About this file
This document is an Addendum to a Request for Proposal issued by the San Bernardino County Department of Aging and Adult Services–Public Guardian for Older Americans Act Title III-B Senior Supportive Services (RFP HS 25-15). The addendum revises Attachment K, the Letter of Intent form, to include a missing second page and clarify the submission deadline as Thursday, January 29, 2026 at 4:00 p.m. local time. Prospective contractors must submit a completed Letter of Intent by this deadline to be eligible for contract award. The Letter of Intent requires organizations to provide their legal name, Federal Employer Identification Number, DUNS number, and legal form of organization (non-profit, government, or for-profit entity). Failure to meet the January 29, 2026 deadline will disqualify proposers from consideration. The full proposal submission deadline is February 27, 2026 at 4:00 p.m., with contract awards anticipated on June 9, 2026 and an initial contract period running from July 1, 2026 through June 30, 2027, with potential renewal options for three additional one-year periods.
The Letter of Intent requires prospective contractors to certify their qualifications, including a record of satisfactory performance, adequate file and records management capabilities, administrative and fiscal capability, compliance with presentation and participation requirements, ability to register on the County's ePro system, possession of a DUNS number, and minimum three years of continuous experience providing senior supportive services. Contractors must also provide a minimum of three references from customers demonstrating their ability to deliver services as outlined in the RFP. Submission of the Letter of Intent does not guarantee contract award and is not a substitute for the full proposal submission. The total estimated budget allocation for this procurement is $600,000, with funding derived from Title III-B federal funds administered by the California Department of Aging. Contractors must provide matching contributions of at least 11.11 percent of total Title III Federal Area Plan revenue and maintain liability insurance of $1,000,000 commercial general liability per occurrence and $1,000,000 professional liability per claim.
