GENERAL REQUIREMENTS SPECIFIED IN THIS SECTION REPRESENT A BASELINE STANDARD.

THEY ARE OPERATIVE FOR PROJECTS PERFORMED FOR SCOTT AIR FORCE BASE TO THE

EXTENT APPLICABLE AND NOT IN CONFLICT WITH OTHER, MORE STRINGENT PROJECT-

SPECIFIC REQUIREMENTS, WHICH WOULD BE GOVERNING.

1. GENERAL

1.1. Description of Work: The contractor shall furnish all labor, equipment, tools, materials, supervision, and other items and services necessary to accomplish what the Statement of Work (SOW) and/or contract issued pursuant to it, calls for to be performed and provided (the work). All work done shall conform to:

the terms of the contract/task order; applicable government laws, regulations (both federal & state of authorities having jurisdiction) and all applicable codes including, building, renovation, energy, electrical, mechanical, landscape, IDOT, ANSI, FAA etc. individually referenced or not; Air Force Instructions, UFC, manufacturers’ instructions; and generally-accepted standards in relevant design professions and building trades.

1.2. Complete Project: Documents the government provides to the contractor describing a project’s design intent and the work are meant to convey what the government expects and requires from the work and to define a complete project. The contractor is responsible to resolve any uncertainty before committing to do the work. Thereafter, any omission of express reference to specific services or materials reasonably inferred to fall within the project scope, or that are commonly understood within the applicable design profession or building trade as necessary to properly complete a project, will not relieve the contractor of the responsibility to perform or provide such at no additional cost to the government.

1.3. Types of Work and Delivery Contracts: As used herein, the types of work are: design of a project (Design); construction of a project designed by others (Construction); or design and construction of a project under the same contract or task order (Design/Build).

1.4. Location: Facilities to be constructed or altered are located at Scott Air Force Base, Illinois. The base is located approximately 20 miles East of St. Louis, Missouri, just South of Interstate Highway 64.

1.5. Standard Workday/Week: Work shall be accomplished during the standard 8-hour day, 5- day week, Monday through Friday, between the hours of 7:30 a.m. and 4:30 p.m. local time, excluding federal legal holidays and non-work days resulting from holidays occurring on weekends.

If desiring to work other than within a normal workday, the contractor shall submit a written request at least three (3) work days prior to the proposed performance of work to the Contracting Officer (CO) and coordinate with the Civil Engineer Squadron (CES) Project Manager (PM) stating the proposed performance of work, day/days, time/amount of work, and reason why permission should be granted to perform work during non-standard hours.

Scott Air Force Base observes the following federal legal holidays on the day shown on the Friday before if it falls on a Saturday or the Monday after if it falls on a Sunday:

New Year’s Day 1 January Martin Luther King Day Third Monday in January President’s Day Third Monday in February Memorial Day Last Monday in May Juneteenth 19 June Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veteran’s Day 11 November Thanksgiving Day Fourth Thursday in November Christmas Day 25 December

1.6. Project Superintendent: Per FAR 36.506, the contractor must employ a full-time superintendent for the project on job site. The superintendent’s duties shall include the overall management of the project, coordination of the sub-contractors’ work, and liaison with the CO and PM. At all times during performance of this contract and until the work is completed and accepted, the Contractor shall directly superintend the work or assign and have on the worksite a competent superintendent who is satisfactory to the Contracting Officer and has authority to act for the Contractor. The superintendent may not manage more than one project site at a time without approval from the contracting officer. A competent representative is as defined by the Occupational Safety and Health Administration (OSHA) Standards for Construction Industry part 1926.32, Definitions (29 C.F.R. 1926 OSHA). This paragraph supplements FAR clause 52.236-6, Superintendence by the contractor.

1.7. 24-Hour Point of Contact: The contractor shall provide the CO with a 24-hour number or numbers where its authorized representative can be contacted at any time day or night. This number shall be provided at the pre-construction meeting, but not later than the first day of contract work at the site or ten (10) calendar days after the Notice to Proceed date.

1.8. Site & Storage Area Maintenance: The contractor is responsible for maintaining the construction site and storage area(s) daily. This includes, but is not limited to, lawn mowing, ice and snow removal, and disposal of trash and debris. Keep fencing in a state of good repair and proper alignment. Grassed or unpaved areas, which are not established roadways, will be covered with a layer of gravel as necessary to prevent rutting and the tracking of mud onto paved or established roadways; gravel gradation will be at the contractor’s discretion. Mow and maintain grass located within the boundaries of the construction site for the duration of the project. Grass and vegetation along fences, buildings, under trailers, and in areas not accessible to mowers will be edged or trimmed neatly.

Cut grass (or annual weeds) within the construction and storage sites to a maximum 4 inch height at least once a week during the growing season unless the grass area is not visible to the public. Trim the grass around fences at time of grass cutting. Maintain grass or weeds on stockpile earth as described above.

