About this file

This document is a Group Critical Illness Insurance Plan brochure from Colonial Life & Accident Insurance Company for the City of Burleson, Texas, effective May 18, 2022. The plan is a voluntary worksite product that provides critical illness and cancer coverage to employees and eligible family members. The plan offers face amounts of $10,000, $20,000, or $30,000, with benefits payable upon diagnosis of covered critical illnesses including heart attack, stroke, end-stage renal failure, major organ failure, coma, permanent paralysis, blindness, and occupational infectious diseases at 100% of the face amount. Coronary artery bypass graft surgery and coronary artery disease are covered at 25% of the face amount. Subsequent diagnoses of different critical illnesses are covered at the original percentage, while subsequent diagnoses of the same critical illness are covered at 25% of the original face amount, provided diagnoses are separated by at least 180 days.

The plan also provides cancer benefits, including 100% of the face amount for diagnosis of internal or invasive cancer, 25% for carcinoma in situ, $500 for skin cancer, and $50 for FDA-approved cancer vaccines. The policy excludes benefits for critical illnesses and cancers resulting from alcoholism, drug addiction, felonies, psychiatric conditions, suicide, intentional self-harm, war, or pre-existing conditions unless the covered person satisfies the pre-existing condition limitation period. Cancer benefits are excluded for diagnoses or treatments outside the United States, Canada, or Mexico, and for pre-existing conditions unless the limitation period is satisfied; however, no pre-existing condition limitation applies to dependent children born or adopted while coverage is in effect. The plan requires applicants to be actively employed at least 20 hours per week, non-tobacco users within the last 12 months for non-tobacco policies, and spouse/dependent coverage limited to no more than 50% of the employee's face amount. Face amounts reduce by 50% on the certificate anniversary date after the insured's 75th birthday.

NOTICE TO APPLICANT REGARDING REPLACEMENT OF ACCIDENT AND SICKNESS INSURANCE

According to your application and the information you have furnished, you intend to lapse or otherwise terminate existing accident and sickness insurance and replace it with a policy to be issued by the Company. For your own information and protection, you should be aware of and seriously consider certain factors which may affect the insurance protection available to you under the new policy.

1. Health conditions which you may presently have, may not be immediately or fully covered under the new policy. This could result in denial or delay of a claim for benefits under the new policy, whereas a similar claim might have been payable under your present policy.

2. Any benefits you have accrued under your present policy may not be payable or transferrable under the new policy.

3. If you are transferring from individual coverage to group coverage, the circumstances under which you may continue your coverage may be limited or no longer available.

4. You may wish to secure the advice of your present insurer or its agent regarding the proposed replacement of your present policy. This is not only your right, but it is also in your best interests to make sure you understand all the relevant factors involved in replacing your present coverage.

5. If, after due consideration, you still wish to terminate your present policy and replace it with new coverage, be certain to truthfully and completely answer all questions on the application concerning your medical/health history. Failure to include all material medical information on an application may provide a basis for the Company to deny any future claims and to refund your premium as though your policy had never been in force. After the application has been completed and before you sign it, reread it carefully to be certain that all information has been properly recorded.
