About this file

This is a Proposer's General Questionnaire template issued by the University of Texas Health Science Center at San Antonio (UTHSA) Department of Purchasing for the San Antonio Breast Cancer Symposium (SABCS) Audiovisual Services contract opportunity (RFP 745-26-P34). The questionnaire requires proposers to provide comprehensive information about their company, qualifications, and approach to delivering large-scale audiovisual production services for SABCS, a 48-year-old global breast cancer research conference entering its 2025 iteration. The event attracts over 10,000 international attendees with approximately 2,000 presentations across four days at the Henry B. Gonzalez Convention Center in San Antonio. Required services include multi-room support, projection, audio, lighting, presentation management, and live streaming capable of handling simultaneous sessions with plenary audiences exceeding 7,000 attendees. Proposers must complete the questionnaire as part of their RFP response, addressing both questions within this document and additional requirements presented in the main proposal submission.

The questionnaire mandates disclosure of company profile information including legal name, business addresses, years in operation, state of incorporation, employee count, annual revenue, and parent corporation details if applicable. Proposers must provide customer references from at least three organizations with similar contract experience within the past five years, including contact information, project descriptions, and relationship length. Additional requirements include summary resumes for key personnel, disclosure of any litigation or financial defaults, financial statements upon request, ownership information for individuals with 25% or more ownership interest, a detailed work plan with tasks and milestones, service support and quality assurance program descriptions, and a contingency or disaster recovery plan. Proposers must also select one of three options regarding cloud computing services to be used in performing the contract. The document does not specify response deadlines, award dates, contract term, or pricing information, indicating these details are contained in the primary RFP documentation.
