About this file
This is an Open Enrollment Benefits Supplement document from the City of Burleson for the 2026 plan year. The document outlines the annual benefits election period for regular, full-time employees working 30 or more hours per week. Open Enrollment runs from October 20 through October 31, 2025, with elections taking effect January 1, 2026. Enrollment is active, meaning current coverage will not automatically roll over and employees must affirmatively elect benefits to maintain coverage. Employees can enroll through mybentek.com/cityofburleson. Dependent eligibility verification documentation must be submitted within 30 days of adding dependents. The city offers medical, dental, health savings accounts (HSA), flexible spending accounts (FSA), short-term disability through Symetra, pet insurance through Spot, 457(b) and Roth retirement plans through Voya, employee assistance programs, and voluntary benefits including gym memberships and discounts through BenefitHub and TicketsatWork.
Medical and dental premiums are increasing 5 percent for both employee and City contributions in 2026. The health premium discount will increase to $48.96 per month for employees completing the Annual Wellness Form. HSA annual limits are increasing to $4,400 for individual coverage and $8,750 for family coverage. Enhanced paid parental leave eligibility has expanded to employees with less than one year of service, effective October 1, 2025. The UHC prior authorization program for prescription medications is expanding January 1, 2026, with UHC notifying affected employees before the plan year begins. Additional benefits include enhanced first responder health services, one paid personal day annually, catastrophic leave donation programs, and will and estate planning resources through Alliance Work Partners and LegalShield. Support is available through BenTek Support Line, Human Resources at (817) 426-9642, and UnitedHealthcare's Advocate 4 Me program.

Elect the Coverage You Need for 2026 Open Enrollment is your annual opportunity to make the best choices for your household by electing benefits for the new plan year. This year, Open Enrollment begins Monday, Oct. 20 and will end Friday, Oct. 31 at 11:59 p.m. Your Open Enrollment elections will take effect on Jan. 1, 2026.
Eligibility and Enrollment All regular, full-time employees working 30 or more hours a week are eligible to enroll in benefits during Open Enrollment.
Enrollment is active this year, which means your current coverage will not roll over to the 2026 plan year. You must enroll to have coverage next year.
Flexible spending account (FSA) enrollment and health savings account (HSA) elections will not roll over—you must take action during Open Enrollment. If you do not take action by the deadline, FSA members will not have an account and HSA members will only have the City contribution to their HSA for the 2026 plan year.
How to Enroll:
• Beginning Oct. 20, you can visit mybentek.com/cityofburleson to make your benefit elections.
• Click on the Menu button and select “Employee Benefits
Center.” Then, select “Open Enrollment.”
Dependent Eligibility Verification If you are adding dependents to your insurance coverage, you must submit copies of documentation verifying their eligibility (marriage/birth certificate, Social Security number, etc.) in the BenTek Document Center or to Human Resources within 30 days.

EXPANDED PRIOR AUTHORIZATION LIST
The UHC Prior Authorization program works to save you money on pharmacy costs by ensuring that prescribed medications are medically necessary, meet the plan coverage and are used effectively.
On January 1, 2026, we will be expanding our prescription prior authorization program through UHC. This means new prescriptions will be added to the prior authorization list. If you have a prescription that will now require pre-authorization, UHC will notify you before the 2026 plan year begins. You do not need to take any action—your provider will obtain the necessary information and approval for any eligible medications.
Note: This change was originally scheduled to take place January 1, 2025, but was delayed. You may have medications that were not affected, but may now require prior authorization in 2026.
