SECTION 011000

SUMMARY OF WORK

1.1 PROJECT DESCRIPTION

A. This project is for a roof replacement at the Pea Ridge National Military Park Visitor Center consisting of the removal and disposal of the existing asphalt shingle roofing and associated components and installing a standing seam metal roof system and associated components as set forth in these Specifications.

B. Project Location: Pea Ridge National Military Park, Visitor Center, 15930 Highway 62, Garfield, AR 72732

C. All work will be performed under one single firm fixed price contract.

D. Contractor will provide all labor, supervision, materials, tools, equipment, supplies, transportation, vehicles and other items, services and incidentals necessary to successfully complete the project as described below, within the contract’s period of performance.

(1) Roof area replacement is anticipated to be approximately 11,500 sq. ft.

(2) Work will include removal of all existing asphalt shingles and underlayment, removal and replacement of approximately 200 sq. ft. of damaged roof decking; installation of roofing underlayment, standing seam metal roof panels, vented ridge caps on the two main roof ridges, 8 roof vents, 6 plumbing vents, flashing, counter flashing, drip edge and all necessary hardware and accessories to provide for a fully weathertight roofing system.

(3) Approximately 365’ of gutter, 200’ of downspout, 680’ of soffit, and 680’ of fascia are to be replaced.

(4) In addition, approximately 80’ of new gutter and two (2) downspouts (approximately

30’) will be installed on the northwest roofline of the building’s maintenance shop area.

E. Unless otherwise specified, all materials shall be new, unused and undamaged. They shall be the current and standard designs of manufacturers regularly engaged in their production.

F. All materials used will be of the proper specifications. Inferior products are unacceptable and will not be utilized.

G. Comply with the manufacturer’s instructions for rigging and handling equipment.

H. Install all parts plumb and level, firmly anchored in locations of previous footprint, and maintain manufacturer’s recommended clearances.

I. Painting of fascia and soffit boards.

J. Disposal of all removed roofing materials and construction debris that is generated from the work being performed.

K. All work shall be performed in accordance with applicable local, state, and federal codes, standards and regulations, and the product manufacturers’ recommendations. Should the manufacturers’ recommendations, the contract’s project documents and local, state and federal regulations be in conflict, Offeror should address with the Contracting Officer for resolution.

L. Provide and erect barricades, partitions, fences, signage, and other equipment, materials and supplies needed for the safety of and to prevent harm, damage or injury to Park employees and visitors, and their properties. Coordinate the appropriate type and placement of safety apparatus with the COR.

M. All work and materials required to satisfactorily complete the work of this project for the

Government’s acceptance will be provided and accomplished by the Contractor even though they may not be listed or specified in the project documents.

N. Upon completion of the installation, Contractor, in the presence of the COR, shall conduct an industry standard test to detect if leaks are present in the roofing system.

O. Offeror is responsible for field verification of measurements prior to submitting a proposal.

Failure by the offeror to verify the government’s approximated measurements prior to submitting a proposal shall not constitute grounds for a claim after contract award.

1.2 QUALITY ASSURANCE

A. The removal and installation of the roof and components is to be accomplished by a firm who possesses the knowledge and technical capability, and has the experience in the installation of standing seam metal roofing.

B. The work shall be performed by competent craftsmen skilled in the metal roofing trade and shall be done in a manner consistent with industry standards, product manufacturers’ recommendations, and in compliance with applicable local, state and federal codes and regulations.

1.3 PERIOD OF PERFORMANCE. It is estimated that a contract for this project shall be awarded in the mid- to late-May 2018 timeframe. All work is to be completed no later than one hundred twenty (120) calendar days from the CO’s issuance of the “Notice to Proceed” to the Contractor.

This also includes final government inspection, completion of punch list items, and final cleanup by Contractor of the contract site.

1.4 JOB CONDITIONS, CONTRACTOR’S USE OF PREMISES, AND OTHER PERFORMANCE RELATED

REQUIREMENTS

A. Construction Camp: Establishment of a camp within the Park will not be permitted.

B. Contractor Conduct: At all times while on the contract site, Contractor and Contractor’s subcontractors and suppliers shall conduct their operations in a professional and courteous

manner and abide by National Park Services rules and regulations and the regulations prescribed by the Park Superintendent.

