This project requires all labor, tools, equipment, transportation, and supervision, as required for the project elements as described for the following work:
Scope of Work: The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform security door replacement services to enhance the security and confidentiality of Room 123. This includes removing the existing doors and frames and installing three (3) STC 50 rated doors, frames, and hardware packages to ensure adequate sound isolation and prevent unauthorized disclosure of sensitive information
Background: Room 123 will be utilized as a stand-up area for alternate operations, requiring enhanced security and confidentiality due to its designation as a mission-restricted area. The STC 50 rated doors will provide the necessary sound isolation to prevent unauthorized disclosure of sensitive information discussed within the room.
Room 123 is a designated mission-restricted area for alternate operations stand-up. The existing door and frame do not meet required security and acoustic standards.
Once the construction services are deemed complete, the Government Point of Contact (POC) shall conduct a walk thru with the 154 WG information protection officer (WG/IPO) to confirm all STC- 50 rated security door requirements are met.
All work shall be completed as indicated on this SOW, the Contract drawings, and in accordance with specifications and codes. Contractor personnel shall be licensed by the State of Hawaii as qualified licensed contractors and provide a copy of the license/certifications to the contracting officer (for each employee) at the time of contract start. The work shall be performed following all applicable Federal and State codes and regulations, including the requirements stated in this SOW.
The Contractor must coordinate with the 298 ADG to access the work site as well any hazardous materials storage/disposal.
The access/haul routes are shown below.
The Government does not provide training to contractors. Contractors must ensure that any personnel performing under a contract are fully trained, licensed, certified, and qualified for the position in which they will be serving.
It will be the responsibility of the Contractor to restore the facilities to their original condition if any damage occurs during construction.
LOCATION: This project is located at Room 123, Building 300 at the 298th Air Defense Squadron, Wheeler AAF, HI 96786.
NORMAL HOURS OF OPERATION: 6:00 AM – 4:30 PM, Monday through Friday. The field investigation and subsequent construction can be accomplished anytime during these hours unless the base is closed for a holiday and 5-4-9 “Down Mondays” (Schedule can be provided upon request). Prior coordination, via Contracting Officer is required.

The Contractor shall field verify all dimensions prior to bidding. Contractor will provide qualified onsite Supervision with proof of 5-years minimum experience, managing similar type and size projects, for approval, prior to commencement of work.
ADMINISTRATIVE REQUIREMENTS:
Contractor will be required to file and attain the following approvals prior to project mobilization:
1. Contractor shall be responsible for obtaining base passes for all employees to include and associated subcontractors. All contract employees shall be subjected to background checks before base passes are issued and shall follow all base security procedures.
2. Obtaining a temporary construction waiver for the duration of the project.
3. Project shall commence not later than thirty (30) calendar days after Notice to Proceed (NTP) and shall be completed within Ninety (90) calendar days.
4. Antiterrorism Awareness Level I Training (AT Level I). This standard language is for contractor employees with an area of performance within a US Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to US Government installations, facilities and controlled access areas shall complete AT Level I training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. AT Level I may be accomplished by a Level I qualified instructor; completing the Force Protection computer-based training (CBT) course on the Advanced Distributed Learning System (ADLS), or Joint Knowledge Online at http://jko.jten.mil. Tracking for each contractor or subcontractor employee is the responsibility of the COR or unit AT Representative.
5. Contractor shall be responsible for obtaining and submitting Operation Security training (OPSEC) certificates of completion for all contractors and subcontractors. The required training can be completed online at https://www.cdse.edu/catalog/elearning/GS130.html
OPSEC Control Plan (OCP): All contractors (including subcontractors) identified in the Operations Security (OPSEC) Plan under APPENDIX A of the Statement of Work shall supplement their current security practices by requiring any personnel involved in executing the KNMD250021 contract to complete Government-sponsored and administered Operations Security (OPSEC) training. Upon contract award, all identified contractors (including subcontractors) shall sign the contractor's conformance statement contained within the Operations Security (OPSEC) Plan and submit it to the contracting officer thereby acknowledging that they will meet the requirements of the KNMD250021 PMO generated OPSEC Plan. The contractor specialist security officer (CSSO) shall contact the contracting officer or designated contracting officer's representative (COR) in accordance with the OPSEC Plan to schedule key employees to attend the Government-sponsored OPSEC training.
