About this file

This document contains responses to questions regarding a federal solicitation for corrective safety and facility condition assessment issues for sterile processing services at a Department of Veterans Affairs medical center. Key details include requirements for temporary and permanent sterile processing facility upgrades to meet Association for the Advancement of Medical Instrumentation water quality standards, including testing for parameters such as bacteria, endotoxins, pH, and ionic contaminants. The solicitation also specifies installation of related infrastructure, utility connections for a mobile sterile processing trailer, hazardous material abatement exclusions, and clarifications to drawings and specifications. Reporting of critical water testing results to the Veterans Integrated Service Network sterile processing management board is mandated on a regular basis.

Department of Veterans Affairs Memorandum Date: August 28, 2020

From: Assistant Under Secretary for Health for Operations (10N)

Subj: Required critical water systems testing and reporting for Sterile Processing Services

(SPS)

To: Veterans Integrated Service Network (VISN) Directors (10N1-23)

VISN Chief Medical Officers (10N1-23)

VISN Quality Management Officers (10N1-23)

VISN Sterile Processing Services Leads (10N1-23)

1. This notice is provided by 10N in collaboration with the National Program Office for

Sterile Processing Service (NPOSP) and the Office of Healthcare Environment and Facilities Program (HEFP) to provide awareness and reinforce the mandatory requirement for a multidisciplinary team approach to ensure testing, routine preventative maintenance, and reporting requirements are performed for Sterile

Processing Service (SPS) critical and utility water systems per VHA Directive 1116(2). There has been an increase in critical water issue briefs recently that are impacting patient care and are attributed to reduced water usage. Appropriate water testing must occur per schedule, with flushing as necessary regardless of whether SPS is operating at full capacity.

2. Water quality requirements within SPS are defined in the Association for the

Advancement of Medical Instrumentation (AAMI) Technical Information Report (TIR)34: 2014/(r)2017, “Water for the Reprocessing of Medical Devices” which is referenced in AAMI ST79:2017, “Comprehensive Guide to Steam Sterilization and Sterility Assurance in Health Care Facilities”. These documents not only define the parameters to be monitored but also outline the frequency of quality testing and limits for the noted contaminants in utility water and critical water.

The following AAMI TIR34 Table 1 provides the contaminants and the limits to be tested for. All VA Medical Centers must comply with this guidance for water quality limits.

3. All critical water testing results for reprocessing activities must be reported to the

VISN SPS Management Board, through the Facility RME Committee.

4. The VISN SPS Management Board is responsible for ensuring all facilities:

a) Perform the required critical and utility water testing as defined in AAMI

TIR34.

b) Have a program in place for testing and performing routine preventative maintenance on the critical water system.

c) Institute a flushing program for both critical and utility/potable water during times of low water usage in SPS.

5. The VISN SPS Management Board will provide an attestation of these requirements to NPOSP by October 1, 2020. Any abnormalities will require

Subj: Required critical water systems testing and reporting for Sterile Processing Services corrective actions and must be reported through the VISN SPS Management

Board to NPOSP until complete. NPOSP contact: mailto:vhaspshss@va.gov.

6. For questions pertaining to this memorandum, please contact:
