Note: The quantity of eligible employees enrolled in the vision plan shown in each table above is estimated for the purpose of establishing a quote amount for each performance period. The OCC employee population may fluctuate within each performance period resulting in an increase or decrease throughout the year (i.e., there may be a variation in quantity). The number of eligible employees enrolled in the vision plan is also subject to change. OCC assumes financial risk/cost for all claims.

3.0 BACKGROUND

The OCC is unique from most other federal government agencies because it has independent authority to offer its employees compensation and benefits not offered by other agencies. Any reference to

“employees” here within, means OCC and OFR active employees. The OFR employs approximately 101 employees primarily in the District of Columbia. The OCC, as a governmental plan, is specifically exempted from Title I of The Employee Retirement Income Security Act (ERISA).

The Contractor must provide services in accordance with Subsections 4.0 through 6.0 regardless of the actual number of services or fluctuation in services.

4.0 DESCRIPTION OF SERVICES AND SUPPLIES

4.1 Scope

The OCC seeks an experienced and qualified Contractor to provide vision care insurance services to OCC and OFR active employees, OCC disabled retirees, and eligible family members. The Contractor must be experienced in providing plan administration services for an employer-sponsored vision care insurance program.

In addition, the Contractor must:

• Support OCC wellness initiatives by collaborating with the OCC Wellness Coordinator on ways to promote healthy eye care for employees and their eligible family members. The Contractor must support OCC’s wellness fairs (no more than one per year) at its headquarters location in

Washington, D.C. At a minimum, the Contractor must secure a vendor to provide wellness demonstrations or screenings;

OCC Vision Insurance RFQ #2031JW26Q00058 P a g e 9 | 50

• Attend an onsite, 1-day, health benefits fair during OCC’s annual benefit open season period.

Open season is held over a 4-week period between the months of November and December. The health benefits fair is held at OCC’s headquarters location in Washington, D.C. and the

Contractor is required to attend and provide program materials;

• Prepare a vision program Administration Manual that describes, at a minimum, the enrollment, invoicing, and claims processes. This includes the claims submission requirements and the dispute process for denied claims. The manual must include both the responsibility of the

Contractor as well as that of the OCC. The Contractor must provide the OCC a copy of the manual in electronic format annually. The administration manual will be used by the Human

Resources Benefits staff;

• Produce a plan brochure which will be used as the official statement of benefits. The plan brochure will be accessible to all employees;

• Provide vision program materials in electronic form. Forms must be in a fillable format (e.g., pdf) to include electronic signature for posting on OCC’s Intranet for employee use; and

• Host a biannual online seminar to educate employees on the vision program services/benefits, networks, claims, self-service tools, and resources

4.2 Plan Brochure

The Contractor must produce a plan brochure that, at a minimum, must include the following program information:

• Eligibility

• Enrollment

• How you obtain benefits

• Vision services

• Your cost for covered services

• Claims filing and disputed claims processes

• Definitions of terms

• Exclusion and limitations

• Summary of benefits

• Frequently Asked Questions (FAQs)

The FAQs must include clear, concise answers to common questions about the program and services, organized logically with a search function and relevant links for further information.

4.3 Plan Design

The OCC will offer the benefits as outlined in the plan design below without any deviations by the

Contractor that result in a reduction of benefits to employees. The Contractor must provide vision discount programs. The Contractor must provide key aspects of how the programs work and how members obtain these benefits.

The vision benefits plan design is as follows:

4.4 Eligibility

The OCC will determine eligibility of benefits for employees. The OCC will permit all full-time and part-time employees with an offer of employment that is expected to last for more than one year to participate in the vision program. Employees will have the option, within the first 60 days of their date of hire, to enroll their eligible family members. Then, if the employee experiences a qualifying life event resulting in the addition of eligible family members, the employee will be permitted to enroll their newly eligible family members within 60 days of the qualifying life event. If employees fail to enroll eligible family members within the specified timeframe of their initial eligibility, then employees will be permitted to enroll eligible family members during OCC’s annual open season. Employees will be permitted to cancel the coverage of an eligible family member at any time without having to experience a qualifying life event.

Below is a chart that defines eligible family members of OCC/OFR employees.

Eligible Family Members OCC Definition

Spouse A lawful spouse or domestic partner of the same-sex or opposite-sex

Child An unmarried child under the age of 26 who is the employee’s natural offspring, stepchild, foster child, legally adopted child, child under guardianship, or child of his/her domestic partner

OCC Vision Insurance RFQ #2031JW26Q00058 P a g e 12 | 50

Eligible Family Members OCC Definition

Disabled Child A child with a disability that began before attaining age 26 who is incapable of self-support and has been certified as disabled by a county, state or federal entity or through the Federal Employees Health Benefits Program.

Other than eligible employees actively employed by the OCC and their eligible family members, the

Contractor must include as a covered class for disabled retirees of the OCC who have been approved for a disability retirement by OPM. This covered class (disabled retirees) is not extended to OFR and is applicable to OCC employees only. The Contractor must also include a separate covered class for OCC active employees and their family members and OFR active employees and their family members to separately identify the population.

