About this file
This is a Past Performance Questionnaire template used to evaluate federal contractor performance on completed or ongoing contracts. The form is designed to be completed by agency personnel or company representatives who have direct knowledge of a contractor's performance.
The questionnaire captures essential contract information including contractor name, contract number, procuring agency details, period of performance, and dollar amounts (award amount, annual amount, and total value with modifications). Respondents must identify the contract type (Firm Fixed Price, Time and Materials/Labor Hours, Cost Reimbursement, or other) and provide the contract title and scope description. The form includes a complexity of work section and requires evaluation ratings across seven performance factors: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance. For each factor, evaluators must provide mandatory comments and assign adjectival ratings from a provided guidelines sheet that uses five levels: Unsatisfactory, Marginal, Satisfactory, Very Good, and Exceptional. The form concludes with a question asking whether the evaluator would select the contractor again, with response options of Definitely, Probably, or Definitely Not, and requires the evaluator's signature, name, title, and date.