C. Work Restrictions: Work shall be performed during the Park’s normal business working hours of 7:00 a.m. to 4:30 p.m. local time, Monday through Friday, excluding federal holidays, and except when otherwise indicated by the Contracting Officer. If desiring to work outside the standard hours, Contractor must submit a written request to the CO at least three (3) work days prior to the proposed performance of work. The written request should state the proposed performance of work, day/days, time/amount of work, and reason why authorization should be granted to perform work during non-standard hours. Work outside the standard hours is prohibited unless approved by the CO. Work during non-standard work hours shall be at no additional cost to the Government.

D. Project Site Superintendent: Contractor must have a designated superintendent at the contract site at all times during work performance, to include work that is being performed by the Contractor’s subcontractors at any tier. The superintendent’s duties shall include the overall management of the project, coordination of the subcontractor’s work, and liaison with the CO and COR. The designated Project Superintendent, or a competent representative, shall be thoroughly familiar with the scope of work, specifications, drawings, and terms and conditions of the contract. No work shall be performed without the presence of the superintendent or competent representative on the job site. A competent representative is as defined by the Occupational Safety and Health Administration (OSHA) Standards.

E. 24-Hour Point of Contact: Provide the CO and COR with a 24-hour number or numbers where the designated superintendent and an alternate can be contacted at any time in the event of an emergency or urgent matter.

F. Work Coordination

1. The Contractor shall coordinate all work schedules with the CO or COR prior to the start of work. Notify the COR at least three (3) working days prior to moving onto the work site to commence work. Before initiating any work, the Contractor shall meet with the COR to discuss, at a minimum: a sequence of procedures; parking; a means of access to premises and buildings; placement of dumpsters if authorized by the COR; haul routes;

delivery of materials and use of approaches; use of corridors, stairways, elevators, and

similar means of egress; communications, the location of partitions and barriers;

availability or non-availability of restroom facilities for contractor’s employees, and any other incidentals to the project. Contractor to interview COR with regard to other information the Contractor may need to be aware of; i.e., maintenance and repair history, known location of utility systems, panel boxes, construction sequencing, allowable impacts to staff and visitor activities, etc…

2. A contractor staging area will be determined and discussed during site visit. During construction, Contractor will have limited use of Visitor Center, parking area and adjacent grounds. Contractor's use of premises is limited by Government’s need to perform work and the accommodation of additional contractors performing contract work within the Park.

3. With the exception of inclement weather, Acts of God, and conditions experienced or events planned by the Park which would prevent the Contractor from performing, it is expected the Contractor shall be on the site performing each work day. Delays due to other project schedules will not be acceptable. If it becomes necessary for the Contractor to interrupt work activities, the Contractor shall submit a written request to the Contracting Officer at least five (5) working days in advance. Contractor must provide notification the day Contractor moves back to the work site after an interruption of work of two or more work days.

G. Public Access: The Contractor shall at all times conduct his operations to ensure the least inconvenience to, and heightened safety awareness for, Park employees and the public. The building will be occupied by Park employees and open to the public during construction.

Contractor is to provide adequate barriers in work areas to protect persons from injury from construction operations. The Contractor shall submit written requests for street closings to the COR for approval at least ten (10) work days prior to the requested street closing date.

H. Access to Sites: The sites for contract performance do have special access requirements and require coordination with the COR. Contractor shall have limited use of the site for construction operations. Limit use of the premises to the area(s) where work is to be performed. Do not disturb portions of project site beyond area in which the work is indicated.

I. Full Government Occupancy Requirement for Building: Government will occupy building during the entire contract period. Cooperate with Government during construction operations to minimize conflicts and facilitate Government’s normal usage and activity.

Perform the work so as not to interfere with the Government’s day-to-day operations.

1. Do not close or obstruct walkways, corridors, sidewalks, streets, roads and other adjacent occupied or used facilities. Closures to sidewalks, streets, roads, or other access means will only be permitted when required and only upon written approval by the CO, and when proper directional signage and barriers are provided by the Contractor.

2. Keep driveways and entrances serving premises clear and available to Government, Government’s employees, Park visitors, and emergency vehicles at all time. Do not use these areas for storage of materials.

J. Temporary Construction: Contractor to provide temporary weatherproofing of all exposed roof surfaces during Contractor’s non-work hours as well as during inclement weather conditions. Any damage to the existing structure due to water infiltration from inadequate temporary weatherproofing will be contractor’s responsibility to repair.

K. Dust Control / Noise from Operations: Contractors shall be responsible to pick up, clear and/or sweep away any mud or debris from roadways left by heavy equipment or trucks.

Contractors shall use whatever means necessary to ensure equipment and trucks do not leave mud and debris on the roadways, to the satisfaction of the COR. Noisy operations such as those involving the use of air hammers, etc., demolition, or cutting of openings shall be scheduled with the COR. Protect the occupied areas from dust, smoke, etc., by a method reviewed and accepted by the COR.

