Any 'No' answer disqualifies a Contractor from bidding on this Project.
Answer the following questions: YES NO
Do you have a minimum of 5 years of experience on construction projects similar in magnitude, scope, and complexity to that described in the solicitation?
Valid experience must have occurred within the last 15 years.
Do you have experience as a prime contractor with the Federal government on projects similar in both scope and magnitude to this project?
Valid experience must have occurred within the last 10 years.
Contractor must have had contract directly with a Federal agency.
Do you have experience as a prime contractor on at least 2 projects similar in scope and magnitude to this project?
Valid experience must have occurred within the last 10 years.
Do you have a minimum of 5 years of experience in heavy construction that included primarily cast-in-place concrete work?
Valid experience must have occurred within the last 15 years.
Do you have experience as a prime contractor in concrete drilled piers?
Valid experience must have occurred within the las 10 years.
6 Do you have experience in earthwork construction?
Valid earthwork experience must have occurred within the last 15 years.
7 Do you have a Dun & Bradstreet (D&B) Viability Score of 5 or better?
1-Best, 9-Worst 5 or better indicates score of 5, 4, 3, 2, or 1
Do you have a Dun & Bradstreet (D&B) Portfolio Comparison Score of 5 or better?
1-Best, 9-Worst 5 or better indicates score of 5, 4, 3, 2, or 1
Notes: Magnitude refers to dollar amount of work.
Scope refers to technical requirements of work.
Complexity refers to difficulty of performing work due to many varied interrelated parts.
Similar in magnitude refers to projects priced at 85% of your bid and above.
Similar in scope refers to the same type of work with comparable or greater quantities.
Complete the attached forms, IBWC Form 245B-Backup Information, for each project which supports your 'yes' answers above.
I certify that my answers to these questions are true and correct.
If additional backup information is required by the Contracting Officer to further aide in determining responsibility in accordance with the general standards under Federal Acquisition Regulation (FAR) 9.104-1, I certify I will be able to submit the information within twenty-four (24) hours of the request.
If backup information is not provided when requested by the Contracting Officer or if backup information fails to support the answer provided, the Bidder will be found to be ineligible for award.
