STLI – Rehabilitate Ellis Island Service Bridge and Ellis Island Paving
PMIS 213901
AMENDMENT 004
1. Please reconsider the response to Question #2 issued in Addendum #3 and allow the United States of America to be listed as an additional insured on Contractors insurance policies. The United States of America will be protected by being listed as an additional insured on the policy and by the indemnity language in the Contract. Being listed as a Named Insured on the insurance policy means you are the entity purchasing and paying the premium of the policy. Our experience with all government agencies we have completed work for in the past allow for the government entity to be listed as an additional insured on the policy. As a standard insurance policy will not allow another entity to be listed as a named insured, Contractors would need to purchase project specific insurance policies for this specific project. Project specific insurance is an onerous requirement for a project of this size and would add a significant cost to the project.
NPS Response: The requirement is as written in the solicitation.
2. Please refer to Drawings C6, C11 and C13. Please provide the elevation of the tie in for the drain line at existing cleanout.
NPS Response: All known elevations are marked out in the survey drawings. Consult sheet C3 for elevation of that drain.
3. Please confirm that the National Parks Service will be considered the generator of any existing hazardous materials encountered during construction.
NPS Response: The NPS will be considered the generator of any hazardous materials encountered during construction.
4. Please clarify how fields 17 and 18 are to be completed on Standard Form 1442. In field 17, we have seen “please see price schedule” inserted to the right of the amounts field.
NPS Response: Offeror may response to field 17 “See price schedule”. Field 18 does not require a response.
5. A snapshot of sheet C3 is provided to clarify elevations.
