About this file
This document is a Conflict of Interest Questionnaire (CIQ) issued by the City of Fort Worth's Purchasing Division for a Request for Quotation (RFQt) with reference number 25-0219 for an Online Calendar Platform. The CIQ is a mandatory form pursuant to Chapter 176 of the Local Government Code, which requires any person or agent seeking to contract with a local governmental entity to disclose potential conflicts of interest. The questionnaire must be filed with the City of Fort Worth Secretary no later than seven days after the person begins contract discussions, submits an application, or responds to a request for proposals or bids.
The document provides detailed instructions for completing the Conflict of Interest Questionnaire, highlighting the legal requirements for disclosure of business relationships that might cause a conflict of interest. Vendors must specify any employment or business relationships with local government officers or their family members, including situations involving taxable income or gifts. The form requires vendors to provide information about their connections to local government entities, potential financial interactions, and any gifts given to government officials. Failure to comply with these disclosure requirements can result in a Class C misdemeanor. The form is part of the Texas Ethics Commission's efforts to ensure transparency and prevent potential conflicts of interest in local government contracting processes.
