About this file
This is an amendment (SF-30 form) containing questions and answers for solicitation 36C24224R0083, a design-build project for SPECT CT site preparation at the VA Medical Center in New York. The amendment clarifies design phase drawing requirements, specifying deliverables at 65% CD, 95% CD, 100% Construction, Redline, and As-built phases with specific format requirements for each phase.
Key submission requirements include SF330s for A/E services, proposal breakdowns for both A/E services and construction labor/materials, VETS 4212 form, SBA status approval, limitations on subcontracting, Buy American certification, project schedule, and safety/environmental ratings. The RFI submission deadline is November 27, 2024, at 4:00 PM, with final proposals due December 6, 2024, at 2:00 PM via email to Vladimir.Stoyanov@va.gov. Contract award is anticipated by December 30, 2024. The amendment also addresses several technical questions regarding room sizes and design requirements, confirming that deviations from VA Nuclear Medicine Design Guide standards are acceptable for the injection room, and clarifying that a connected bathroom is not required.

RFI QUESTIONS AND ANSWERS.
1. Q: Please clarify the Design Phase drawing requirements. The Design-Build specs indicate packages at 30%, 95%, 100%, and As-Built. The SO2 indicates Design drawings, Redline drawings, and As-Built drawings. The 30% package is the equivalent of the design drawings, the 95% package is the equivalent of the redline drawings, and the 100% drawings in the design-build specs are the equivalent of the as-built drawings in the SO2. Please indicate the specific sets that are required.
A: The following Design requirement is required:
1. 65% CD (Instead of 30%, since sample drawing is provided), 2x half, PDF.
1. 95% CD, 2x half, 1x full, completed spec document, PDF.
1. 100% for Construction, 2x half, 1x full, spec in hard-copy, AutoCAD, PDF, sealed stamp.
1. Redline Drawing (During Construction markup by Construction Contractor), 1x half, PDF; refer to Construction shop drawings in spec 001121.
1. As-built Drawing (Finalized drawing for the record), 2x half, 2x full, revised spec in hard-copy, AutoCAD, PDF, refer to Project Close-out in spec 001121.
Ensure drawings are submitted with the A/E Design Review Checklist of all applicable disciplines, found in VA A/E Submissions & Review. All relevant VA TIL specifications are required, with the highlight of the relevant section.
1. Q: Please advise what documents are required for submission by the contractor for Phase I.
A: Submission must have the following documents attached:
4. SF30 filled out and acknowledging the mod(s).
4. SF330’s for the A/E.
4. The proposal must have two parts: 1) a breakdown of the A/E services and 2) a labor and material breakdown by division for the construction portion. Interviews will be scheduled only with the top three firms selected.
4. VETS 4212.
4. SBA status approval with UEI #.
4. Limitations on subcontracting filled out.
4. Buy American Certification
4. Project schedule – Assuming the award is issued on or before 12/30/2024.
4. Safety and Environmental Rating – EMR.
1. Q: The proposed 2021 layout for the Gamma Camera area, which was included in the bid documentation, includes an Uptake/Injection Room that does not comply with the current VA Guidelines. VA Guidelines require a 12’ x 12’ room with specific features such as cabinetry, countertop, infusion recliner, medical gas, patient lift system, IV stand, injection stand, infectious waste disposal, computer workstation, and vital sign monitor. The existing width of the current layout cannot accommodate these features. Please advise if the VA intends to forgo these features or if more space will be provided to expand the size of the injection room.
A: It is acceptable that the design will differ from the VHA Nuclear Medicine Design Guide for Injection Room. The required room size is specified in SOW.
1. Q: The width of the injection room in the 2021 proposed layout for the Gamma Camera area does not comply with the accessibility requirements as required by ANSI 117. Please advise if the VA intends to forgo this requirement or expand the size of the injection room.
A: It is acceptable that the design will differ from the VHA Nuclear Medicine Design Guide for Injection Room. The required room size is specified in SOW.
1. Q: VA Guidelines also require Injection/Uptake Rooms to have an accessible patient bathroom connected to the uptake room. The space limitation of the existing proposed layout from 2021 does not allow for the inclusion of an accessible bathroom. Please advise if the VA intends to forgo this requirement or expand the size of the injection room.
A: A bathroom is not required.
1. Q: According to the VA Imaging Services Design Guide, released September 1, 2020, the patient uptake room is sized at 12’-0”x12’-0”. It includes several major features, such as a countertop spanning an entire wall with cabinets and storage, a large infusion incliner, medical gas, and a patient lift. Also included in the Design Guide are required equipment such as the injection stand, IV stands, infectious waste disposals, computer workstations, and vital sign monitors. Our current room is approximately 5’-4” wide x 18’-6” deep. We do not have the width to support almost any of this equipment individually; nonetheless, all are in the same room. Additionally, the current width cannot comply with accessibility requirements as required by ANSI 117.
We want to be certain the end user is aware of the space limitations of the proposed injection room 6503D and it has been stated that the proposed injection room cannot comply with the VA Design Guide.
A: It is acceptable that the design will differ from the VHA Nuclear Medicine Design Guide for Injection Room. The end user is aware of the reduced room size.
1. Q: The VA Design Guide incorporated a bathroom inside the exam room; there is no room for that either. Are we to adhere to this requirement?
A: No, a bathroom will NOT be needed as part of this design and construction.
1. Q: When are sf330’s are due, and will there be an interview?
A: SF-330s are due on 12/06/2024 at 2:00 PM EST, together with a proposal for the project – BY EMAIL ONLY to Vladimir.Stoyanov@va.gov. The proposal must have two parts: 1) a breakdown of the A/E services and 2) a labor and material breakdown by division for the construction portion. Interviews will be scheduled only with the top three firms selected.
