About this file

This is a sworn statement form required pursuant to Kentucky Revised Statute (KRS) 45A.395 for the City of Owensboro, Kentucky. The document serves as a prerequisite for bidders or offerors to be determined as responsible bidders in any procurement process. The form requires completion with company information, signatory details including office or title, signature, and must be dated in 2025, though no specific project details, quantities, response dates, or contract terms are specified in this general compliance form.

The statement requires bidders to attest under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated Kentucky's campaign finance laws, and that awarding a contract to them would not violate these laws. The form defines "knowingly" as being aware or should have been aware of the conduct or circumstances. No pricing terms, set-asides, incumbent information, or funding details are included as this is a standardized legal compliance document that would accompany specific procurement opportunities from the City of Owensboro's Purchasing Department.
