About this file

This is a cost sheet for a Salt Lake City bid (No. SLCI26012) for the rental of trench shoring equipment. The bid solicits pricing for various construction-related rental items including steel roadway plates in multiple sizes, portable shoring rental parts, shore pump components, release tools, vertical shoring, adjustable struts, and trench box rental parts. The document indicates that bidders may submit pricing for any and all listed items, with the award to be made to the lowest responsible and responsive bidder.

The cost sheet requires bidders to complete yellow-shaded cells with pricing, with clear instructions that blue and green cells should not be altered. Pricing must include all associated costs such as materials, supervision, labor, insurance, transportation, delivery, and fuel. Delivery rates are specified for different time periods (Monday-Friday 7:00 am to 5:00 pm, after 5:00 pm, and weekends). The bid includes an ethics clause requiring vendors to confirm they have not provided illegal gifts to city employees and must comply with Utah's work eligibility verification requirements. Bidders must upload a copy of their completed cost sheet or risk being deemed non-responsive.
