About this file

This is a Past Performance Questionnaire template created by DHS Immigration and Customs Enforcement (ICE) for evaluating vendors bidding on an IDIQ contract for Transportation Services. The questionnaire must be submitted directly to brittany.tobias@ice.dhs.gov and Shereen.demarais@ice.dhs.gov by May 2020 (specific date not provided).

The questionnaire evaluates contractors across three main areas: Corporate Commitment, Quality of Service, and Timeliness of Performance using a four-tier confidence rating system (High Confidence, Some Confidence, Low Confidence, Unknown Confidence). Key evaluation factors include contract/project management effectiveness, understanding of requirements, quality control standards, service quality, personnel qualifications, emergency response capabilities, and adherence to delivery schedules. The form also asks about any contract terminations, contractor strengths and weaknesses, and whether the evaluator would award future contracts to the contractor.