Remove construction debris, waste materials, packaging material and the like from the work site daily.

Any dirt or mud which is tracked onto paved or surfaced roadways must be cleaned away daily. Store within the fenced area described above or at the supplemental storage area any materials resulting from demolition activities which are salvageable. All material should be neatly stacked for storage.

1.9. Disposition of Removed Government Property: All materials and equipment to be removed by the contractor and not reinstalled under this contract, including debris generated by the project work, shall be removed at regular intervals from the site and disposed of off- base by the contractor. Such disposal shall be in conformity with applicable federal, state, and local pollution laws and regulations and at the contractor’s expense.

1.10. Excavations & Landscaping: The contractor shall backfill all excavations within 7 calendar days unless ongoing work in an excavation precludes such action. In no event shall the contractor stockpile removed earth material on the construction site unless it is to be used in another portion of the same project. For all disturbances to grounds, the contractor shall complete final grading within one (1) week of the disturbance/backfilling. The contractor shall complete final landscaping or seeding within three (3) weeks of the beginning of the planting season if possible or as soon as possible when suitable conditions exist.

Scarify existing subgrade and provide 4 inches of topsoil for newly graded finish earth surfaces and areas disturbed by the contractor. Topsoil shall not be placed when the subgrade is frozen, excessively wet, extremely dry, or in a condition otherwise detrimental to seeding, planting, or proper grading. Topsoil shall be provided with the following characteristics: vegetative sustaining soil from the A horizon with up to 10% organic matter, a maximum particle size of 3/4" with a maximum 5 percent retained on the 1/4" sieve and otherwise free of sticks, stones, roots and other debris and objectionable material.. Provide seed at 5 pounds per 1000 square feet and match existing vegetation. Provide CID A-A-1909, Type I, Class 2, 10-10-10 analysis fertilizer at 25 pounds per 1000 square feet. Provide straw matting to protect newly seeded areas and stake as required. Water newly seeded areas to establish seed germination and an acceptable stand of grass. Sod shall be provided in flight line areas. In accordance with the SWPPP and all areas subject to erosion, control methods shall be implemented and maintained in conformance with Federal Guidelines and State of Illinois Department of Transportation Standards.

The contractor shall coordinate erosion mat installation and review with CES PM. Any erosion mats that become unattached due to improper installation will be subject to a warranty claim.

1.11. Brand Names: References in specifications to any brand name equipment or materials shall be regarded as establishing a standard of quality and – to the extent not conflicting with 2.3 of these General Conditions, regarding substitutions – shall not be construed as limiting competition. The contractor may submit proposed equal equipment and/or materials for government consideration during the bidding process. Equal equipment after the award of the contract must be approved by the Project Manager, approval is not guaranteed.

2. SUBMITTAL REQUIREMENTS

2.1. Schedule of Material Submittals: The contractor is responsible for developing and submitting a material submittal schedule using the government-provided format (e.g. AF form 66), to the CO for approval. The contractor shall identify all planned submittals and dates, and shall submit the schedule to the CO within 14 calendar days after contract award or within another agreed-to date as directed by the

CO.

2.2. Submittals: The contractor shall submit all items; whether as a one-time submittal or periodic reports;

as required by the solicitation documents, specifications and submittal register to the CO for approval.

Material submittals shall comply with Unified Facilities Criteria (UFC) http://www.wbdg.org/ffc/dod/unified-facilities-criteria-ufc, the Scott Air Force Base – Installation Facilities Standards (IFS) (6 September 2019), https://www.wbdg.org/FFC/AF/AFIFS/Scott_AFB_IFS_Final_2019.pdf, all other design guides applicable to this project, and comply with all contract and solicitation documents.

The CO may request additional submittals when deemed necessary to adequately describe the intended work.

Each submittal shall include an original AF form 3000, Material Approval Submittal and supporting documentation. The government may request hard copies of submittals, in lieu of or in addition to electronic copies when applicable. The availability and practicality of submitting electronically versus hardcopy should be discussed and agreed to during the initial pre-construction conference after award.

In general, submittals shall be provided a minimum of 14 calendar days in advance of scheduled purchase to allow sufficient time for government review. Additionally, unless stated otherwise, submissions of test results and other submittals of that nature shall be made within 14 calendar days, after the work or test is performed. Samples of all finish items to be submitted on a Form 3000.

Each submittal shall be complete and in sufficient detail for ready determination of compliance with the contract requirements. Submittals shall include, but not limited to, items such as contractor or manufacturers shop drawings, descriptive literature, catalog cuts, diagrams, operation charts or curves, certifications, samples, test reports, test cylinders operating manuals, installation procedures and recommendations, templates, and other descriptive material. Catalog cuts and other descriptive data which may have more than one model or type shall be clearly marked to show the model, type, size, and all optional accessory equipment the contractor proposes to install. Submittals on component items forming a system or interrelated items shall be submitted at one time as a single submittal in order to demonstrate that the items have been properly coordinated and will function as a unit. Submittal items which are disapproved shall be promptly corrected and resubmitted within 10 calendar days by the contractor. No material shall be installed until the submittal is approved by the CO. Approved submittals shall be available at the job site for inspection by the PM and the inspector at all times.

