About this file
The document is an Affidavit of Compliance with Section 287.138, Florida Statutes, for the City of Fort Myers, pertaining to contracting with foreign entities of concern. This standard legal form requires a vendor to certify that they are not owned by, controlled by, or principally located in certain specified foreign countries, including China, Russia, Iran, North Korea, Cuba, Venezuela, and Syria. The affidavit is specifically related to the ROWTP Production Wellfield Expansion project, a grant-funded initiative to expand the city's potable water supply wellfield capacity.
The affidavit is designed to prevent the county from entering contracts with foreign entities that could potentially access sensitive personal identifying information. By executing this document, the vendor affirms they do not have significant connections to the listed countries of concern, thereby ensuring compliance with Florida state statutes. The form requires notarization and includes spaces for the vendor's business details, signature, and official notary seal, demonstrating a formal legal commitment to transparency and national security considerations in local government contracting.