The contractor must immediately notify the Government upon the discovery of any nonconformance with the OPSEC Plan.
PRINCIPLE ELEMENTS OF WORK:
The Contractor shall field verify all dimensions, materials required, and locations prior to bidding.
The below items are the major elements of work and may not contain all items required to construct the full systems as described in the Scope at the start of this document.

2. Submit an assessment report advising of any discrepancies not included in this SOW.
3. Remove existing doors, install STC-50 rated doors and hardware.
a. Ensure Safety protocols are in place.
b. Removal of the existing doors and frames.
c. Preparation of the door opening.
d. Installation of new STC 50 rated doors and frames assembly.
e. Acoustic sealing of the door frame to the wall.
f. Verification of proper operation of all hardware.
g. Disposal of all debris.
h. Post-installation STC testing, effectively blocking speech and reducing louder noises to a faint murmur.
i. Ensure all work is met and completed to meet building security requirements.
4. Submit any drawings generated during the project.
PROJECT SUPERVISION/KEY PERSONNEL:
This project requires the following supervisory roles with the minimum qualifications as described:
Project Manager: The Contractor’s Project Manager shall have either: (1) an engineering degree and five (5) years of experience as a Project Manager working on projects similar in size and scope to the solicited project OR (2) 10 years of experience as a Project Manager working on projects similar in size and scope to the solicited project.
Site Superintendent: The Contractor’s Site Superintendent shall have: (1) a minimum of five (5) years of experience working on projects similar in size and scope to the solicited project OR (2) 10 years of experience as a Site Superintendent working in general construction areas that included extensive interactions with the Government. The Site Superintendent shall visit the site daily, submit detailed progress reports (AF Form 3065), schedule and coordinate subcontractors and materials, and attend all weekly status meetings, site visits, and pre-final/final inspections.
Quality Control Manager: The QC Manager must be a graduate engineer, graduate architect, or a graduate of construction management, with a minimum of five (5) years of construction experience OR a construction person with a minimum of 10 years of experience working on projects similar in size and scope to the solicited project. Certified QCM having completed the Construction Quality Management for Contractors by the US Army Corps of Engineers, or equivalent, within the past 5 years. The QCM shall visit the site daily to ensure full compliance with all quality requirements and verify that the materials and workmanship are in accordance with approved construction drawings, shop drawings, material submittals, and technical specifications. The QCM shall prepare and coordinate material submittal sheets and shop drawing submittals, prepare QC reports, schedule and coordinate testing procedures, and attend all weekly status meetings, site visits, and pre-final/final inspections.
Safety Manager: The Safety and Health Officer shall have a minimum of five (5) years of experience working on projects similar in size and scope to the solicited project and five (5) years of experience working with the requirements described in the US Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1. Certified Site Safety and Health Officer (SSHO) having completed the 40-hr EM 385-1-1 training within the past five (5) years and current with at least 8-hrs of training in each subsequent year. The Safety Manager shall visit the site daily to ensure full compliance with all safety requirements.
During all working hours, either the Project Manager or Site Superintendent is required to be onsite. In addition, the Quality Control Manager and Safety Manager are to be onsite.
Due to the complexity of this project, the Superintendent may also fill the role of Safety and Project Manager. The Quality Control Manager may also fill the role of Safety. The Superintendent shall NOT be the same person as the Quality Control Manager.
General Notes:
a. All work will be accomplished in a clean, safe manner with quality workmanship in accordance with accepted Government and construction industry standards & practices.
b. The Contractor shall be responsible for verification of all quantities and dimensions prior to commencement of work. Further, the Contractor shall follow the enclosed Submittal Register.
c. All materials and/or equipment shall be of high quality, installed in accordance with manufacture’s recommendations and accomplished by workman skilled in this type of work, using standard trade practices.
d. There shall be no deviations or substitutions without prior Government approval.