Material samples shall be submitted for all color selections, patterns and other aesthetic attributes of finish materials. Printed charts will not be an acceptable material submittal. Finish material for interior application and exterior application, shall each be submitted at one time.

2.3. Substitution of Materials & Equipment: If the contractor proposes to use materials, equipment or accessories other than those specified, the contractor shall supply the following data for justification:

scaled plans and elevation drawings identifying all applicable pipe locations, electrical connections, penetrations, equipment data and dimensions, and all other supporting equipment/materials associated with the proposed substitute; all required catalog cut sheets and/or descriptive data and a written justification for the proposed substituted item. The CO reserves the right to accept or reject any or all of what is proposed to be changed based on the design intent conveyed in the drawings and specifications, and the completeness and clarity of the request submittal.

For reasons of compatibility, and experience with certain critical existing equipment, systems, components, materials etc., the government will not entertain requests to substitute others for those identified as “base standard” in project drawings or specifications, e.g., the base standard lock cylinder core is by Stanley-Best Access Systems and, unless otherwise noted, no submittal providing for a similar http://www.wbdg.org/ffc/dod/unified-facilities-criteria-ufc https://www.wbdg.org/FFC/AF/AFIFS/Scott_AFB_IFS_Final_2019.pdf core by another manufacturer will be approved.

It shall be understood and agreed that any substitution of materials and/or equipment, if approved, will be made by the contractor at no additional cost to the government in consequence of it, regardless of any changes in connections, spacing, electrical service, or appurtenances the substitution may require.

2.4. As-Built Drawings & Specifications: The contractor shall submit plans and elevations drawings if the contractor purposes a piece of equipment and/or accessories on other than that specified. During the course of construction, the contractor shall keep an accurate record of differences in as-built drawings from the original designed or specified, by means of editing computer-aided design (CAD) files of contract drawings and word-processing files of specifications, saving them as separate files without overwriting the originals. When available, the government may provide record drawings of applicable facilities for use by the contractor to aid in developing as-built drawings. Drawings shall be editable in AutoCAD 2018 or compatible and specifications in Microsoft Word 2016 or compatible. The contractor is responsible for providing any computer hardware and software required to prepare as-built drawings and specifications.

All variations from the contract drawings and specifications, including those made in consequence of the selection of optional or alternate materials, shall be clearly indicated. They shall be shown in the same general detail utilized in the original versions. The as-built drawings and specifications shall be kept current during construction and readily available for review by the PM and construction inspector at all times via hard copy. The contractor shall provide the 99% as-built drawings, including any pre-final punch list items, via an electronic submittal that must be approved by the CO before the final inspection will be scheduled. Upon completion of the final inspection, the final as-built drawings shall be furnished to the CO not later than 14 calendar days after the final inspection date and prior to the issue of the final payment to contractor.

As part of the as-built drawings submittal, a separate set of drawings shall be marked showing areas containing existing/remaining asbestos-containing materials and lead-based paint and methods of encapsulation, as applicable.

2.5. GeoBase Requirements: All geospatial deliverables shall be submitted no later than 30 calendar days after project completion date. All geospatial data shall conform to the following requirements:

The contractor shall use conventional surveying and other methods, such as total station or Global Positioning Systems (GPS) for field data collection at an accuracy level of +/- 2cm. All survey data collected shall be provided to the government in a digital format with an attached survey report identifying survey method, equipment list, calibration documentation, survey layout, description of control points, control diagrams, and field survey data. The contractor shall identify the horizontal and vertical order, classification, ID number, elevation, coordinate location and any other necessary attributes for all surveyed features. All geospatial data shall overlay on the installation’s most current orthorectified imagery (orthomosaic) provided by Scott AFB. Spatial data shall be established and submitted in the Illinois West State Plane Coordinate System, NAD83, Feet. The contractor shall prepare and submit an ESRI, ArcGIS 10.6.1 or higher format geodatabase. The contractor shall utilize ESRI ArcGIS 10.6.1 topology rule to ensure the following:

• No erroneous overshoots, undershoots, dangles or intersections in the line work.

• Lines should all be continuous (i.e. do not create dashed lines with many small line segments).

• Point features should be digitized as points not graticules, cells, symbols or icons.

• All polygons completely close and have a single unique centroid

• Digital representation of the common boundaries for all graphic features must be coincident, regardless of feature layer.