L. Preservation of Structures: Every precaution shall be taken to prevent damage to the Visitor

Center, and the surrounding structures and areas. Should any damage be incurred as a result of the Contractor’s operations, it will be repaired and/or the damaged materials replaced, in kind, by the Contractor, to the satisfaction of the Contracting Officer at no additional expense to the Government. Any materials used to repair or replace damaged areas are to be approved by the COR prior to performing the repair or replacement.

M. Preservation of Natural Features: Confine all operations within the work limits of the project. Exercise special care to leave the natural surroundings undisturbed and undamaged.

Restore disturbed and damaged areas within one week of the disturbance/damage, repairing or replacing damaged lawns, trees, plants, and other vegetation and natural features at no additional expense to the Government. Contractor will be responsible for all costs associated with restoration of the project area.

N. Equipment Restrictions: Use only equipment that can safely negotiate work area.

O. Storage Restrictions: Any storage required for the Contractor’s operation shall be the responsibility of the Contractor. Contractor will be authorized to have one storage trailer at the construction site at a location as approved by the COR. All other trailers, storage, and equipment will be stored at a site designated by the COR. The Government is not responsible for damages or losses.

1. Contractor must provide suitable storage for all combustible and flammable materials.

Type and location of storage to be approved, in advance, by the COR.

2. Contractor is responsible for general maintenance of construction site and storage area(s) on a daily basis. This includes, but is not limited to, cleaning, safety and security checks, and disposal of trash and debris.

3. Schedule deliveries to minimize space and time requirements for storage of materials and equipment on-site.

P. Office Space: Office space which may be required for Contractor’s operation shall be the responsibility of the Contractor. Any vehicle or trailer to be utilized by Contractor at the Park for office space purposes must first receive approval by the COR. If approved by the COR, the

Contractor must comply with the conditions COR has established for granting the approval.

The Government will not furnish Contractor an interior location for office space.

Q. Hauling Restrictions: Comply with all legal load restrictions in the hauling of materials. Load restrictions on park roads are identical to the state load restrictions with such additional regulations as may be imposed by the Park Superintendent. A special permit will not relieve Contractor of liability for damage which may result from moving of equipment.

R. Disposal and Recycle:

1. Unless otherwise specified by the COR, or elsewhere in this specifications document, all removed material and equipment that is not reinstalled under this contract, including debris generated by this project, shall be removed at regular intervals from the site and disposed of offsite by the Contractor. Disposal of materials shall be in conformance with applicable Federal, state, and local environmental regulations. All recyclable construction debris/scrap/materials will be turned in to a recycling center when possible. Contractor is to provide the Government with documentation from the disposal site and recycling center identifying the type and weight/volume of the materials accepted from the Contractor.

2. Recycle all ferrous and non-ferrous metals from the project. The Contractor shall also recycle general administrative refuse associated with this project. This refuse includes cardboard, plastic bottles, plastic soda bottles, aluminum cans, and mixed paper.

S. Park and Safety Regulations: Comply with all applicable OSHA, National Park Service regulations and directives, and Pea Ridge National Military Park regulations. These are available online at:

T. Safety Briefing: Primary contractors will brief subcontractors, at a minimum, on safety, occupational health, fire prevention regulations, and the requirement to obtain a welding permit, when welding, cutting and brazing operations are required. Maintain documentation that all subcontractors have been briefed.

U. Force Protection/Anti-Terrorism Standards: Contractor to adhere to all park policies and procedures regarding Force Protection/Anti-Terrorism Standards. This includes, but not limited to securing vehicles, tools and materials as well as access to facilities with keys, door codes, etc. Under no circumstances will contractors be allowed to escort unauthorized personnel without written notification from the COR. Contractor is to provide a list of names of all contractor’s employees and sub-contractors to COR who will be on the job site.

V. Fire Prevention and Protection:

1. A capable and qualified person shall be placed in charge of fire protection. The responsibilities shall, at a minimum, include locating and maintaining fire protective equipment provided by Contractor.

2. Hazard Control: Take all necessary precautions to prevent fire during construction. Do not store flammable or combustible liquids in historic structures or existing buildings.

Provide adequate ventilation during use of volatile or noxious substance.