Unless indicated otherwise, all Equipment and Material shall be new, undamaged and meet the requirements of Underwriter's Laboratories, Inc. (UL). Where UL requirements are not applicable, equipment and material shall be identified as such by Contractor and approved by Owner before purchase and installation.
e. All products and materials/equipment installed shall be approved by AF Form
3000 material approval submittal.
QUALITY CONTROL PLAN (QCP):
The QCP shall be submitted to the Government as a complete document that addresses all types of inspections/tests for work covered by the Final Document submissions.
Content: For each performance and installation requirement, the QCP shall clearly identify:
• Item/system to be tested,
• Exact test(s) to be performed,
• Measured parameters,
• Inspection/testing organization, and
• The stage of construction development when tests are to be performed.
• Each inspection/test shall be included in the overall construction schedule. The Contractor is not relieved from required performance tests should these not be included in the plan. The QCP is intended to document those inspections/tests necessary to assure the Government that product delivery, quality, and performance are as required. It also serves as an inspection coordination tool between the Contractor and the Government. The QCP shall be prepared and submitted to the Government for acceptance during final design development.
Updating: During construction, the Contractor must update the QCP if any changes are necessary due to any changes or schedule constraints. The Government's Contracting Officer and Government POC must be notified immediately by the Contractor of any schedule and/or procedural changes.
INSPECTIONS AND TESTS
The Contractor shall perform inspections and tests throughout the design and construction process, including: construction installation (placement/qualification measurements) and final inspections/tests (commissioning/performance certification). Periodic "Quantity" inspections shall also be conducted to support progress payment requests.
Government Witness: All inspections and tests called for in this RFP and/or required to verify documented contract assumptions, to establish work accomplishment, or to certify performance attainment shall be witnessed by the Government and coordinated through the Quality Control Plan (QCP). The contractor shall notify the Contracting Officer in writing at least two (2) weeks prior to conducting.
SITE MAINTENANCE
The contractor shall dispose of all trash and debris generated during the contract. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal & Wheeler AAF guidelines and regulations.
The contractor shall store all refuse in appropriate trash containers on the jobsite. Location of a dumpster at the site shall be as directed by the Contracting Officer or Government POC. The dumpster shall be covered to prevent possible use by base personnel. The contractor shall have all refuse removal trucks covered and secured prior to entering or leaving the jobsite. If any materials are observed falling off the trucks, the truck shall be stopped and reloaded to prevent damage. The contractor shall bear the monetary responsibility to repair all damage associated with falling debris.
The contractor’s dumpster shall be removed prior to Final Inspection. The contractor shall repair any damage the dumpster causes to concrete, asphalt paving or lawns.
The base refuse contract supplies dumpsters for Government produced refuse only. The contractor shall not dump refuse from this project in the Government dumpsters. If the Contracting Officer discovers project refuse in Government dumpsters, he may instruct the contractor to immediately remove the refuse, or have Government resources remove it at the contractor’s expense.
Protection of Contractor’s Equipment: All contractor supplied equipment is the sole responsibility of the contractor. The Government is not liable for the theft, vandalism, or damage of any contractor supplies, equipment or property. The contractor shall secure his materials and protect all equipment from damage. Any equipment/material the contractor removes and which requires reinstallation as part of the job requirements shall be the responsibility of the contractor.
SHOP DRAWINGS/PRODUCT DATA
Submissions: Shop Drawing/product data submissions to the Government shall be made after review and approval by the Contractor's designer of record. This is in addition to and separate from specification submission material. Product data shall be delivered to the Government within 30 calendar days following the Government's review of Final Document design and/or the Contractor's establishment of subcontract agreements to provide involved work: Shop drawings shall be provided within 60 days of these occurrences. Shop drawing and product data submissions shall be made to the Contracting Officer and the Government POC in an electronic format. Material samples shall be sent to USPFO-HI.
