About this file

This is a Reference Questionnaire form issued by Collier County, Florida's Procurement Services Division for use in evaluating firms and key personnel responding to Solicitation No. 26-8644 for Tourism Research Services. Proposers are required to submit three to ten client references from similar projects completed within the past two years, and each reference must be documented using this standardized Form 5. The form requests project-specific information including project description, completion date, project budget, and project duration. Proposals must be submitted electronically by April 8, 2026, at 3:00 PM EST, and all submitted references must be completed and returned with the bid/proposal submittal. The contract term is three years with two optional one-year renewal options.

The Reference Questionnaire evaluates ten performance criteria on a scale of 1 to 10, assessing the referenced firm's ability to manage project costs and schedules, quality of work, consultative advice, personnel management, project administration, communication, risk management, contract compliance, and overall customer satisfaction. Evaluators must provide a total score across all items, with blank responses scored as zero. This reference evaluation process is a component of Collier County's broader selection methodology, which allocates evaluation points across multiple criteria including business plan and methodology (20 points), cost of services (20 points), firm experience and capacity (20 points), specialized expertise of team members (20 points), cover letter and management summary (5 points), certified woman and minority business enterprise status (5 points), and local vendor preference (10 points). The County anticipates historical annual expenditures of approximately $120,000 for tourism research services, though this may not reflect future spending patterns.