3. Locate internal combustion equipment so that exhausts discharge well away from combustible materials.

4. Welding: Welding or cutting by torch will not be permitted in or on the structure unless approved in advance by the COR. Obtain permit prior to conducting any operation that will create a spark or flame. If an open burn permit is required, the permit request shall be routed PRIOR to actual open burn efforts. The Government shall be notified before starting welding or cutting. Fire extinguishers shall be immediately accessible when welding or cutting with an open flame or arc. Welding or cutting with an open flame or arc shall be stopped not less than one hour before leaving the premises.

W. Enabling/Disabling Fire Alarm Panels: If in the performance of the contract, the enabling/disabling of fire alarm panels is required, the Contractor shall bear all costs associated with their enabling/disabling. All fire alarm panels shall be properly operating prior to the contractor securing at the end of each workday. To have a fire alarm panel enabled/disabled the Contractor should first contact the COR.

X. Nonsmoking Building: Smoking is not permitted within the building or within 25 feet of entrances, operable windows, or outdoor air intakes. Smoking shall only be allowed in smoking areas as designated by the Park.

Y. Public Utilities

1. The Contractor is permitted the use of the utility services such as electric, gas, sewer, and water in the performance of this contract at no charge to the Contractor. Should Contractor require utilities for operation of storage or office trailer, Contractor must first receive written approval by the COR. Such temporary connections must be separately metered, are the responsibility of the Contractor, and at the Contractor’s expense. Contractor is also responsible for the disconnection and removal of temporary service lines. When making a temporary connection for water, backflow devices will be used.

2. If the relocation of existing and/or new construction of commercial utilities is required by the contract, to include but not limited to cable, gas, telephone, electrical, etc., Contractor will be responsible for the coordination with the appropriate utility companies, acquiring the required permits, and completion of their relocation and/or installation.

3. Contractor shall coordinate with COR prior to conducting any work on or near any utility lines, above or below ground. Notify COR not less than two (2) working days in advance of proposed utility interruptions. Do not proceed with utility interruptions without COR’s written approval. If damage occurs, repair utility at no additional cost to Government.

4. If refuse will be generated by Contractor in the performance of this contract; the

Contractor will be responsible for providing its own dumpster. The dumpster size and staging location shall be approved and coordinated with the (COR). Contractor must not leave refuse on the ground or around the dumpster should it reach or be at full capacity.

5. Contractor will comply with applicable Federal, state and local regulations and codes regarding utilities operations and services in the performance of this contract.

Z. Permits: Contractor is required to investigate, submit for any permits and the marking of utilities which may be required in the performance of the work. All permit applications shall be submitted to the COR.

AA. Security:

1. The security of Contractor’s materials, tools, equipment and other properties remain the responsibility of the Contractor at all times. The Park will accept no responsibility and shall not be liable for Contractor’s properties left on site should they become lost, damaged or stolen at any time during the contract period.

2. Contractor shall comply with all security requirements as imposed by the Park, which may include but not limited to, Contractor’s and subcontractor’s personnel, drivers and vehicles being subject to checks for valid registration and wants/warrants. Individuals who do not pass the security check will not be authorized access to the site.

BB. Environmental Management System: Contractor will reduce environmental risks, promote pollution prevention, and meet all environmental regulatory requirements during the execution and completion of the contract.

CC. Patching and Repairing: After the work has been completed, any Contractor-caused damages to Government-owned structures, facilities, streets, curbs, walks, lawns, etc., shall be properly patched, repaired, or replaced to the complete satisfaction of the Contracting Officer. Any holes, cuts, breakages, and damages made in finished surfaces of the area in which work has been accomplished, shall be properly repaired and the disfigured areas then painted over or otherwise repaired and resurfaced to match the existing finishes to the complete satisfaction of the COR.

DD. Final Cleanup: Upon completion of the project, all Contractor’s equipment and construction debris will be promptly removed from the site and left in an acceptable condition to the satisfaction of the COR.

EE. Inspection: Obtain the COR’s approval prior to accomplishing tasks such as closing walls, ceilings, floors, filling in trenches, or performing tests. The request for such approval shall be

given with sufficient advance notice to permit an inspection of the area before the work is covered or the test is accomplished.

FF. Warranty Items:

1. In addition to any other warranties in this contract, the Contractor warrants, that work performed under this contract conforms to the contract requirements and is free of any defect in equipment, material, or design furnished, or workmanship performed by the Contractor or any subcontractor or supplier at any tier.

2. This warranty shall continue for a period of one (1) year from the date of final acceptance of the work. If the Government takes possession of any part of the work before final acceptance, this warranty shall continue for a period of one (1) year from the date the Government takes possession.

