About this file

This is a Subcontractor/Teaming Partner Consent Form for the release of past performance information to prime contractors during federal contract source selection processes. The form is required as part of the Past Performance Volume III Proposal submission for the Mission and Installation Contracting Command Request for Proposal (Solicitation Number W911SD26QA036) for Branded Merchandise services to the United States Military Academy Admissions Directorate.

The form authorizes major subcontractors and teaming partners to allow the Government to disclose their past and present performance information to the prime contractor during source selection exchanges. Completion of the consent form by identified subcontractors and teaming partners is necessary because the Government cannot disclose subcontractor or teaming partner performance data to private parties—including prime contractors—without explicit written consent. The document includes a sample consent letter format requiring signature and title of an individual with legal authority to bind the company, along with the company name and address. This consent facilitates the Government's Past Performance Evaluators' assessment of contractor and subcontractor relevancy and confidence in meeting contract requirements.