Submittal data shall be in compliance with the Spatial Data Standards for Facilities, Infrastructure and Environment (SDSFIE). Entity naming conventions, attribute fields, and domain names will be collected in the format defined by the CADD/GIS Technology Center’s Spatial Data Standards release SDSFIE-V

Feature Attributes: The contractor shall identify the classification, type, location, ID number, and any other necessary attributes, specified by the government, for all surveyed, mapped, designed, or proposed features. Metadata should be submitted in ESRI ArcGIS 10.6.1 Format using the Federal Geographic Data Committee (FGDC) Content Standards for Digital Geospatial Metadata (CSDGM) and/or current Geobase standards. The contractor shall complete all metadata elements marked mandatory and mandatory-if-applicable as defined by the FGDC CSDGM for each layer collected. This standard document can be found at: http://www.fgdc.gov/metadata/csdgm.

The contractor shall be responsible for providing the updated data in the aforementioned specifications.

When available, the Government may provide example key feature datasets, metadata formats, imagery, and misc. data to assist the contractors in the submission of the required geospatial deliverables. The CES geospatial group can be available to answer questions concerning the deliverables and will not be held responsible for any type of training the contractor’s require to complete the deliverables in the required format. The 99% complete data, including any pre-final punch list items, shall be approved via an electronic submittal (CD) before the final inspection will be scheduled. Any changes to this data as a result of the final inspection shall be furnished to the CO not later than 14 calendar days after the final inspection date and prior to the issue of the final payment to contractor.

2.6. Real Property Records: Prior to the pre-final inspection, the contractor will coordinate with the PM to provide all pertinent data regarding the project, in order to complete a DD Form 1354. If information is not provided for the PM to complete the DD Form 1354, pre-final will be rescheduled.

2.6.1. At the pre-final inspection, a representative from the Real Property Office will do a thorough walk-through and annotate any findings or corrections required.

2.6.2. If Beneficial Occupancy is taken at any point during the project, an interim 1354 must be completed at time of beneficial occupancy.

2.6.3. The contractor shall prepare an accounting of all installed/constructed property, by facility number (if multiple facilities are included in the project/task order).

2.6.4. The contractor shall list government-furnished/contractor-installed/removed items and contractor-furnished/ contractor-installed items.

2.6.5. The contractor shall list items including, new or removed, but not necessarily limited to, the following:

2.6.5.1. Electrical equipment (special lighting fixtures, metering equipment, overhead door operators, exterior floodlights and poles, fire alarm and burglar alarm systems, etc.)

2.6.5.2. Mechanical equipment (commercial refrigeration equipment, heating equipment, air conditioning systems equipment, pumps, boilers, sprinklers, meters, and other items of mechanical equipment)

2.6.5.3. Other items that are considered to be real property installed equipment.

2.7. Builder Data: At the completion of construction and before final payment, the following information, by building number, shall be provided by the contractor to the CO.

2.7.1. The contractor shall provide BUILDER data for government validation at least 30 calendar days prior to O&M handover walk-through (conducted prior to contract close out). Following government validation of the BUILDER data the contractor will provide the government two copies of CD-ROM media with an electronic copy of the excel database file for the entire facility as part of the final close out document package.

2.7.2. List complete nomenclature of all removed and installed items:

2.7.2.1. Manufacturer.

2.7.2.2. Model Number.

2.7.2.3. Serial Number.

2.7.2.4. Electrical rating, full nameplate data.

2.7.2.5. Capacity (MBTU, KVA, Tons, Sprinkler Heads, Etc.).

2.7.2.6. Cost of each item (Equipment cost only)

2.7.2.7. Other pertinent information (Warranty/guarantee information-start date, stop date)

2.8. A/E Design-Build Submittals: The contractor shall hire an Illinois licensed A/E in order to submit the following design submittals to 375 CONS and 375 CES for projects requiring a formal design.

2.8.1. Submit list of design-related items the contractor needs to complete design in a timely manner.

List shall include detailed description of action item and name of individual or agency responsible.

2.8.2. Unless otherwise specified, submit design documents for 35%, 65%, and 95% reviews must include one (1) full size set and two (2) ½ size sets of plans, and two (2) CD’s in AutoCAD and PDF. Include on each CD, specifications, reports, studies or design analysis and supporting engineering data in Microsoft Word and PDF. Allow up to twenty-one (21) calendar days for Government review and comment for each submittal and then CONS will schedule a Government review meeting, NET five (5) business days following government review, to be held on base after each submittal. The Government will pay no additional compensation to the Contractor for the incorporation of comments. The Contractor shall respond to all comments. The contractor’s response shall identify action taken with drawing number or specification paragraph. If the contractor disagrees technically and does not intend to comply with the comment, the contractor must give the reasons for non-compliance within five (5) business days after close of the review period in order that the comment can be resolved. If the contractor believes the action required by any comment exceeds the requirements of this contract, they should “flag” the comment as being outside the design scope, and add a specific reference to the scope item the contractor believes defines the scope, and a statement with what the contractor believes to be "within" the scope for that item. Government directed changes to the scope will be addressed via the changes clause of the contract. Disputes will be resolved in accordance with the disputes clause of the contract.

