? Are there any off-site monitoring requirements? e.g. clinics, mobile, additional sites? The number of staff members will vary. The training may be done in a team format as needed. Yes, there are offsite Community Based Outpatient Clinic (CBOC) monitoring requirements.

Does the VA have iOS and/or Android devices? The VA has iOS devices Does the VA support 802.1x Enterprise Security with PEAP-MSCHAPv2 protocol? This is an internal information; the VA would work and support the request of vendor at the point of installation When connecting to the VA Wireless Network will each tag require an SSL Certificate? This information is internal to the VA.

What is the physical address of all the locations that will have TempTrak?

i) Eastern Baltimore County VA Clinic 5235 King Avenue Franklin Square Professional Center Suite 200 Rosedale, MD 21237-4068 – Community Based Outpatient Clinic (CBOC)

ii) Glen Burnie VA Clinic (CBOC) 808 Landmark Drive Suite 128 Glen Burnie, MD 21061

iii) Fort Meade VA Clinic (CBOC) 2479 5th Street Fort Meade, MD 20755

iv) Loch Raven (LR) VA Medical Center 3901 The Alameda Baltimore, MD 21218-2100

v) Loch Raven VA Clinic (CBOC) 3901 The Alameda Baltimore, MD 21218-2100

vi) Baltimore (BT) VA Medical Center 10 North Greene Street Baltimore, MD 21201-1524

vii) Baltimore VA Clinic (CBOC) 209 West Fayette Street Community Resource & Referral Center (CRRC) Baltimore, MD 21201-340

viii) Pocomoke VA Clinic (CBOC) 101 Market St Ste B, Pocomoke City, MD 21851

ix) Cambridge VA Clinic (CBOC) 830 Chesapeake Drive Cambridge, MD 21613-9408

x) Perry Point (PP) 59 Avenue D, Perry Point, MD 21902 How do we best correlate those locations with the sheet provided? Refer to number 6 acronyms and check the inventory list Does VA intend to do the full rollout of equipment in year 1? Yes, this is the objective The current pricing table appears to indicate that new hardware will be purchased each year, which is not necessary with our solution. If the hardware is purchased in the base year Option Year 1 will consist of subscription/support plus any optional new hardware for new assets to be monitored; Option Year 2 would consist of Subscription/Support + Calibration hardware (probes); Option Year 3 Subscription/Support; and Option Year 4 Subscription/Support + Calibration hardware (probes). Calibration can also be performed every year if VA prefers. Would it be permissible to show the recommended hardware, subscription/support, and calibration items in the price table according to this schedule? The first year is for the purchase and installation of hardware and equipment. The subsequent years are for support and calibration as required by OEM, additional probes as needed, and battery replacement or other maintenance items as required.

Pricing Table: Can the pricing for installation be listed as one single total? No Do you anticipate the proposal due date to remain the same? No-See Amendment.

As per the bid request, it would seem that periods 2,3,4 & 5 request a new system, please confirm that these periods refer to servicing and maintenance for the new installation requested in period 1. See #9 response Will basic floor plans with dimensions be made available prior to the bid response in order to review and use for bid determination? Upon contract award, this information can be made available.

It was determined, during the walkthroughs, that differential pressure sensors are desired at several pharmacy locations. Will differential pressure sensors also be required in any isolation areas? Yes, you are right about the differential pressure, but we don’t have a full list of all the areas that need it yet.

Will an addendum to the original equipment list in the RFP be provided to include the differential pressure units among other additions found during our visits in Baltimore & Perry Point? Yes, based on proposals submitted Is TRM approval required for this bid?- See #5 response Is ability to purchase Universal Temperature Monitoring System through ECAT required for this bid? No We were note able to join the site visit, but can you tell me if there are electrical outlets close to each of the monitoring areas? Or are you expecting the monitoring devices to work via battery? We expect the equipment to have available reliable energy and backup power as needed.

Please confirm that the total obligated funding is not to exceed $1 Million over 5 Years and is expected to include all equipment, installation, warranty, maintenance and service in the base year plus four additional years of maintenance and service? No.

In the RFP, Section B.2 Price/Cost Schedule calls out “Description of Supplies/Services” that are specific to a single known vendor. Is this the only acceptable vendor or will alternative businesses and products that meet the requirement be considered? The specific scope and requirement of service are what we are expecting.

If alternative businesses and products are acceptable, descriptions of supplies/services and quantities may differ. Is this acceptable? See #20 response.

In the RFP, Section B.2 Price/Cost Schedule the same 12 items are repeated giving the impression that a new system and equipment is to be installed each year. Is this understood correctly and/or necessary? Please elaborate. See #12 response.

After initial purchase and installation of our system and equipment, we recommend an annual service and maintenance agreement to include annual calibrations or annual precalibrated probe replacements. Is this acceptable and if so, is there a preference? See #12 response.

The total quantities on the RFP are different from the total breakdown and quantities of equipment to be monitored in the Inventory spreadsheets. How will this delta be addressed should additional quantities be needed upon completion of the RFP requirement? The RFP was estimated to be higher to give room for additional quantities should it be needed, that is the reason for the delta.

The Inventory includes many duplicates as many items are listed on more than one sheet by Location and/or Department. Please confirm if this assessment is correct. It is estimated to be duplicated to give room for errors.

In the RFP, Section B.2 Price/Cost Schedule Item 0008: Please elaborate on 25 Physical Building Site Visits. Please elaborate on 4 Devices Installed. This line item along with (1008/2008/3008/4008) will be deleted from the Price Schedule.

In the RFP, Section B.2 Price/Cost Schedule Item 0009: Please identify where the number 1,146 comes from. See #24 response.

The number of points needed may have been doubled. The total points should be 617, and 1,222 are listed. Whoever reviewed the equipment list perhaps did not realize that items also appeared on other lists, which could have resulted in doubling of the point count. At implementation, aside from the base, the invoice would be based on actual equipment delivered, we anticipated excess on purpose because of those areas unknown to us now.

Wi-Fi Infrastructure Who is your Wi-Fi infrastructure vendor?

If Cisco:

· What version of MSEs and/or CMX do you have? See #5 response

· Are there plans to upgrade any sites to Cisco DNA Spaces? If so, what is the timeframe? See #5 response

· What version of Cisco controllers do you have? See #5 response

· What are the model #'s of your Access Points (APs)? See #5 response

· Does VA Health utilize Flexible Radio Assignment? If so, will VA Health agree to turn it off? See #5 response

If Aruba:

· Wireless Controller make/model See #5 response

· Wireless Access Point (AP) make/model See #5 response

· If not Cisco or Aruba, please provide Model and firmware of AP’s, as well as Controllers if applicable and number of sites. See #5 response