3. The Contractor shall remedy at the Contractor’s expense any failure to conform, or any defect. In addition, the Contractor shall remedy at the Contractor’s expense any damage to Government-owned or controlled real or personal property, when that damage is the result of:

a. The Contractor’s failure to conform to contract requirements; or

b. Any defect of equipment, material, workmanship, or design furnished.

4. The Contracting Officer (CO) shall notify the Contractor, in writing, within a reasonable time after the discovery of any failure, defect, or damage. There shall be a maximum 24-hour response time by the Contractor on all warranty items installed under this contract and identified as either malfunctioning or inoperative after acceptance of the item. The Contractor shall notify both the CO and the COR of the date, place, and time that the contractor will be on location to determine the cause of the problem.

Response time shall be defined as the time from when the CO contacts the Contractor until the time the Contractor is present at the location of the equipment with the appropriate personnel to determine the problem and what is necessary to correct the problem. Warranty items shall be defined as all equipment, components, appurtenances, and wiring installed and the labor and materials necessary to install said items.

5. If the Contractor fails to remedy any failure, defect, or damage within five (5) working days after receipt of notice, the Government shall have the right to replace, repair, or otherwise remedy the failure, defect, or damage at the Contractor’s expense.

1.5 PRODUCTS

A. Products Not Allowed: Materials containing asbestos, transformers containing PCB fluid, and the use of lead based paints or paints containing chromates, as well as other known hazards, unless otherwise approved by the COR, are prohibited.

B. Substitution of Materials and Equipment: The Contracting Officer reserves the right to reject or accept any or all substitutions proposed by Contractor, based on completeness, clarity, PERI – PMIS 242009 011000-10 and other requirements of the plans and specifications. When the Contractor requests approval of substitute materials and/or equipment, it shall be understood and agreed by the Contractor that such substitution, if approved, will be made without cost to the Government, regardless of the reason, rationale, or cause for the substitution.

C. Brand Names: References in specifications to any brand name materials or equipment shall be regarded as establishing a standard of quality and shall not be construed as limiting competition. The Contractor may submit proposed equal material and/or equipment for the Government’s consideration during the post award materials submittal process.

SECTION 013100

PROJECT MEETINGS

1.1 PRECONSTRUCTION CONFERENCE

A. Before start of construction, the Contracting Officer will arrange an on-site meeting with the

Park and Contractor which is referred to as the Preconstruction Conference. After award of contract, the Contracting Officer will notify Contractor of the tentative date for the meeting.

Attendance by the Contractor is mandatory. At the discretion of the Contracting Officer, the meeting may also include the Contractor’s subcontractors, and other Park personnel who have an interest or are impacted by the project. The meeting agenda will be prepared by the Contracting Officer and distributed at the Preconstruction Conference.

B. Within ten (10) calendar days after award of the contract, and prior to the Preconstruction

Conference, the Contractor must provide and submit to the Contracting Officer for the Government’s review, the documents and items listed below:

1. Certificates of Insurance in the limits and as required by the terms of the contract.

2. Payment and Performance Bonds as required by the terms of the contract.

3. Comprehensive list of submittals that will require the Government’s approval prior to and throughout contract performance.

4. Proposed Construction Progress Schedule, utilizing June 11, 2018 as the tentative baseline date the contractor receives the Notice to Proceed. Contractor is to commence work within ten (10) calendar days from the receipt date of the Notice to Proceed.

5. Contractor to submit a Schedule of Values.

6. Letter designating the Key Personnel for the positions Contractor identified in

Attachment 6 – Technical & Financial Capability Statement, Item 5. (For example, Site Superintendent and, if applicable, Alternate Site Superintendent; Project Manager, Safety Officer, Quality Control Supervisor, etc.). As determined appropriate by the Contractor, responsibilities may be combined and performed by one or more qualified individuals. At a minimum, provide a 24-hour number or numbers where the designated Contractor and Site Superintendent can be contacted at any time should an emergency or urgent matter arise.

7. Referencing the contract’s DBA Wage Determination, Attachment 4, provide a list of the position classifications to be used in the performance of this contract, to include those positions utilized by subcontractors.

8. List of subcontractors, if any, being used for this project.

9. Written statements from subcontractors certifying compliance with applicable labor standard clauses. (SF Form 1413 – Statement and Acknowledgment)

10. Accident Prevention Program / Safety Plan.

11. Waste/Recycling Plan.

C. Should the above documents not be received by the Contracting Officer within ten (10) calendar days after contract award, the Pre-Construction Conference will not be scheduled, Notice to Proceed will not be issued, and the Contracting Officer will consider other contractual remedies. No work shall commence under this contract until the Contractor has been issued the written Notice to Proceed by the Contracting Officer.

