Regarding the statement "If a subcontractor/teaming member is proposed, subcontractor/teaming member independently certifies affirmatively to all responses in #3 above." Is the Government requesting a separate submission of subcontractors responses to the questions in #3 or just a signed statement from the subcontractor that they indpendently certify affirmatively to all responses in #3 above?

The Prime should submit the certification for the team. The prime can submit the subcontractors' statement of certification for the team.
