About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in New Mexico for the On-Call Landscape Architecture, Multidisciplinary Architectural, and Engineering Services solicitation. The form requires offerors and bidders to certify that neither they nor their principals are currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any federal, state, or local department or agency. Offerors must also certify compliance with various responsibility standards and ethical requirements established by the County Procurement Code.

The certification requires offerors to declare that within the three-year period preceding submission, they have not been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records; have not violated contract provisions or demonstrated unsatisfactory performance; have not been indicted or criminally or civilly charged with enumerated offenses; are not current or former County employees or immediate family members of County employees or public officials; and have not had public transactions or contracts terminated for cause or default. Offerors unable to certify any statements must provide written explanations with their submission. Falsification of any statement on the form constitutes grounds for non-consideration of the proposal or rescission of any contract award.
