About this file

This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for vendors participating in the FEMA Emergency Food Service contract opportunity. The affidavit must be completed and submitted with the vendor's initial proposal or, if not provided at that time, within three business days of the County's request. The form applies upon contract award, renewal, or extension. Vendors who fail to comply with the submission requirement within the stated timeframe may be deemed nonresponsive. The contract involves furnishing all labor, materials, equipment, services, and incidentals necessary to provide catered food and beverage services to Water and Wastewater Services and other Broward County agencies following declared emergencies or disaster-related events. Services must comply with Federal Emergency Management Agency (FEMA) reimbursement requirements. All vendor responses must be submitted electronically through BPRO by the specified closing date and time, with the County providing a designated period for vendors to submit questions and seek clarification regarding solicitation requirements.

The affidavit certifies that the vendor does not use coercion for labor or services as defined in Section 787.06, Florida Statutes. Vendors must execute the form under penalties of perjury, providing their name, signature, printed name, title, and date. The County emphasizes that while certain documents are listed as optional in BPRO, vendors are strongly encouraged to submit all available files at the time of initial submittal to enable timely and comprehensive review. Procurement preferences apply to Small Business Enterprises (SBE) and County Business Enterprises (CBE) as administered by the Office of Economic and Small Business Development (OESBD). County and state licensing requirements apply to all participating vendors.
