About this file

This is a Human Trafficking Attestation form required by the City of Tallahassee for nongovernmental entities executing, renewing, or extending contracts with the municipality. The attestation pertains to a Battery Testing for Electric Generation and Electric Generating Stations contract. The City of Tallahassee requires battery load testing services at two mandatory site visit locations: Hopkins Generating Station (1125 Geddie Road, Tallahassee, FL 32304) and Purdom Generating Station (667 Port Leon Drive, St. Marks, FL 32355), with the initial site visits scheduled for May 28, 2026, at 10:00 a.m. The contractor must provide all services, labor, materials, equipment, and supplies necessary to complete the battery testing work, with the City retaining the right to increase or decrease the number of tests as needed. Site visit dates and times are subject to change at the City's discretion.

The attestation form requires a company officer or authorized representative to affirm under penalty of perjury that the entity does not use coercion for labor or services as defined in Florida Statutes Section 787.06, and that the entity will immediately notify the City of Tallahassee if such practices occur in the future. The signatory must be over eighteen years of age and authorized to execute the attestation on behalf of the entity. Non-compliance with these requirements subjects the signatory to potential criminal penalties for false statements. This document serves as a compliance mechanism for the City of Tallahassee to ensure ethical labor practices among its contract vendors.