2.8.2.1. The 95% drawings should be of the completeness ready for seals/signatures and is intended to ensure all previous comments have been addressed.

2.8.3. Submit 100% or “for-construction” design documents to include one (1) full size set and two (2) ½ size sets of plans with seals, signatures, stamps and dates, and two (2) CD’s of AutoCAD and PDF with seals, signatures, stamps and dates. Include on each CD, specifications, reports, studies or design analysis and supporting engineering data in Microsoft Word and PDF. No government review meeting will be required.

2.8.4. Upon request by the government, submit shop drawing list and shop drawings on equipment and finishes for review.

2.8.5. Submit as-built drawings to include one (1) ½ size set of plans, and two (2) CD’s of AutoCAD and PDF plans with seals, signatures, stamps and dates, include on each CD, specifications, reports, studies or design analysis and supporting engineering data in Microsoft Word and PDF with seals, signatures, stamps and dates, along with two (2) CD’s of O&M manuals.

2.9. Construction Rubble/Debris Report: The contractor will provide a quarterly report (Jan- Mar, Apr- Jun, Jul-Sep, and Oct-Dec) by the 10th of the following month and at the completion of construction detailing disposition of debris and ruble generated and removed from the site. The report will give weights in tons for materials taken to a landfill, materials recycled, and materials reused.

2.10. Photographs: When permitted, the contractor shall provide electronic photos prior to beginning actual construction and at two (2) week intervals thereafter for the duration of the project’s construction. The contractor shall coordinate with Public Affairs (618-256-4206) and the facility user prior to taking any photographs of the construction area. Photographs shall be taken at different angles/locations so that the existing conditions, along with the construction progress, are fully represented. Photographs shall be taken inside and outside for all projects involving interior and exterior work. Photos shall be submitted electronically. The photograph file name/title shall include a general description of the photo along with the date the photo was taken. Photos shall be submitted in accordance with the submittal schedule.

3. WORK COORDINATION

3.1. General: The contractor shall coordinate all work schedules with the PM and the construction inspector prior to the start of work. The contractor shall notify the PM and the construction inspector five (5) work days prior to moving onto the work-site to commence work. The contractor must provide notification the day when moving back to the work-site after an interruption of work of two (2) or more work days.

3.1.1. Before initiating construction, the contractor shall meet with the CO, PM and inspector and agree on:

3.1.1.1. sequence of procedures

3.1.1.2. means of access to premises and buildings

3.1.1.3. placement of dumpsters and haul routes

3.1.1.4. delivery of materials and use of approaches

3.1.1.5. use of corridors, stairways, elevators, and similar means of egress

3.1.1.6. requirements and means of communication

3.1.1.7. location of partitions and barriers

3.1.1.8. location of eating spaces and restroom for contractor’s employees

3.1.1.9. location of laydown area

3.1.1.10. other matters incidental to the project

3.1.2. Interruption of work: If it becomes necessary to interrupt work activities in buildings and/or areas for construction purposes (except as otherwise described in the solicitation documents), the contractor shall request permission, in writing, from the CO, or designated representative(s), at least ten (10) business days in advance.

3.1.3. Temporary Construction: Any contractor-installed temporary construction intended to prevent interruption of normal work activity or loss of utility services is subject to approval of the Base Civil Engineer (BCE) through the CO.

3.2. Inspection: The contractor shall obtain the construction inspector's approval prior to doing work that will prevent inspection of previous work such as closing walls or performing tests. The request shall be made three (3) work days in advance to permit an inspection of the area before completion.

3.2.1. Contractor shall conduct inspections and tests prior to CO and PM arriving on site to ensure that all systems are working prior to Government inspection. CO and PM may request contractor punch list prior to arrival to ensure this was complete. In the event that the contractor has not conducted the self-inspection, the Government maintains the right to end the Government inspection and request the contractor re-schedule the Government inspection once they have performed their self-inspection.

3.3. Road Closures: If any closure of a road is required, the contractor shall submit a request to do so to 375 CES, three (3) weeks prior to the planned date for the closure. The request shall include a map showing the location of the closure and a suitable alternate route for detouring traffic normally using the road to be closed. The contractor shall provide all signage, personnel and barricades required to properly direct traffic having to detour.

3.4. Contractor’s Trailers or Storage Area: Trailers or storage buildings will be permitted for the duration of the contract, where space is available, subject to the approval of the Contracting Officer. The trailers or buildings shall be in good condition, free from visible damage rust and deterioration, and meet all applicable safety requirements. Trailers shall be roadworthy and comply with all appropriate state and local vehicle requirements. Failure to maintain storage trailers or buildings to these standards shall result in the removal of non-complying units at the contractor’s expense. A sign not smaller than 24 by 24 inches shall be conspicuously placed on the trailer depicting the company name, business phone number, and emergency phone number. Trailers shall be anchored to resist high winds and must meet applicable state and local standards for anchoring mobile trailers. The location must be approved by the CO in advance. All other trailers, storage, and equipment must be stored at a site designated by the inspector.

