About this file

This is an Environmental Site Records (ESR) Rolling Consultation Sheet template prepared for the State of Utah Division of Purchasing in connection with archaeological survey work for the Stookey Fire restoration project affecting BLM and Utah Trust Lands. The document provides a standardized framework for documenting archaeological sites across three survey sessions, with instructions for archaeologists to record site numbers, types, dimensions, artifact counts, features, fire effects, condition assessments, and cultural resource eligibility determinations. The file includes example entries demonstrating proper completion procedures, such as a Historic Trash Scatter with 89 artifacts and a structural footprint representing a potential Navajo Hogan, and a Lithic Scatter comprising 20 flake chert materials without diagnostic tools. Each entry requires notation of direct or indirect fire effects, preliminary eligibility determinations, State Historic Preservation Office (SHPO) concurrence, and implementation recommendations such as site avoidance during project execution.

The contract is issued by the State of Utah Division of Purchasing on an Invitation for Bid (IFB) basis restricted to vendors on the approved archaeology vendor list (AVL). No mandatory site visit is required for bid submission. Award will be made to the highest-scoring responsive and responsible vendor under the established AVL for Archaeology scoring system. Vendors must guarantee pricing for the entire project duration. The ESR Rolling Consultation Sheet serves as the primary documentation tool for archaeological fieldwork, enabling Fuels Archaeologists to make determinations regarding site eligibility and appropriate treatment actions prior to project implementation.