Reviews: Reviews of shop drawings and product data by the Government are not to be interpreted as an approval or acceptance of the Contractor's selections or progress toward meeting contract requirements but are intended to discover problems/errors which may avoid costly redirection during construction. The Contractor shall remain completely responsible for design and construction in accordance with contract requirements. The Government shall be allowed two (2) weeks to review these submission materials from the date of receipt from the contractor.
Changes: Should shop drawing and/or product changes take place after initial Government review, the Contractor shall resubmit the altered product data material and shop drawings within two (2) weeks of the change.
Manuals: The Contractor shall combine all product data submission material into hard copy manuals for reference during all phases of construction. Shop drawings shall be bound with product data or separately bound in drawing racks. The Contractor shall maintain duplicate copies for the Government.
WARRANTY
In addition to any other warranties in this contract, work performed under this contract shall conform to the contract requirements and is free of any defect in equipment, material, or workmanship performed by the Contractor or any subcontractor or supplier at any tier. This warranty shall continue for a period of 1 year from the date of final acceptance of the work. If the Government takes possession of any part of the work before final acceptance, this warranty shall continue for a period of 1 year from the date that the Government takes possession.
The Contractor shall remedy at the Contractor’s expense any failure to conform, or any defect. In addition, the Contractor shall remedy at the Contractor’s expense any damage to Government-owned or controlled real or personal property, when that damage is the result of (1) The Contractor’s failure to conform to contract requirements; (2) Any defect of equipment, material or workmanship; or (3) Contractor negligence. The Contractor shall restore any work damaged in fulfilling the terms and conditions of this clause.
With respect to all warranties, express or implied, from subcontractors, manufacturers, or suppliers for work performed and materials furnished under this contract, the Contractor shall obtain all warranties that would be given in normal commercial practice: Require all warranties to be executed, in writing, for the benefit of the Government, if directed by the Contracting Officer; and enforce all warranties for the benefit of the Government, if directed by the Contracting Officer. In the event the Contractor’s warranty has expired, the Government may bring suit at its expense to enforce a subcontractor, manufacturer or supplier warranty.
SUBMITTALS
Materials that do not comply with the criteria listed in this SOW may be considered non-adherent or unacceptable. Non-adherent or unacceptable materials shall not be used in the performance of this SOW. All deviations from this statement of work must be specifically requested in writing by the Contractor and receive formal acceptance from the Contracting Officer before commencing with installation or work. The Contractor shall provide a completed master submittal schedule in AF Form 66 format for review and acceptance by the Contracting Officer within ten days of the receipt of the Notice to Proceed. Further, the Contractor shall follow the enclosed Submittal Register.
CLOSE OUT DELIVERABLES
Closeout pursuant to ANG Engineering Technical Letter (ANGETL) 09-11, Project Closeout Process and any other requirements called out by the Contracting Officer.
APPENDIX A
GENERAL CONTRACTOR OPSEC REQUIREMENTS
During the course of this contract, in addition to those restrictions, instructions and guidelines delineated in the contract Statement of Work and/or other references provided, the contractor will adhere to the following minimum requirements in support of the 154 CES OPSEC Program:
Antiterrorism Awareness Level I Training (AT Level I). This standard language is for contractor employees with an area of performance within a US Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to US Government installations, facilities and controlled access areas shall complete AT Level I training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. AT Level I may be accomplished by a Level I qualified instructor; completing the Force Protection computer-based training (CBT) course on the Advanced Distributed Learning System (ADLS), or Joint Knowledge Online at http://jko.jten.mil. Tracking for each contractor or subcontractor employee is the responsibility of the COR or unit AT Representative
a. Awareness Training. Initial training may be provided by both computer-based training and live training. OPSEC training is required initially within 90 days of assignment and annually thereafter.