1.2 PROGRESS MEETINGS

A. Contracting Officer will schedule progress meetings with Contractor throughout the duration of the contract. Included in the meeting will be those individuals appropriate to the agenda topics. These may be held at the contract site or telephonically as determined by the Contracting Officer.

B. Topics to be discussed during the progress meetings may include, but not be limited to submittals, delivery schedules, quality control, requests for information, work in progress, schedule updates, safety, and other business relating to work under the contract.

SECTION 013300

SUBMITTALS

1.1 DESCRIPTION. The work of this section consists of submittal requirements before and during construction and at Project Closeout.

1.2 DEFINITIONS

A. Action Submittals: Written, graphic information, and physical samples that require

Government’s responsive approval action. Submittals may be rejected for not complying with requirements.

B. Information Submittals: Written information that does not require Government’s responsive action; but does require the Government’s acceptance. Submittals may be rejected for not complying with requirements.

1.3 RELATED REQUIREMENTS

A. All Sections of this Document

B. Contractor is required to complete the Daily Log of Construction Form (a copy of which is located at the end of this section) and submit it to the Contracting Officer’s Representative daily, commencing on the date the Contractor is in receipt of the Notice to Proceed.

Additional instructions on Contractor’s submission of daily logs shall be provided at the preconstruction conference. A transmittal form is not needed in the submission of the daily logs.

1.4 SUBMITTAL AND APPROVAL PROCEDURES

A. All submittals shall be transmitted using National Park Service form CM-16 dated R03/10 (a copy of the form is located at the end of this section). No action will be taken on a submittal item unless accompanied by this form.

B. The Contracting Officer reserves the right to require submittals in addition to those called for in the contract documents and which are identified in the Contractor’s Comprehensive Summary of Submittals. No materials will be installed or used until submittals have been approved and signed by the COR and CO.

C. Forward submittals to the Contracting Officer at least fourteen (14) calendar days before need for approval (action submittals) or acceptance (informational submittals). Unless a different number is specified, electronically (via e-mail) submit one (1) copy of each shop drawing, manufacturer’s catalog sheets (cut sheets), and one (1) copies of all other submittals required and requested for this project. Specimen samples are to be delivered to the COR with the accompanying transmittal form submitted to the CO. At the completion of the project and as part of the final closeout, Contractor shall also be required to submit two

(2) hard-copies, and digital files on two (2) CDs or USB flash drives, of final shop drawings, schedules, Operations and Maintenance Manual, product information, and

manufacturer/dealer warranties, Contractor’s warranties, and other such documents called for within the contract documents. Acceptable applications for digital files are .pdf, .docx, .xlsx. .jpeg files are to be used for photographs only. Prior to generating and submitting files in any other application, check with the CO for acceptability.

1. Shop Drawings: Include the following information with each copy of shop drawings:

a. Date

b. Date of revisions (when applicable)

c. Contractor’s certification that shop drawing has been checked for compliance with contract documents.

d. Details of fabrication, assembly and erection including connections and engagement to contiguous work.

e. Materials used.

f. All required dimensions.

g. The term “by others” shall not be used. All work to be performed by others shall be identified by Contractor or subcontractor name, discipline, or trade.

2. Samples: Samples shall be large enough to illustrate clearly the functional characteristics and full range of color, texture, pattern, or grade.

3. Manufacturers’ Catalog Sheets (Cut Sheets): Submit only pertinent pages; mark each copy of standard printed data to identify specific products proposed for use.

4. Manufacturers’ Installation / Application Instructions: When contract documents require compliance with manufacturers’ instructions, and should such instructions provide options for installation and/or application, indicate on the instructions which option is being recommended for utilization.

5. Construction Materials: The Contractor is encouraged to submit for approval products made out of recycled or environmentally responsible material. Every effort will be made by the National Park Service to approve these materials.

D. Coordinate preparation and processing of submittals with performance of construction activities. Review them for legibility, accuracy, completeness, and compliance with contract requirements. Forward submittals that are related to or affect one another as a package to facilitate coordinated review and so as not to delay contract performance. The Contracting Officer reserves the right to withhold action on a submittal requiring coordination with other submittals until related submittals are received. If the contractor intends to supply materials that differ from the salient characteristics which are specified in the contract documents, then a submittal will be required.

