About this file

This is a Cost Schedule and Fee submission form for a Construction Manager/General Contractor (CMGC) Request for Proposals issued by the City of South Jordan Public Works Department for the West Side Campus & Public Works Water Annex Facility project in Utah. Proposers must submit completed fee schedules as separate PDF documents and are required to attend a mandatory pre-proposal meeting on May 26, 2026 at 1:00 p.m. at the City of South Jordan Public Works Building located at 10996 S. Redwood Road, South Jordan, Utah. The CMGC will provide both pre-construction and construction phase services in conjunction with the Architect's design team, working to deliver the project within the approved construction budget. All proposals must be submitted electronically through the State of Utah's BONFIRE procurement platform, with submission deadlines and additional details available on that website.

The fee schedule requires proposers to provide lump sum costs for Design Phase/Pre-Construction Services, General Conditions expenses for the construction phase (including monthly rates and expected construction duration), Construction Supervision and Management Fees expressed as a percentage of the Owner's Construction Budget, and Performance & Payment Bonds and Insurance coverage costs calculated as a percentage of the Owner's Construction Cost. Proposers must also provide a comprehensive list of items included in their General Conditions to enable objective comparison among bidders. The form specifies that all quoted fees and costs must cover all expenses for both pre-construction and construction phases as outlined in the RFP and Qualifications Submission, and an authorized company representative must sign and date the completed form.

As part of Section 6 of the Request for Proposals, this Fee Schedule Form is provided for convenience to the Proposers and to assist the Selection Committee with the evaluation process. It should be completed by an authorized agent of the Proposer and submitted as a separate PDF document (not as part of the Proposal document). The undersigned, responsive to the "Request for Proposals Construction Manager/General Contractor Services for the for the City of South Jordan Public Works Department West Side Campus & Public Works Water Annex Facility", propose the following Fees at the costs, percentages and/or prices stated below. These values are to cover all expenses incurred in the performance of the services as outlined herein – both in the pre-construction and construction phases - and in our Qualifications Submission, of which this Fee and Cost Schedule is a part.

DESIGN PHASE / PRE-CONSTRUCTION SERVICES FEE:

For the Design Phase / Pre-Construction Phase, and associated services outlined in the RFP, I / We agree to perform these professional Services for the lump sum of:

Provide written and numerical value.

1. CONSTRUCTION MANAGER / GENERAL CONTRACTOR’S GENERAL CONDITIONS:

To provide clarity, the Owner considers a General Contractor’s “General Conditions” to be inclusive of the direct and indirect costs (sometimes referred to as General Conditions and General Requirements) associated with the appropriate service, management and performance of delivering the Construction Phase of the Contract.

Typically, these General Conditions will include costs & expenses for a wide range of construction support items including, but not limited to: temporary facilities; project management software; phone / communication costs; construction supervision; mobilization / demobilization; accounting / administrative support; temporary utilities; erosion control / SWPPP; cleaning; field office & supplies; safety & site security; etc.

Because every General Contractor defines their GC’s differently, the Selection Committee would like to understand what is included in each Proposer’s General Conditions as a basis of comparison and in order to more objectively assess a CMGC’s total value. On a separate page, provide a comprehensive list of the Proposer’s General Conditions. If supervision, administrative or other personnel expenses are NOT a part of the identified GC’s, the assumption is those costs are part of the Supervision and Management Fee, unless otherwise noted. Costs per line item are not required.

Provide the Proposer’s expense for General Conditions.

3. CONTRACTOR’S PERFORMANCE & PAYMENT BONDS and INSURANCE:

For all work during the Construction Phase of the contract, I/we agree to provide bond & Insurance coverage based upon the indicated percentage as a factor of the Owner’s Construction Cost:
