About this file

This is a Domestic Partnership Act Certification form issued by Broward County, Florida, for vendors responding to a solicitation for FEMA-compliant Pipe and Valve Maintenance & Installation Services. The certification requirement applies to all vendors bidding on contracts valued over $100,000 and must be completed and returned at the time of solicitation submittal. Vendors failing to submit the form by the stated deadline may be deemed nonresponsive to the solicitation or ineligible for the Domestic Partnership tiebreaker provision. The form requires vendors to certify their compliance status with Broward County Code of Ordinances Section 16-157, which mandates that vendors provide benefits to registered domestic partners of employees on the same basis as benefits provided to employees' spouses.

Vendors must select one of four primary certification options: current compliance with the Domestic Partnership Act, commitment to comply at the time of contract award, non-compliance, or eligibility for one of six enumerated exceptions. Exceptions are available for vendors employing fewer than five employees, those not providing spousal benefits, governmental entities, non-profit and charitable organizations, religious organizations, vendors providing cash equivalents of benefits, and vendors whose compliance would violate federal or state law or grant conditions. The form requires vendor name, authorized signature, printed name, title, and date, with the form dated September 9, 2024.