E. Submittal Identification

1. All sets of drawings, manufacturer’s catalog sheets, samples, and other documents submitted to the Contracting Officer must have the Park Name, Contract Number, and Submittal Number applied to each.

2. This identification information shall be applied to the bottom right margin on each document. If there are multiple pages to a document, the identification information need only be printed on the first page or cover sheet.

3. Identification information on samples shall be applied to the most readily visible area on the sample or on a label or tag affixed to the sample.

F. Submittal Numbering

1. Number each submittal consecutively.

2. For re-submittals use the original submittal number, plus a letter suffix, beginning with the alpha character “A”.

3. Additional re-submittals of the same item shall contain the original number with the next consecutive letter; e.g., “B”, “C”, “D”, etc.

G. Transmittal Numbering. Number each transmittal consecutively. No two transmittals shall have the same number. The transmittal number is entered in the top right block on the Transmittal CM-16 Form.

H. Contracting Officer’s Review

1. Submittals will be returned disapproved without technical review if identification information is missing, not filled in, or if placed on the back of the submittal; an incorrect number or format of submittals is provided; the transmittal form is incorrectly filled out; submittals are not coordinated; or submittals do not show evidence of Contractor’s approval.

2. Any work done or orders for materials or services placed before approval shall be at the

Contractor’s own risk.

3. After reviewing submittals, the Contracting Officer will return one copy of the

Transmittal CM-16 Form and one copy of applicable (marked up) submittal sheets to the Contractor. Shop drawing review notations will be returned on the reproducible original shop drawing. All other submitted items will be retained. The Contractor is responsible for producing additional copies for his/her own use.

4. The returned Transmittal will be marked in one of three ways as defined below:

a. APPROVED: Acceptable with no corrections.

b. APPROVED WITH NOTATIONS: Minor corrections or clarifications required. All comments are clear and no further review is required. The Contractor shall address all review comments when proceeding with the work.

c. DISAPPROVED - RESUBMIT: Rejected as not in accordance with the contract or as requiring major corrections or clarifications. The Contracting Officer will identify the reasons for disapproval. The Contractor shall revise and resubmit with the changes

clearly identified within three (3) business days from Contractor’s receipt of the Returned Transmittal. A greater number of days may be granted by the Contracting Officer if the resubmittal is of such a complexity, or the period of time for the Contractor to gather the information for the re-submittal, warrants the extension.

The Contracting Officer shall then review and return the submittal to the Contractor within five (5) business days from the date it is received from the Contractor.

5. Use for Construction: Use only final submittals with mark indicating “Approved” or

“Approved with Notations.” Ensure all notations have been incorporated and, at a minimum, keep one copy of the final approved submittal on site for use during construction.

SECTION 013600

ACCIDENT PREVENTION

1.1 DESCRIPTION. The work of this section consists of establishing an effective accident prevention program and providing a safe environment for all personnel and visitors.

1.2 SUBMITTALS

A. Accident Prevention Program: After contract award and before the Preconstruction

Conference, submit for approval an Accident Prevention Program / Safety Plan. The Contracting Officer’s Representative will review the proposed program for compliance with OSHA and project requirements. If the program requires any revisions or corrections, the Contractor shall resubmit the program within two (2) business days. No work on the contract site shall be performed without an approved Accident Prevention Program/ Safety Plan. The program shall include:

1. Name of responsible supervisor to carry out the program.

2. Weekly and monthly safety meetings.

3. First aid procedures.

4. Outline the hazards associated with the work and the methods proposed to ensure property protection and safety of the public, National Park Service personnel, and Contractor’s employees.

5. Training, both initial and continuing.

6. Planning for possible emergency situations such as floods, fires, power outages, explosions, spills, and/or other emergency situations giving consideration to the nature of the construction, site conditions, and degree of exposure of persons and property.

B. Submit a copy of test reports, as required by OSHA, for personnel working with hazardous materials.

C. Submit a report of safety meetings and of inspections.

1.3 QUALITY ASURANCE

A. Comply with contract clauses entitled “Accident Prevention” and “Permits and

Responsibilities”. In case of conflicts between Federal, state, and local safety and health requirements, the most stringent shall apply. Equipment or tools not meeting OSHA requirements will not be allowed on the project sites. Failure to comply with the requirements of this section and related sections may result in suspension of work.

B. Qualification of Employees:

1. Ensure that employees are physically qualified to perform their assigned duties in a safe manner.

2. Do not allow employees to work whose ability or alertness is impaired because of drugs, fatigue, illness, intoxication, or other conditions that may expose themselves or others to injury.

