3 Project Site Description:

PSEG 15kV line, Elwood Feeder, is connected to the existing B209 outdoor switchgear to provide power to the hospital.

PSEG 15kV line, Pulaski Feeder, is connected to new B209 outdoor switchgear to provide power to the hospital.

• A new outdoor fusible load break switch is installed and a new 2.0 MW Reciprocating Engine Generator (CTG) is connected to the fusible load break switch via a 480V:13.2kV, 2500kVA Step-up Transformer.

5 Test Procedures

5.1 Safety Brief:

5.1.1 The Commissioning Engineer should review the existing Lock Out Tags Outs to insure all lock outs to be lifted , properly identified and signed off. Special consideration should be taken to verify that equipment not associated with test has been properly tagged out in compliance with VISN 2 VA Northport Hospital safety procedures. Lockoul/tagout procedure shall be in place and followed.

5.1 .2 Verify that all safety precautions outlined in each individual equipment manufacturer's operating manuals have been reviewed and are understood by all start-up personnel.

All manufacturers' recommended operating and maintenance procedures shall be followed. Manuals will be available onsite prior to start of work.

5.1.3 Review evacuation plan and ensure portable fire extinguishers are available. Sketch and sign off needed.

5.1.4 WARNING: Possibility of injury or death from electrocution including serious burns.

Medium voltages (13.SkV, 125VDC) may be present in or about the equipment at any time.

5.1.4.1 Only authorized personnel should be in the vicinity during testing and start-up.

5.1.4.2 Only qualified persons who are familiar with the installation and maintenance of medium and low voltage switchgear should be permitted to work on this equipment.

5.1.4.3 Do not use a stored energy circuit breaker by itself as the only means of isolating a medium or low voltage circuit. All components should be disconnected with a visible break, and securely grounded for the safety of maintenance personnel.

5.1.4.4 Do not leave a breaker in an intermediate position in a cell. Always have the breaker in either the test or connected position .

5.1.5 Always remove maintenance tools from the breaker after charging the springs.

5.1.6 Do not attempt to defeat safety interlocks. This may result in bodily injury or equipment damage.

5.2 Punch List

5.2.1 A punch list will be generated by the Commissioning Agent (Owner/Engineer) when the system declared complete. Items on the list will be ranked in categories of importance, depending upon their relevance to the safe & final acceptance of the equipment and systems. Construction completion is ultimately at the discretion of the Commissioning Agent.

5.4 Procedure: Effective safety policies and procedures must be in place, and all persons on site must be familiar with and committed to following such safety policies and procedures, in order to prevent accident or injury during the witness test.

5.5 Minimum Requirements

Conduct an overall site assessment to ensure that everything at the site is in good condition and working conditions are safe

Lockout/ tagout (LOTO) procedures: For reference see, without limitation 29 CFR 1910.147 and NFPA ?OE.

Personal Protective Equipment (PPE) and other safety equipment as per Occupational Safety and Health Administration ("OSHA") requirements and NFPA ?OE.

Verify that the most current procedures , drawings and documents needed for the job are available

Ensure the proper tools and equipment required for the job are on hand and in good condition

Procedure for safely disconnecting live circuits: Designated qualified personnel shall only operate the electrical disconnects as per Company approved energization plan .

Safety specific signage and warnings .

Local jurisdiction has permitted and signed off to energize electrical equipment.
