About this file
This document is a Conflict of Interest Questionnaire (CIQ) issued by the City of Fort Worth Purchasing Division for Invitation to Bid (ITB) Reference #25-0213, titled "Signs, Banners, and Installation". The document is a standard form required by Chapter 176 of the Local Government Code, which mandates that any person or agent seeking to contract with a local governmental entity must disclose potential conflicts of interest. The form must be filed with the City of Fort Worth Secretary no later than seven days after the person begins contract discussions, negotiations, or submits a bid or proposal.
The questionnaire requires vendors to provide detailed information about their business relationships with local government officers, including any employment connections, potential financial interactions, or gift exchanges that might constitute a conflict of interest. Specifically, vendors must disclose if they or their family members have received taxable income exceeding $2,500 from the local government entity in the preceding 12-month period, or if they have given gifts to government officials valued at more than $100. The form is designed to ensure transparency and prevent potential conflicts that could compromise the integrity of government procurement processes.