Each individual will submit a certificate of training within 30 days of signing this document. The computer-based training can be found through the following link:

b. Introduction of personnel electronic devices into government spaces, laptops, tablet PCs, cellular phones, cameras, recording devices, and data recording/storage devices is STRICTLY controlled and forbidden in most cases. Company issued equipment required for the performance of work must be approved by the Contracting Officer Representative (COR). Photography and recording is not allowed except for official use and by permit only. (Unless otherwise stipulated in the contract, contact the COR for approval.) Photographs will be reviewed by Security to ensure sensitive and/or classified information is not revealed. Secure electronic transmission and storage of critical information. Critical information must be transmitted and stored in accordance with the SOW.
Secure storage of hardcopy critical information. Critical information in hardcopy form must be stored in secure areas and/or containers in accordance with the SOW.
c. Contractor personnel shall not discuss government operations in public or over unprotected or unencrypted communications. Official Business, controlled unclassified information may only be transmitted as directed in the SOW.
d. The Contractor shall not post to company websites, publications, newsletters or other media any images, data or information that reveal sensitive government operations, personnel, equipment, and/or classified or controlled unclassified information. When in doubt, company press releases related to this contract should be coordinated through the COR. Pre-publication procedures are established to ensure no public release concerning program information occurs without the prior written approval of the OPSEC Coordinator.
e. Because observation of events, operations, physical changes, etc. may reveal National Security information, specific restrictions are needed to preclude unintentional release of this information to unauthorized parties. (Unauthorized disclosure and transfer of National Security Information is http://jko.jten.mil/ https://www.cdse.edu/catalog/elearning/GS130.html punishable under 18 USC § 793.) Therefore, contractor personnel shall not disclose to unauthorized third parties, post to unofficial sites (including Social Networking sites) any images, data or information, or observed events that reveal sensitive government operations, personnel, equipment, including, but not limited to:
(1) Tactics, techniques and procedures, production or work schedules, any visible or concealed modifications, upgrades, additions to aircraft, weapons or equipment; increases, change, or decreases in work/deployment frequency or government personnel, vehicle, vessel or aircraft movements;
specialized equipment orders, deliveries, shipments, etc., unauthorized disclosures and attempts to solicit this type of information by unauthorized third parties or others not affiliated with this contract shall be reported to the COR.
f. Service. Non-Disclosure requirements remain in effect during the duration of this contract and indefinitely thereafter. See Annex A.
g. Government issued badges, identification shall be removed and/or concealed from plain sight when off station and shall not be left in vehicles or unprotected. Badges and passes may not be duplicated or copied or loaned to others. Lost or stolen identification badges, vehicle passes etc. will be immediately reported to the COR.
h. Contract personnel must mark and protect related internal production schedules, deliverables, inventories and shortages and identified vulnerabilities related to production of government material.
Internal company markings e.g., Business Sensitive, etc., are appropriate for identifying the aforementioned as sensitive information. Specific Government-provided information, drawings etc., will be protected in accordance with guidance in applicable paragraphs of the SOW/PWS and clauses included in the solicitation/contract.
i. All government information must be destroyed at contract termination or returned to the government at the government’s discretion. Critical information must be disposed of by cross-cut shredder or burning. Critical information shall not be disposed of in trash receptacles.
j. Visitor Control. All visitors are required to process through established checkpoints for verification of identity, citizenship, personnel security clearances, appropriate certification of purpose of visit, issuance of badges, and inspection of articles being brought into and out of the facilities and other such measures to assure proper visitor control.
k. Escort Procedures. Escorts for visitors shall be advised of proper escort procedures, limitation on disclosure, and other applicable controls involved in the visit.
l. Unauthorized Personnel. Program personnel shall be alerted when visitors or other unauthorized personnel are admitted to work areas. Personnel shall refrain from inadvertent release of information by visual and aural means when visitors are present. Activities of visitors and non-assigned personnel in the program areas shall be observed to determine that their presence is required by business needs and that no suspicious activities are detected which may pose a threat to the security of information.
m. Conference Rooms. During meetings, attendees will be reminded of conference room procedures to be followed when discussing critical information.
