About this file

This is a Past Performance Questionnaire template designed for the Department of State's Passport Book Personalization Facilities program. The questionnaire requests detailed evaluations across seven key areas: Technical Services, Quality of Services, Timeliness of Performance, Price/Cost Control, Business Relations, Customer Satisfaction, and Overall Contract Performance.

The evaluation form requires reference information including contractor name, customer agency details, technical point of contact, contract numbers, and contract values. Performance ratings use a four-tier scale: Excellent, Satisfactory, Marginal, and Unacceptable. Key technical service areas being evaluated include IT testing, support and integration services, security clearance facility operations, team partner management, and program management capabilities. The questionnaire particularly emphasizes assessment of contractor performance in areas such as personnel retention, cost control, business relationships, problem resolution, and overall contract execution effectiveness.