4.5 Enrollments and Effective Dates

New hires are automatically enrolled. New hires have 60 days from their hire date to add eligible family members to the enrollment record. When an employee experiences a qualifying life event, employees have 60 days from the date of a qualifying life event to make changes to their enrollment record.

Employees have 4 weeks during the open season period to make changes to their enrollment record. Open season is held the second Monday in November and ends the second Monday in December each year.

Enrollments are effective at the beginning of month from the receipt of the enrollment data. Open season enrollments are effective January 1st following the end of the open season period.

Below is a chart that identifies the enrollment opportunities and the corresponding effective date of the enrollment or change.

Immediately upon receipt of OPM approval notification

Day following the employee’s date of separation

Buyout (OCC employees only) Employee terminates from the OCC under a

“severance” type of agreement and continues coverage for

6 months

60 days from the date of separation

Day following the employee’s date of separation

As part of the enrollment process, and to provide an additional level of protection for employees’

The confirmation and welcome packet must be sent to employees at OCC/OFR email address.

4.7 Termination of Enrollment

When an employee’s employment ends, the coverage terminates on the date of separation. A separated employee is not extended the option to continue coverage beyond their date of separation unless one of the following occurs:

1. Employee is approved for disability retirement by OPM;

2. Employee is offered a “buyout” by the OCC that includes benefits

For family members who become ineligible for coverage (i.e., divorce, dissolution of domestic partnership, employee dies, child aging out, or getting married) their eligibility will end on the date they no longer meet OCC’s eligibility requirements. For family members whose eligibility has been voluntarily terminated by the employee, their coverage will end at the end of the month in which the

OCC Vision Insurance RFQ #2031JW26Q00058 P a g e 14 | 50 employee initiated the cancellation of coverage. The OCC is responsible for providing this information to the contractor.

Coverage for a family member will terminate when:

2. The dependent child attains age 26;

3. A dependent child under age 26 gets married;

4. The dependent dies;

5. The employee divorces their spouse or terminates their relationship with their domestic partner;

6. A stepchild or a child of the domestic partner is no longer in a “parent-child relationship” with the employee; or

B. General Requirements

Location. Information collected, used, stored, maintained, or otherwise processed by the contractor in the performance of this contract shall be accessed, transferred, stored or processed only within the United

States. In addition, the maintenance and support operations of the contractor's technology and information must take place, and originate from, within the United States.

OCC Vision Insurance RFQ #2031JW26Q00058 P a g e 20 | 50

The contractor shall not host any portion of the information, information system, infrastructure, or environment in facilities outside the contiguous United States, Alaska, Hawaii, and other U.S. Territories.

Authorization to Use, Store, or Share Sensitive Information. The contractor shall be responsible for properly protecting all information used, gathered, or developed because of work under this contract. The contractor shall also protect all government data, equipment, etc. by treating the information as sensitive.

All information gathered or created under this contract shall be considered as Controlled but Unclassified

(CUI) information. It is anticipated that this information will be gathered, created, and stored within the primary work location. If contractor personnel must remove any information from the primary work area, they shall protect it to the same extent they would their proprietary data and/or company trade secrets.

Confidentiality. The contractor agrees to assume responsibility for protecting the confidentiality of government records and data associated with this contract, which are not public information.

Deliverables. The deliverables in this contract will be considered CUI and shall not be shared with any other organization without prior written approval from the [insert specific OCC role].

C. Security and Privacy Authorization

Authorization Boundary. The contractor shall provide a diagram specific to the OCC, that clearly demarcates the information system authorization boundary and depicts all its connections and components, including the means for monitoring and controlling communications at the external boundary and at key internal boundaries within the system. The contractor shall address all components and managed interfaces of the information system authorized for operation (e.g., routers, firewalls). The diagram must include a predominant border drawn around all system components and services included in the authorization boundary.

If there are OCC-specific components (e.g., components developed or customized for the OCC, OCC-hosted components, etc.) and/or other components supporting this solution that are not in the scope of the contractor's existing security documentation, then identify those components in the diagram as well, as part of a separate OCC-specific boundary since those are unique to OCC and shall be part of the OCC-specific security authorization. The diagram must be easy to read and understand. Formal names of components as they are known at the service provider organization in functional specifications, configuration guides, other documents and live configurations shall be named on the diagram and described. The contractor shall provide a key for symbols used.

External Service Provider Compliance & Monitoring Background. The FISMA and OMB policies require that federal agencies using external service providers to process, store, or transmit federal information or operate information systems on behalf of the federal government, assure that such systems meet the information security protections commensurate with the information and system sensitivity.

Federal agencies are responsible and accountable for the information security risk incurred by the use of information system services provided by external providers.

Such risk is addressed by incorporating the Risk Management Framework (RMF)1 and security requirements for external information systems and services as part of the terms and conditions of the contracts with external providers. Federal agencies can also require external providers to provide appropriate evidence to demonstrate that they have complied with protecting federal information. Prior to the implementation of the contractor's system or services, the OCC ensures compliance with the RMF by issuing an Authorization to Operate (ATO) or Authorization to Use (ATU), based on its review and analysis of the external provider's security documentation (described in detail in the next section) and assessment of customer (OCC's) security responsibilities.

The contractor shall also summarize any deficiencies identified by the auditor in the bridge letter.
