About this file
This is a URS Post-Retired Verification Form for independent contractors (1099 employees) associated with Jordan School District in Utah. The form requires contractors to disclose whether they have retired from the Utah Retirement Systems (URS) and, if applicable, provide their signature and date of submission. The form must be submitted to the Accounting Department along with a W-9 form. The document outlines restrictions on reemployment for URS retirees receiving monthly benefits, defining reemployment as work performed either as a payroll employee or as an independent contractor for a participating URS employer if the retiree is listed as the contractor, owner, partner, or principal.
Contractors who are URS retirees are required to contact URS's Retirement Benefits Department at 801-366-7770 before commencing work with Jordan School District to complete and file Form RTRT-27A, the Post-Retirement Employment form. Failure to report reemployment in violation of Title 49 of the Utah Code exposes retirees to liability for repaying URS any overpayment benefits received as a result of noncompliance with Utah Code Section 49-11-1207(2). This verification requirement applies to all independent contractor arrangements with the district.