3. Operators of vehicles, mobile equipment, hoisting equipment, and hazardous plant equipment shall be able to understand signs, signals, and operating instructions, and be capable of operating such equipment. Provide operating instructions for all equipment.

Newly hired operators shall be individually tested by an experienced operator or supervisor to determine if they are capable of safely operating equipment.

1.4 ACCIDENT REPORTING

A. Reportable Accidents: A reportable accident is defined as death, occupational disease, traumatic injury to employees or the public, property damage by accident in excess of $100, and fires. Notify Contracting Officer immediately in the event of a reportable accident.

Within seven (7) days of a reportable accident, fill out and forward to Contracting Officer an Accident/Property Damage Report (Form CM-22). Form may be obtained from the Contracting Officer.

B. All Other Accidents: The Contractor shall report all other accidents to the Contracting Officer and Contracting Officer’s Representative as soon as possible and assist the Contracting Officer and other officials as required in the investigation of the accident.

1.5 FIRST AID FACILITIES. Provide adequate facilities for the number of employees and the type of construction at the site.

1.6 PERSONNEL PROTECTIVE EQUIPMENT. Meet requirements of NIOSH and MSHA.

1.7 EMERGENCY INSTRUCTIONS. Post telephone numbers and reporting instructions for ambulance, physician, hospital, fire department, and police in conspicuous locations at the work site.

1.8 FIRE AND LIFE SAFETY

A. Provide and maintain the fire and life safety requirements in NFPA 241 (Standard for

Safeguarding Construction, Alteration, and Demolition Operations).

B. Contractor shall have a Hazard Communications Plan; store hazardous materials in accordance with manufacturer’s and OSHA recommendations; immediately report all spills of hazardous materials to the Park; and maintain a spill emergency response kit.

1.9 PROTECTIVE EQUIPMENT

A. Inspect personal protective equipment daily and maintain in a serviceable condition. Clean, sanitize, and repair, as appropriate, personal items before issuing them to another individual.

B. Inspect and maintain other protective equipment and devices before use and on a periodic basis to ensure safe operation.

1.10 SAFETY MEETINGS

A. As a minimum, conduct weekly 15-minute “toolbox” safety meetings. These meetings shall be conducted by a foreman and attended by all construction personnel at the work site.

These meetings shall be used to review the effectiveness of the Contractor’s safety effort, to resolve current health and safety problems, to provide a forum for planning safe construction activities, and for updating the Accident Prevention Program.

B. At the discretion of the Contracting Officer or Contracting Officer’s Representative, the CO or

COR may attend the meeting.

1.11 HARD HATS AND PROTECTIVE EQUIPMENT AREAS

A. A hard hat area will be designated by the Contracting Officer. The hard hat area shall be posted by the Contractor in a manner satisfactory to the Contracting Officer.

B. It is the Contractor’s responsibility to require all those working on or visiting the site to wear hard hats and other necessary protective equipment at all times. As a minimum, provide hard hats for use by visitors. Change liners before reissuing hats.

1.12 TRAINING

A. First Aid: Provide adequate training to ensure prompt and efficient first aid.

B. Hazardous Material: Train and instruct each employee exposed to hazardous material in safe and approved methods of handling and storage. Hazardous materials are defined as explosive, flammable, poisonous, corrosive, oxidizing, irritating, or otherwise harmful substances that could cause death or injury.

SECTION 014300

CONTRACTOR QUALITY CONTROL

1.1 DESCRIPTION. The work of this section consists of preparing and executing a Quality Control

Program.

1.2 RELATED REQUIREMENTS

A. “Inspection of Construction” clause of this contract.

B. Contractor’s completion and submission of the Daily Log of Construction Form as described in

Section 013300.

1.3 SUBMITTALS. A written Quality Control Plan is not required to be submitted.

1.4 RESPONSIBILITIES

A. The quality of all work shall be the responsibility of the Contractor.

B. The Contractor must have a designated and qualified Quality Control Supervisor on the project site whenever contract work is in progress. The Quality Control Supervisor may also perform the duties of Site Superintendent, and/or Project Manager, and/or Safety Officer.

C. Inspect and test all work at various stages of construction to ensure that the quality of materials, workmanship, construction, finish, and functional performance is in compliance with applicable specifications and drawings, and are in strict accordance with the latest trade practices, standards, and Codes.

1.5 OFF-SITE CONTROL. Items that are fabricated or assembled off-site shall be inspected for quality control at the place of fabrication. Inspections conducted, tests performed, and their results, must be documented on the Daily Log of Construction Form.
