About this file

This is a Non-Collusion Affidavit form for Columbia County, Georgia, required for bidders submitting proposals in response to the Request for Proposal for Disaster Debris Monitoring Services (RFP 2026005-RFP1313). The affidavit serves as a sworn statement certifying that the bidder's submission is genuine and has not been developed through collusive or fraudulent means. The document requires an individual representing the bidding company to attest under oath that neither the bidder nor any of its officers, partners, owners, agents, representatives, or employees has engaged in any collusion, conspiracy, or unlawful agreement with other bidders or interested parties regarding bid prices, overhead, profit elements, or any other aspect of the competitive bidding process. The affidavit must be notarized and submitted with the bid proposal.

The form specifically references O.C.G.A. 36-91-21(e), Georgia state law governing noncollusion in public bidding. Bidders are required to certify that all quoted prices are fair, proper, and untainted by any collusive or fraudulent activity. The affidavit must be signed by an authorized representative of the bidding entity, include the individual's title and company name, identify the specific project being bid upon, and be notarized with the notary's seal and commission expiration date. This document is a standard compliance requirement for public procurement in Columbia County and ensures the integrity of the competitive bidding process for disaster debris monitoring and management services.