The government is not responsible for damages or losses. This paragraph supplements FAR clause 52.236-10, Operations and Storage Areas.

Contactor must coordinate with the Instillation Energy Manager to set up a Utility Agreement allowing connection to utility services and metering. All utility payments will be billed to the contractor and the responsibility of the contractor to pay. The installation Energy Manager can be reached at 618-256-1081 or jeremi.savin@us.af.mil.

Trailers or storage buildings must be removed by 7 days after the final inspection and before final payment. Failure to remove trailers or storage buildings may result in relocation of the trailers or storage buildings by the government at the contractor’s expense. Exceptions to this rule must be submitted in writing and approved by the 375 CES and the CO.

3.5. Pesticide Applications: All pesticide applications must be approved by the CO through the base Entomology shop. The Installation Pest Management Coordinator (IPMC) will determine if pesticides are on the approved chemical list for use on Scott AFB. A label and Safety Data Sheet (SDS) of the insecticide must be available for review. All contract employees that apply pesticides must have an Illinois pesticide application certificate/license and be registered in the Integrated Pest Management Information System (IPMIS). Following approval, the contractor shall maintain an inventory listing of all pesticides used on the installation for performance of this contract. The inventory listing shall contain the name of each pesticide used, location of the material, the container size, and location of the SDS. The contractor shall provide a monthly report of pesticides to the IPMC.

3.6. Radioactive Material (RAM): In accordance with AFMAN 40-201 “Radioactive Materials (RAM) Management”, the use of any device (e.g. soil density meters, XRF meters, etc.) which contains RAM must be approved by the Base Radiation Safety Officer, Bioenvironmental Engineering Flight, at (618) 256-7307, at least 30 calendar days prior to bringing RAM onto Scott AFB.

3.7. Backflow Prevention Devices: All newly installed back flow prevention devices must be inspected by a certified inspector within one (1) week of installation and a follow-up inspection performed 3-months later. The contractor shall submit appropriate documentation of these inspections in accordance with the submittal register.

4. PERMITS

4.1. General: The following outlines permits which may be required incidental to design or construction. The list is not a complete list and additional permits may be required. It is the responsibility of the contractor to determine which permits are required, if any; and when they must be applied for, whether listed here or not. The contractor shall submit all permit applications to the CO and pay all permit fees due. Once completed application and fees have been received from the contractor through the CO, environmental permits will be submitted to applicable state agency by the SAFB Environmental Management Office.

4.2. Special Permits Required (as applicable to work):

4.2.1. Emergency Power Generator Permit: The contractor shall contact the SAFB Environmental Management Office, Air Quality program manager, to find out the requirements for an emergency power generator prior to having the generator delivered to the base. The Air Quality program manager will prepare and submit the application upon receipt of permit fee from contractor. The generator shall be certified by the Family Emissions Limits (FELs). The Point-of-Contact (POC) can be reached at (618) 256-2167 or at william.markham.4@us.af.mil.

4.2.2. Hot Work Permit: A USAF Welding, Cutting, and Brazing Permit (burn permit) must be obtained by the contractor from the base fire department prior to conducting any operation that will create a spark or flame. The POC there can be reached at (618) 256- 5130.

4.2.3. Open Burn Permit: If an open burn permit is required, the permit request shall be routed through the SAFB Environmental Management Office, Air Quality program manager, no-later-than three

(3) months prior to actual open burn effort. The Air Quality program manager will prepare and submit the application upon receipt of the permit fee from the contractor. Visible emissions will be covered during open burning to the greatest extent possible. The Point-of-Contact (POC) can be reached at (618) 256-2167 or at william.markham.4@us.af.mil

4.2.4. Notice of Proposed Construction or Alteration: As described in CFR Title 14 Code of Federal Regulations, part 77, when planning to build within 20,000 feet of an active runway or use equipment in construction which will penetrate a 100 to 1 slope from the nearest point of the runway, among other defined circumstances, notice to the Federal Aviation Administration (FAA) is required unless expressly exempted by the FAA or the Scott AFB Community Planner. The contractor is responsible for ensuring that FAA Form 7460-1 is completed and submitted to the FAA at least 45 calendar days prior to the start of any construction and approved prior to the start of the project by the Regional FAA Office. The FAA Form 7460-1 can be submitted either to (Chicago Airports District Office, CHI-ADO-600; 2300 East Devon Avenue, Des Plaines, IL 60018; (847) 294- 7336; FAX (847) 294-7036) or electronically at https://oeaaa.faa.gov/oeaaa/external/userMgmt/permissionAction.jsp. For any construction activities within the boundaries of Scott AFB, FAA Form 7460-1 shall be submitted as an on-https://oeaaa.faa.gov/oeaaa/external/userMgmt/permissionAction.jsp airport case. The contractor shall provide a copy of the submitted form to the PM. If the project involves the use of a crane, the PM must also notify the airfield manager at (618)256-4101 of the dates/times of a crane’s operation at the construction site.

