About this file
This is a Supplier Contact List Designation Form issued by the Clark County School District Purchasing Department in Las Vegas, Nevada. The form serves as an administrative document enabling vendors and suppliers to designate authorized business contacts who may conduct various transactional activities on behalf of their company. The form allows suppliers to establish and maintain up to two principal contacts, one sales representative/account manager, two accounts receivable contacts, and one technical contact. The principal contact holds authority to manage company information including bank account details for payment processing, while account representatives may request and provide purchase order and invoice data. Accounts receivable contacts are authorized to provide invoices and inquire about payment status, and technical contacts may request file resends and inquire about delivery information and system issues. The Clark County School District acknowledges this form as valid until written notice of changes is received and processed.

The document is part of the vendor management process for the Clark County School District and does not establish contract terms, delivery schedules, or financial obligations. Suppliers are directed to submit completed forms to the Purchasing Department at 4212 Eucalyptus Avenue, Building 7, Las Vegas, Nevada 89121, with contact available by telephone at (702) 799-5225 or fax at (702) 799-5018.
