About this file
This document is a draft exhibit for a competitive solicitation issued by the State of Washington Department of Enterprise Services (DES) for a new contract for Forklifts. The exhibit contains detailed specifications and requirements for seven categories of forklifts, including electric, internal combustion, rough terrain, and telehandler models. The specifications cover warranty periods, preventative maintenance, repair services, manuals and training, ordering processes, and other technical requirements. The prospective contract will be available to state and local government agencies within the state of Washington, including counties, cities, and other jurisdictions listed in the document. The contract term is not specified, but the document indicates that the awarded contractor must be able to meet the stated commitments throughout the contract term.
The document does not provide pricing terms or set aside requirements. It is a draft being circulated to gather feedback on the performance requirements and pricing structure prior to issuing the final Request for Proposals (RFP). No incumbent contractor information is provided. The funding sources and amounts for the purchases under this contract are also not included in the document.

| _________________________________ |
Type/print full legal name of bidder company
Instructions:
Note: Bidder must follow the instructions set forth in this Exhibit. Unless Enterprise Services, in it’s discretion, determines that a bidder’s variance from these instructions is immaterial or timely corrected, Bids that do NOT follow these instructions will be deemed to be nonresponsive and rejected.
1. Add bidder’s business name above and follow the instructions below.
2. Bidder Compliance. Review the specifications set forth below in the table and mark either ‘Yes/No’ in the ‘Bidder Compliance’ column depending upon whether bidder’s products meet or exceed the specification. Bidders must mark a response for each specification. If any specification does not have a response in the Bidder Compliance column, the bid will be considered nonresponsive.
3. Assessment. The will be assessed as follows:
a. Pass/Fail Specifications: These specifications are mandatory minimum specifications. Bidder must meet each and all of these specifications. Questions 1-11 are required responses for all categories. If bidder selects ‘No’ for any of these specifications, or bidder’s proposed alternative or partial solution (if applicable) is determined to be insufficient, the bid will be deemed to be nonresponsive and bidder will be disqualified from further consideration.
b. Category Specifications: Questions 12, 13, 14, 15, 16, 17, and 18 apply only to categories 1, 2, 3, 4, 5, 6 and 7, respectively. If you are not bidding on a category, do not mark the question. If bidder selects ‘No’ for any of these specifications, or bidder’s proposed alternative or partial solution (if applicable) is determined to be insufficient, the bid will be deemed to be nonresponsive and bidder will be disqualified from further consideration.
4. Supporting Information. Enterprise Services may request additional information or references to help verify the information provided by bidder in this exhibit. A bidder’s failure to provide such requested information to Enterprise Services within five (5) business days will result in disqualification.
5. Bidder’s Commitments. Please note that, if bidder is awarded a contract, bidder’s responses will be incorporated into the resulting contract. Accordingly, Bidder is cautioned to state only commitments that bidder can meet throughout the term of the contract. An awarded bidder (i.e., contractor) who is not able to meet such commitments throughout the contract term, will be subject to contract termination for breach of contract and damages. Please see Exhibit D – Contract.
