About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance on federal contracts. The form captures comprehensive information across five main sections: contract identification (contractor name, UEI, contract number, type, competitiveness, period of performance, and initial versus final contract costs); customer or agency identification (agency name, description, and geographic scope of services); evaluator identification (evaluator name, title, contact information, and tenure on the contract); evaluation ratings using a four-point scale (Satisfactory, Marginal, Unsatisfactory, Not Applicable); and a narrative summary section requesting discussion of any marginal or unsatisfactory ratings, termination history, and overall assessment of future suitability.

The evaluation criteria span ten technical performance areas (quality of operations, testing, data preparation, quality control, and environmental safety), nine program management areas (contract management effectiveness, cooperation, problem resolution, compliance with objectives, emergency response, subcontractor quality, material and acquisition management, cost-saving initiatives, and process improvements), two transition and phase-in areas (resource transition and continuity of mission support), three employee retention and attraction areas (workforce qualification, retention, and compensation effectiveness), and two small and disadvantaged business participation areas (meeting goals and technical performance management). Completed questionnaires should be returned to Lakenia M. Brown at the mailing address provided (681 Rogers Place, Room 104, West Point, NY 10996).
