About this file

This document is an Iran Contracting Act Verification Form required by the California Department of General Services (DGS) for vendors bidding on or executing a California state contract valued at $1 million or more. The form requires vendors to certify that they are not on the DGS list of entities prohibited from contracting with public entities in California due to investment activities in Iran. Vendors must either provide this certification or demonstrate they have received an exemption approved by the state.

The form provides two options for vendors to comply with this requirement - either certifying they are not on the prohibited list, or providing documentation to demonstrate they have received an exemption. Vendors must provide their company name, federal ID number, authorized signature, and printed name and title of the signing official. The document outlines the penalties for providing false certifications, including civil penalties, contract termination, and ineligibility to bid on future contracts.
