About this file

This is a Vendor's Statement form pursuant to Kentucky Revised Statute (KRS) 45A.343 issued by the City of Owensboro, Kentucky Purchasing Department. The form serves as a compliance certification document that vendors must complete when contracting with the City of Owensboro, requiring attestation regarding violations of specific Kentucky statutes including corporate taxes (KRS 136), sales and use taxes (KRS 139), income taxes (KRS 141), wage and hour laws (KRS 337), occupational safety (KRS 338), unemployment (KRS 341), and workers' compensation (KRS 342). The document is dated for 2025 completion and includes signature fields for company name, authorized officer signature, and title.

The form establishes three key compliance requirements: vendors must certify they have not been finally determined to have violated the specified KRS chapters within the five-year period preceding the statement, acknowledge they will maintain compliance with applicable provisions throughout the contract duration, and understand that failure to disclose violations or maintain compliance will result in contract cancellation and two-year disqualification from future City of Owensboro contracts. The document does not specify particular products, services, contract values, or funding sources, as it serves as a general compliance certification form for any contracting opportunity with the City of Owensboro.