4.2.5. Sewer Permit: Sewer extension, new installation, and connection to the existing sanitary sewer system require a permit. The application shall be signed, certified and stamped by an Illinois Registered Engineer and submitted to the SAFB Environmental Management Office, water permits manager for local coordination, and signatures. CEIEC will submit the final package to the Illinois Environmental Protection Agency, Division of Water Pollution Control. Permit applications with instructions are available at http://www.epa.state.il.us/water/forms.html#permits-wastewater. The POC for specific questions and guidance is the water permits manager at (618) 256-2167.

4.2.6. General NPDES Permit: General NPDES Permit: Storm water discharge from a construction site requires a Notice of Intent (NOI) under the National Pollutant Discharge Elimination System (NPDES) to be submitted to the Illinois Environmental Protection Agency (IEPA) prior to any construction activities by SAFB Environmental Management Office to the IEPA Division of Water Pollution Control. For specific questions and guidance contact the Water Quality Program Manager at (618) 256-2167. The contractor shall provide the following for projects that impact one acre or more of soil disturbance:

4.2.6.1. Storm Water Pollution Prevention Plan (SWPPP): will be written and submitted to SAFB Environmental Management Office for approval as required by IEPA MS4 permit requirements. Lead time is 60 to 75 days prior to the planned date of construction in order for the SAFB Environmental Management Office to submit and receive consultation approvals from IDNR and the IHPO (30-45 days) with follow on completion and processing of the IEPA NPDES Construction General Permit (approximately 30 days).

4.2.6.2. Army Corps of Engineers’ 404/401 permit: application will be submitted to SAFB Environmental Management Office for work performed within the high banks of a channel, including construction of any outfall pipes into a creek (lead time is approximately 60 days for permit application). Coordination with SAFB Environmental Management Office is required to obtained proper signatures for permit submittal.

4.2.7. Water Utility Work: The water utility lines on Scott AFB are owned and operated by American Water Co. The contractors shall coordinate with American Water Co. when conducting any work pertaining to the water utility lines. Compliance with American Water’s requirements does not relieve the contractor from complying with Scott AFB requirements (i.e. – submittals, as-built drawings, coordination, etc.).

4.2.7.1. Water Permit: An application for construction / operation must be submitted to American Water Co. for water main installation and connection. Permit applications, with instructions, from the State of Illinois are available on the IEPA website at http://www.epa.state.il.us/water/forms.html#permits-drinking-water. The contractor may contact the American Water Customer Service at 1-800-422-2782 or infoaw@amwater.com with questions

4.2.8. American Water Work Permit: All work affecting the water utility system, shall be reviewed and a permit for it issued by American Water Co. prior to being done. All work must conform to what is stated on the permit. Water. Applications, with instructions, are available on line at http://www.epa.state.il.us/water/forms.html#permits-drinking- water. American Water has an office on Scott Air Force Base at 34004 New Mexico Street and can be contacted at (618) 744- 9631. Aboveground/Underground Storage Tank Permits: A permit is required to install or work on any aboveground storage tank larger than 110 gallons, to include generator sub-base tanks, and any underground storage tank or associated piping. 4.2.9. Excavation Permit: The contractor shall apply for and obtain a government Excavation Permit, also known as a “digging permit” (Scott AFB Form 1031, AF Form 103, Base Civil Engineer Work Clearance Request), prior to any excavation. The permit can be obtained from the Civil Engineering, Operations – Engineering Services , Building 57, 501 East Winters Street. There is no charge for the permit. The digging permit is required for the government to locate and mark existing underground utilities as well as for the government’s internal notification and coordination. The contractor shall not begin any below-grade work until this permit has been completed and approved. The contractor shall make every attempt to identify and locate all valve and utility shut-off locations for use in the event of accidental damage. The contractor shall note any utilities discovered during excavations not shown or shown incorrectly on the contract drawings (including any erroneous or missing dimensions on government record drawings), and clearly identify those discoveries on the as-built drawings.

The contractor shall initiate the request for a digging permit a minimum of five (5) work days prior to any excavation or ground penetration. The site plan shall clearly delineate the proposed work area and, if within a reasonable distance, the nearest building and cross street to the location of digging, etc. The location of the excavation or ground penetration shall be marked with white, water soluble, spray paint, or white utility marking flags, and the markings maintained until the area is reviewed and cleared by all utility agencies concerned. In the event a representative of a designated utility does not respond, the contractor shall immediately notify the CO. After identified utilities have been cleared through each office affected, a copy of the digging permit will be provided. The digging permit must be on the job site during the excavation process and every precaution taken during excavation to prevent damage to the existing utilities. The contractor must immediately notify Civil Engineering, Operations – Engineering Services and the construction inspector if any utility is damaged during the excavation process. Dig permits and JULIE numbers can be obtained by contacting 375.ces.ceoe@us.af.mil. It is the contractor’s responsibility to maintain all markings for each project.

