About this file

This is a URS Post-Retired Verification Form for Independent Contractors used by Jordan School District in Utah to verify whether contractors have retired from the Utah Retirement Systems (URS) and to ensure compliance with state re-employment restrictions. The form requires contractors to provide their name, business name, and sign a declaration confirming their retirement status from URS. The document applies to individuals who will be compensated as independent contractors (1099 employees) paid through the district's Accounting Department, including any contractors, owners, partners, or principals of contracting businesses working with the district.

Contractors who have retired from URS and are returning to work must comply with Utah Code Section 49-11-1207(2) and complete URS Form RTRT-27A (Post-Retirement Employment form) with URS's Retirement Benefits Department before beginning work with Jordan District. Failure to report re-employment to URS constitutes a violation that renders the retiree liable to repay URS any overpayment benefits resulting from non-disclosure. Contractors must contact URS's Retirement Benefits Department at 801-366-7770 prior to commencing work. This verification form must be submitted to the district's Accounting Department along with a W-9 form as a prerequisite to contract engagement.

(business).

If it is discovered that a URS retiree failed to report reemployment to URS on its Post-Retirement

Employment form (Form RTRT-27A) in violation of Title 49, the retiree will be liable to repay URS the amount of any overpayment resulting from the violation to report. See Utah code section 49-

11-1207(2).

You are directed to contact URS's Retirement Benefits Department at 801-366-7770 before beginning work with Jordan District. You will be asked to complete and file a