5. UTILITIES AND SERVICES

5.1. Commercial Utilities: The contractor is permitted to use utility services, including electric, gas, sewer, water, telephone, cable, as its work may require. All temporary utility connections and metering of the contractor utility use shall be performed by the contractor at the expense of the contractor. The contractor is responsible for the cost and usage of temporary connections to the services and for portable sanitary facilities and other requirements. All utility connections must be approved by 375 CES and the CO. The contractor is responsible for the cost of any new or relocation of existing commercial utility lines. The contractor shall contact 375 CES to coordinate utility payments. Any exterior equipment provided under the contract shall be painted according to the Scott AFB IFS. Contractor shall not, under any circumstances, introduce or dispose of hazardous materials into the Scott AFB sanitary sewer system.

5.2. Utility Service Interruptions: All utility service interruptions require the approval of the CO via coordination with the CES PM, with no single interruptions exceeding eight hours. Service interruptions shall be accomplished at other than normal work hours if feasible.

Exemptions to the maximum interruption time of eight hours or an interruption required within the three

(3) week window may be granted by the CO and in coordination with the CES PM.

5.2.1. Water: As a general rule, water distribution service shall not be interrupted. All water connections shall be made via “hot” tap on the main and shall be observed by American Water personnel.

American Water is responsible for determining what constitutes sufficient reason to interrupt water service. Interruption of water service due to scheduled construction work involving water system components shall be coordinated with American Water at least 14 days in advance to allow for proper water outage planning and water outage notification to affected parties. American Water staff shall operate all valves and hydrants during any outage plan test and implementation of the scheduled water system outages and service restoration. The contractor performing work requiring the interruption of water service shall provide all material, labor, and equipment required to minimize the extent and duration of a water outage and shall ensure restoration of service in the most efficient and complete manner in accordance with accepted industry standards and practices.

This may require one or more line stops to preclude an extensive outage to multiple facilities. The contractor shall comply with all base and state environmental requirements.

5.2.2. Temporary Water Service Connections: The contractor shall notify American Water two (2) weeks in advance for temporary water service. All temporary water connections shall be performed by the contractor at the expense of the contractor. Where appropriate, backflow devices shall be used.

Disconnection of temporary service lines installed and used by the Contactor or any subcontractor will require complete removal of service line at the contractor’s expense by the project’s completion.

5.2.3. Electric: If for any reason the contractor needs to have a power outage then, the contractor shall submit a request to the PM or the construction inspector three (3) weeks in advance to the planned outage date for approval.

5.2.4. Natural Gas: If for any reason the contractor needs to shut the gas off to a facility the contractor shall submit a request to the PM or the construction inspector three (3) weeks in advance to the planned outage date for approval.

5.3. Abandonment of Sewer/Water Lines: Service lines to be abandoned shall be excavated at the curb stop or corporation stop and the valve closed off. The service shall be physically separated so as to have a minimum of three (3) feet horizontal clearance between the main and the cut service line. The location of the disconnection shall be noted on project as-built drawings, with a copy showing the point of disconnection provided to American Water.

Water mains to be abandoned shall be cut and depressurized, then capped with a mechanical cap or concrete cover. Valves shall be closed and the valve boxes removed. All fire hydrants abandoned shall be fully removed. In the event a shut off valve is not available, the contractor shall install a temporary line stop to enable separation of the water mains. Where settlement is a structural concern, putting flowable fill in the main may be required.

5.4. Enabling/Disabling Fire Alarm Panels: All fire alarm panels shall be properly operating prior to the contractor securing at the end of each workday. To have a fire alarm panel enabled/disabled the contractor will coordinate all work through the PM and construction inspector who will contact Scott AFB fire alarm maintenance contractor (contact Operations Engineering Services, (618) 256-2698) or use a fire alarm contractor of its choice, provided the following Scott AFB requirements are met:

5.4.1. The fire alarm contractor maintains a current NICET2 (National Institute of Certified Engineering Technology) license or current NBFAA (National Burglar / Fire Alarm Association license

5.4.2. Any contractor/sub-contractor working on Scott AFB fire alarm transmitters has a current General Radio Operator License Element 1 & 3 issued by Federal Communications Commission

5.4.3. The Scott AFB Fire Alarm service contractor is present at all final acceptances for fire alarm systems. The day and time of the inspection shall be coordinated 3-5 work days in advance. The contractor shall contact the fire alarm QAE (Operations Support at 618-256-2698), to schedule an acceptance inspection of the fire alarm system. The contractor shall bear all costs associated with enabling/disabling fire alarm panels whether the contractor chooses to use Scott AFB Fire Alarm service contractor or use another fire alarm contractor.
